Pathogenic mechanism, prophylaxis, and therapy of symptomatic acidosis induced by acetazolamide.

BACKGROUND: Acetazolamide, a noncompetitive carbonic anhydrase inhibitor, can produce symptomatic acidosis and bone marrow suppression by a mechanism that is still unknown. This presentation occurs in the elderly, patients with renal or liver failure, people with diabetes, and newborns. The objective of this study was to understand the pathogenic mechanism of these adverse effects and to propose a possible prophylaxis and therapy. METHODS: Four human clinical cases were studied, and one animal experiment was performed. Four preterm newborns with posthemorrhagic ventricular dilation developed …

Is acetazolamide effective in the treatment of diabetic macular edema? A pilot study.

AIM: To investigate whether acetazolamide, already found to be helpful in decreasing cystoid macular edema in patients with retinitis pigmentosa, was also effective in the treatment of diabetic macular edema in nonproliferative retinopathy. METHODS: Two randomized age- and sex-matched groups (cases and controls) of 12 diabetics (five Type 1 and seven Type 2) were selected for this pilot study and graded for retinopathy (Early Treatment of Diabetic Retinopathy Study-Airlie House Classification). Cases were treated with acetazolamide for three months according to a standard protocol. The Early …

Impairment of cerebrovascular reactivity to acetazolamide in patients with normal pressure hydrocephalus.

Cerebrovascular reactivity (CVR) to acetazolamide was investigated in 41 patients with normal pressure hydrocephalus (NPH). The aetiology was subarachnoid haemorrhage in 20 patients, trauma in nine, brain tumour in three and idiopathic in nine. Mean cerebral blood flow (CBF) of the whole brain was measured by performing first-pass radionuclide angiography using 99Tcm-hexamethylpropylene++ amine oxime. Cerebrovascular reactivity was measured as the percentage change from the baseline mean CBF value after the administration of 500 mg of acetazolamide. Cerebrovascular reactivity was significantly…

[Salicylism and glaucoma: reciprocal augmentation of the toxicity of acetazolamide and acetylsalicylic acid].

We report a case of salicylism caused by association of acetylsalicylic acid (ASA) and acetazolamide. A 50-year-old woman without chronic renal insufficiency, was admitted for confusion, cerebellar ataxia associated with hyperchloremic acidosis. She was treated with acetazolamide for simple glaucoma and since 14 days with ASA for acute pericarditis. Clinical features evoked salicylism despite low serum salicylate level: 178 mg/l. The carbonic anhydrase inhibitor explained the biochemical pattern. Acetazolamide-induced acidosis enhances the non ionized form of salicylate which crosses the biolo…

Clinical efficacy and indication of acetazolamide treatment on sleep apnea syndrome.

The efficacy and indication of acetazolamide treatment on patients with sleep apnea syndrome (SAS) were discussed from assessing the changes of polysomnographic findings with the treatment in 75 SAS patients. For the patients as a whole, respiratory disorder variables improved significantly during the treatment. However, the number of acetazolamide treatment responders who showed a decrease of apnea hypopnea index (AHI) to 50% or less of the pretreatment value numbered only 34 (45.3%). The lower values of body mass index and AHI in the responder group indicated that monotherapy with acetazolam…

Improvement of muscle strength in familial hypokalaemic periodic paralysis with acetazolamide.

A double blind cross-over study of eight patients with familial hypokalaemic periodic paralysis was made to assess the influence of acetazolamide on muscle strength. All patients had a reduced interictal muscle fibre conduction velocity. Five patients had no attacks at the time of the study. One patient withdrew from the study because of an adverse reaction. The muscle strength of 11 muscle groups was measured with a hand-held dynamometer. The sum of force improved significantly in the seven patients (mean increase: 17%, p less than 0.05; 95% confidence interval: 7.2-26.8%). Endurance tests sh…

Effects of acetazolamide on cerebral blood flow and brain tissue oxygenation.

Oral administration of 1 g of acetazolamide to 8 normal subjects studied at sea level and in normoxia caused an acute increase in cerebral blood flow (CBF). During the subsequent prolonged oral treatment with 1 g of acetazolamide daily, CBF returned to normal within 2 days. The alveolar CO2 tension decreased gradually to 70% of the control value, indicating hyperventilation. At sea level hyperventilation will not increase brain oxygenation significantly in normal man, as the arterial oxygen content only increases minimally, while CBF is unchanged. At high altitude the beneficial effects of ace…

Acetazolamide teratology and its association with carbonic anhydrase inhibition in the mouse.

Acetazolamide, an inhibitor of the enzyme carbonic anhydrase (E.C. 4.2.1.1.), causes a unique congenital anomaly characterized by postaxial reduction of the distal portion of the right forelimb. To gain an understanding of the mechanism of teratogenesis, the activity of carbonic anhydrase in sensitive and resistant mouse strains, and its inhibition by acetazolamide, were examined. Differences in teratologic sensitivity were found not to be attributable to differences in maternal or embryonic drug levels. Enzyme inhibition at acetazolamide concentrations ranging between 10(-11) and 10(-5) M did…

Determination of acetazolamide in biological fluids by high-performance liquid chromatography.

A high-performance liquid chromatographic (HPLC) assay for acetazolamide is presented. A 100-microliter sample is mixed with an aliquot of the internal standard solution and the mixture, buffered at pH 4.5, is extracted with ethyl acetate. The extract is evaporated to dryness and the residue is analyzed by HPLC, using a reverse-phase octadecylsilane column. The wavelength of the detection is 254 nm. The coefficient of variation (CV) in the within-day analysis of replicate 10-microgram/ml acetazolamide samples in human blood plasma was 6.5%, while the between-day CV was 7.1%. The procedures was…

Effect of acetazolamide in blood acid-base and electrolyte values in dogs.

The effects of acetazolamide administration on arterial blood acid-base equilibrium and electrolyte concentrations were evaluated in 13 clinical patients and in a 4-week experiment in 6 conditioned mixed-breed dogs. Findings included persistent acidemia characterized by hyperchloremic metabolic acidosis, elevated partial pressure of oxygen, and mild depletion of potassium. Changes in the partial pressure of carbon dioxide were variable, and there was no change in plasma sodium concentration or osmolality. Measured disturbances were apparent within 12 hours of the commencement of acetazolamide …

Electron microscopic study of the parathyroid gland of the acetazolamide-treated mouse.

Ultrastructural aspects of the parathyroid glands of acetazolamide-treated mice were examined. Many chief cells of the treated mice are deficient in free ribosomes compared with the control mice. Most Golgi complexes are poorly developed and associated with a few prosecretory granules. However, relatively numerous secretory granules are observed in the cytoplasm, and a few secretory granules are present in the peripheral cytoplasm. Large secretory granules (storage granules), homogeneously dense bodies with a low electron density, heterogeneously dense bodies containing osmiophilic substance a…

Effect of acetazolamide on rate of CO2 uptake in the perfused rat brain.

The accumulation of acid-labile CO2 was measured in the rat brain at early times after exposure to 30% CO2 in oxygen. In particular, the effect on CO2 accumulation of inhibiting brain carbonic anhydrase with acetazolamide was studied. For 57 rats, a rat head perfusion technique was used which permits control and rapid alteration of arterial acid-base conditions. At a paCO2 of 35 mm Hg, total carbon dioxide of the whole brain remained at a normal mean value of 13.4 +/- 0.6 (S.E.) mM/kg during perfusions 209-160 min long. On 30% CO2, brain CO2 increased rapidly to a mean of 24 mM/kg at 5 min and…
