Drug susceptibility of a clinical isolate of Balamuthia mandrillaris, a pathogenic free-living amoeba.

Balamuthia amoebic encephalitis (BAE) is a highly fatal infection caused by Balamuthia mandrillaris, an amoeba that lives in soil and water. In Thailand, three fatal cases of BAE have been documented, but no survivors have been reported, raising questions about current treatment regimens. Previous drug repurposing studies reveal some potent pharmacological compounds, but the drug susceptibility of the clinical isolate of pathogenic amoeba remains variable. Given the success in isolating B. mandrillaris from the human biopsied brain, this study aims to assess the amoebicidal effect of several p

Digitally Adapting LGBTQ-Affirmative Cognitive Behavioral Therapy for Chinese Men Who Have Sex With Men Living With HIV: User-Centered Design Approach.

BACKGROUND: Chinese men who have sex with men living with HIV (MSMLWH) experience substantial psychological distress driven by minority stress and HIV-related challenges. However, culturally tailored digital mental health interventions that address HIV-specific maladaptive cognitive schemas and culturally specific psychosocial stressors remain scarce in China. OBJECTIVE: This study aimed to systematically adapt an evidence-based cognitive behavioral therapy (CBT) intervention Effective Skills to Empower Effective Men (ESTEEM) into a WeChat (Tencent) Mini-Program-based intervention (iESTEEM) s

Engaging Zimbabwean men and stakeholders in the co-adaptation of peer-delivered HIV self-testing: iterative prototyping of the IMPERATIVE Trial.

INTRODUCTION: HIV testing and pre-exposure prophylaxis (PrEP) are efficacious HIV prevention strategies, but uptake remains low among Sub-Saharan African men. Peer-delivered approaches may improve engagement. We developed an intervention combining peer-delivered oral HIV self-testing (HIVST) with incentivized peer referral to HIV services and an SMS-based HIV risk assessment among men in eastern Zimbabwe (IMPERATIVE Trial: NCT06370923 ). We co-adapted the intervention through iterative prototyping (IP) to enhance its acceptability, feasibility, and potential effectiveness. METHODS: From Novem

Bisphosphonates in orthopedic applications.

Bisphosphonates (BPs) exert potent effects on the skeleton. As such, there are important questions relating to how treatment with BPs for metabolic disorders might affect outcomes of orthopedic problems. A further question is what role, if any, might BPs play as adjunctive therapeutics for orthopedic problems. This article outlines the research thus far in the application of BPs to the management of osteonecrosis, bone repair, and joint arthroplasty. Many animal studies show a benefit to decreasing bone resorption in models of osteonecrosis. These include studies in both small and large animal

Changes in Quality of Life Among Glaucoma Patients Following Six Months of Niacinamide Supplementation.

Background/Objectives: Glaucoma is the primary cause of irreversible blindness worldwide, with enormous impact on quality of life and activities of daily living. Since one pathogenic mechanism of glaucoma is mitochondrial dysfunction at the retinal ganglion cell level, niacin has been proposed as an adjuvant treatment, with encouraging results. The objective of this prospective, non-randomized, single-arm clinical trial was to investigate the effect of oral supplementation with niacin on the quality of life of a cohort of glaucoma patients in Romania. Methods: Fifty-eight patients diagnosed wi

Community-Based Adaptation and Evaluation of a Peer-Led Intervention to Address Alcohol Use and HIV in Pregnant and Breastfeeding Women in South Africa: Protocol for the "Mentor Mothers Plus" Randomized Control Trial.

BACKGROUND: In South Africa, pregnant and lactating women (PLW) face a dual burden of high alcohol use and HIV prevalence, both of which adversely affect maternal and infant health. However, few interventions address alcohol use and HIV risk concurrently in this population. OBJECTIVE: This study seeks to adapt and pilot-test in a randomized controlled trial (RCT) the peer-led multisession intervention Mentor Mothers Plus (MM+) to integrate alcohol reduction strategies with HIV prevention and care support (in a seroneutral intervention) among PLW in Saldanha Bay, Western Cape, South Africa. Associations of patient knowledge with drug-modifiable cardiovascular risk factor control in coronary artery disease patients with and without diabetes mellitus: results from the cross-sectional KNOW-ABC study.

BACKGROUND: Control of major drug-modifiable risk factors for glycated hemoglobin (HbA1c), blood pressure (BP), and low-density lipoprotein cholesterol (LDL-C) remains unsatisfactory in the secondary prevention of coronary artery disease (CAD). We aimed to analyze patient knowledge and attainment of LDL-C, BP, and HbA1c treatment goals and associated factors in German CAD patients with and without diabetes mellitus (DM). METHODS/RESULTS: A total of 204 CAD patients (68 ± 8 years; 75.0% male; 84 with DM (41.2%)) completed a questionnaire assessing their knowledge of LDL-C (< 55 mg/dL), BP (age

Stress-immune-Repigmentation niche in vitiligo: Mechanistic integration and therapeutic implications.

Vitiligo is an autoimmune depigmenting disorder characterized by selective melanocyte loss and a chronic relapsing course. Increasing evidence supports a network model in which immune activation, cellular stress responses, and microenvironmental imbalance converge to drive disease activity and repigmentation stability. Interferon-γ-driven JAK/STAT signaling induces chemokines such as CXCL10 and promotes CXCR3-dependent recruitment of cytotoxic T cells, reinforcing melanocyte-directed inflammation. In parallel, impaired Nrf2-mediated antioxidant defenses and enhanced lipid peroxidation lower me

BCG Immunotherapy in Equine Sarcoid Treatment: Mechanisms, Clinical Efficacy, and Challenges in Veterinary Oncology.

Equine sarcoids are the most common dermatological neoplasm in horses worldwide, associated with bovine papillomavirus (BPV) infection and characterized by high recurrence rates after conventional therapies. Bacillus Calmette-Guérin (BCG) immunotherapy has historically been used for sarcoid treatment, yet its role in contemporary veterinary oncology remains debated. This narrative review critically examines the immunological mechanisms, clinical efficacy, and limitations of BCG in equine sarcoid therapy, while integrating insights from comparative oncology and One Health perspectives. A system

Update on allergic fungal rhinosinusitis.

Allergic fungal rhinosinusitis (AFRS) is a unique clinical entity that falls under the broader umbrella of chronic rhinosinusitis with nasal polyps with type 2 inflammation. It is characterized by nasal polyposis, production of characteristic thick eosinophilic mucin, and expansile change of involved sinus cavities. The diagnosis is classically made using the Bent and Kuhn criteria. However, recent studies have indicated the lack of specificity of some major criteria. The need to fulfill all 5 criteria before diagnosing AFRS partially mitigates this but renders the criteria cumbersome to use,

Low-dose radiotherapy of osteoarthritis: from biological findings to clinical effects-challenges for future studies.

Osteoarthritis (OA) is one of the most common and socioeconomically relevant diseases, with rising incidence and prevalence especially with regard to an ageing population in the Western world. Over the decades, the scientific perception of OA has shifted from a simple degeneration of cartilage and bone to a multifactorial disease involving various cell types and immunomodulatory factors. Despite a wide range of conventional treatment modalities available, a significant proportion of patients remain treatment refractory. Low-dose radiotherapy (LDRT) has been used for decades in the treatment of

Navigating the Noise: Strategies Used by Autistic Adults to Manage Difficult Listening Situations.

PURPOSE: Listening differences, particularly difficulties listening to speech in background noise, are well documented in autism. They can significantly affect individuals' well-being, social participation, and opportunities. Autistic people report that clinical guidance on managing these difficulties is lacking, meaning that they are left to develop their own strategies through trial and error. Underlying this limited support is limited research on the topic. The present study was co-produced with the autistic community to address this crucial gap in knowledge, gathering data on the listening

Development and evaluation of a patient education programme for children, adolescents, and young adults with differences of sex development (DSD) and their parents: study protocol of Empower-DSD.

BACKGROUND: Differences in sexual development (DSD) are rare diseases, which affect the chromosomal, anatomical or gonadal sex differentiation. Although patient education is recommended as essential in a holistic care approach, standardised programmes are still lacking. The present protocol describes the aims, study design and methods of the Empower-DSD project, which developed an age-adapted multidisciplinary education programme to improve the diagnosis-specific knowledge, skills and empowerment of patients and their parents. METHODS: The new patient education programme was developed for chi

News from the "5th International Meeting on Inflammatory Bowel Diseases" CAPRI 2010.

At the "5th International Meeting on Inflammatory Bowel Diseases selected topics of inflammatory bowel disease (IBD), including the environment, genetics, the gut flora, the cell response and immunomodulation were discussed in order to better understand specific clinical and therapeutic aspects. The incidence of IBD continues to rise, both in low and in high-incidence areas. It is believed that factors associated with 'Westernization' may be conditioning the expression of these disorders. The increased incidence of IBD among migrants from low-incidence to high-incidence areas within the same g

Multicenter Qualitative Study Exploring the Patient Experience of Digital Ulcers in Systemic Sclerosis.

OBJECTIVE: Digital ulcers (DUs) are a major cause of disease-related morbidity and are a difficult-to-treat vascular complication of systemic sclerosis (SSc). Demonstrating treatment efficacy has traditionally focused on clinician assessment of DUs alone. No existing patient-reported outcome (PRO) instrument captures the multifaceted impact of SSc-DU. We report the findings of a multicenter qualitative research study exploring the patient experience of SSc-DU. METHODS: Patient focus groups were conducted across 3 scleroderma units, following a topic guide devised by SSc patients, experts, and
