[Morphological predictors of the efficacy of mitotane therapy in adrenocortical cancer].

BACKGROUND: Adrenocortical cancer (ACC) is an orphan malignant tumor of the adrenal cortex with a predominantly poor prognosis and an aggressive clinical course. Nowadays, mitotane is a non-alternative drug in the treatment of ACC. The search for prognostic parameters that determine the sensitivity of ACC to ongoing treatment is currently an urgent task. Expression levels of the large subunit of ribonucleotide reductase M1 (RRM1), cytochrome P450 2W1 (CYP2W1), and sterol- O-acyltransferase-1 (SOAT1) are considered as potential predictors of response to mitotane therapy. AIM: To assess the imm

[A clinicopathological and ultrastructural study on 147 cases of primary aldosteronism].

147 cases of primary aldosteronism was analysed, including 133 cases of adenoma (90%), 13 cases of cortical hyperplasia, and 1 case of carcinoma. The size of aldosteronoma was usually 1-2 cm in diameter. All the adenomas were well capsulated and the cut surface showed a homogeneous yellow color. The largest one in this group was 6 cm in diameter. The diameter of aldosteronoma was over 10 cm and the cut surface showed a brownish yellow color. Microscopically, the aldosteronoma was composed principally of clear cells arranged in a glomerular pattern, while the malignant one consisted of more gra

Adrenal scintigraphy with 131I-19-iodochlesterol in the diagnosis of Cushing's syndrome associated with adrenal tumor.

Seven patients with Cushing's syndrome secondary to adrenocortical tumors were studied using 131I-19-iodocholesterol. The diagnosis of all cases were verified histologically. In three cases with adenoma the uptake of the tracer was in the tumor only, while the two patients with adrenocortical carcinoma failed to show adrenal accumulation of the labelled compound. In two patients there was a hyperplasia-like scintigraphic pattern, while the stimulation and suppression biochemical tests suggested adrenal tumor. One of these cases was verified as a mixed form (adenoma plus hyperplasia), and the t

Adrenal hyperandrogenism: detection by adrenal scintigraphy.

Hyperandrogenism is a common endocrinopathy in women. Localization of the source of excess androgen production is mandatory for proper management. Ovarian hyperandrogenism has been implicated in the majority of patients studied, though the true prevalence of adrenal hyperandrogenism is not yet known. Adrenal scintigraphy is a simple non-invasive technique which detects adrenal functional abnormalities. The detection of adrenal hyperandrogenism by adrenal scintigraphy in three women with mild hirsutism is discussed.

Does tumor heterogeneity limit the use of the Weiss criteria in the evaluation of adrenocortical tumors?

Adrenal incidentalomas are detected more frequently with high-resolution imaging modalities. It is difficult to distinguish between benign and malignant lesions despite the so-called histologic Weiss criteria, imaging features, and molecular studies. We here present a 52 yr-old man who was found to have an adrenal incidentaloma during an annual check-up at his urologist. An 8 cm large adrenal lesion was detected on ultrasound, computed tomography, and magnetic resonance imaging with imaging features suggestive of malignancy. The lesion was hormonally inactive. A left-sided adrenalectomy was pe

[Extra-adrenal pheochromocytoma associated with bilateral diffuse-nodular hyperplasia of the adrenal cortex and micro-macrofollicular goiter].

At autopsy there was found extra-adrenal pheochromocytoma, bilateral diffuse-nodulous and adenomatous hyperplasia of the adrenal cortex in a female patient who had suffered for a long time from high arterial pressure. In the enlarged thyroid gland there was revealed micro-macrofollicular colloid goiter with basedowification. The morphological changes and results of an investigation of the liquor (drop in the content of potassium) justify the assumption that in this case there is a combination of extra-adrenal pheochromocytoma with hormonally active rearrangement of the adrenal cortex--primary

Ultrasonography of the adrenal glands in the dog, cat, and ferret.

Ultrasonography has become an important modality for imaging adrenal glands in small animals. Normal adrenal glands and structural abnormalities, such as changes in size, shape, and echogenicity, can be identified. In the case of adrenal tumors, ultrasonography is a valuable tool for identifying neoplastic masses with local and regional involvement, as well as potential abdominal metastases. When correlated with clinical findings and results of hematologic, serum biochemical and endocrine tests, ultrasonographic findings make a substantial contribution to the diagnosis of adrenal diseases in d

Contemporary Approaches to Adrenocortical Carcinoma.

Adrenocortical carcinoma is a rare, aggressive cancer with a poor prognosis. Management of early-stage disease relies on surgical resection, which remains the only potentially curative therapy. Open surgery remains the standard of care, with minimally invasive approaches being appropriate in cases with smaller tumors and no evidence of local invasion. Advanced disease traditionally relies on mitotane-based chemotherapies to control disease progression. Clinical trials evaluating the potential efficacy of immunotherapy, checkpoint inhibitors, and tyrosine kinase inhibitors are ongoing but have

Pathological and Genetic Stratification for Management of Adrenocortical Carcinoma.

CONTEXT: Adrenocortical carcinoma (ACC) is a rare endocrine malignancy that affects patients across the age spectrum. Although the overall survival in patients with ACC is poor, there is significant heterogeneity in terms of outcomes, presentation, and underlying genetic drivers. EVIDENCE ACQUISITION: This review is based on the evidence collected from primary research studies, expert reviews, and published guidelines. The studies were identified through PubMed search with key words "adrenocortical carcinoma," "prognosis," "pathology," and "genetics." The PubMed search was complemented by aut

[Primary aldosteronism--personal experience with diagnosis and surgical treatment of adenomatous forms].

Primary aldosteronism (Conn's syndrome) forms about 2% of disease in patients with arterial hypertension. Primary aldosteronism is in 50% of patients caused by aldosteron producing adenoma. Surgical treatment of these adenomas is excellent, or with complete disappearance of the disease, or with simple drug control of the symptoms after adrenalectomy. In the diagnostic process we can meet also seemingly divergent results of investigations. In these cases diagnosis usually requires the catheterization of adrenal veins with selected sampling of aldosteron level and thus determines the localisatio

Mineralocorticoid production of adrenal cortical adenomas.

We studied in vitro and in vivo corticosteroid production as well as the presence of symptoms of an increased mineralocorticoid effect in patients with 'silent' adrenal cortical adenomas, and compared these results to those found in patients with classical mineralocorticoid excess syndromes. We found that under in vitro conditions, cells from 'silent' adrenal cortical adenomas (n = 19) produced substantial amounts of both zona glomerulosa and fasciculata steroids, although the production of steroids in these cells was lower compared to that in mineralocorticoid-producing adenoma cells (n = 26)
