Transdiagnostic types of formal thought disorder and their association with gray matter brain structure: a model-based cluster analytic approach.

Formal thought disorder (FTD) is a complex syndrome affecting language and thought processes in psychotic and affective disorders. Clustering (i.e., identification of data-driven clinical subtypes) establishes latent (sub-) structures into psychopathological syndromes. A latent profile analysis (LPA) of FTD symptoms was conducted in 1 032 patients diagnosed with Schizophrenia-Spectrum-Disorders (n = 107), Major Depressive (n = 800), and Bipolar Disorder (n = 125). Clusters were compared for cognition and psychopathology. Associations with gray matter volume (GMV) and cortical surface (gyrifica

The Roscommon Family Study. II. The risk of nonschizophrenic nonaffective psychoses in relatives.

OBJECTIVE: We sought to clarify the familial relationship between the nonschizophrenic, nonaffective psychoses (schizoaffective disorder [SAD], schizophreniform disorder, delusional disorder, and atypical psychosis) and schizophrenia and affective illness (AI). DESIGN: A case-controlled epidemiologic family study using DSM-III-R criteria. RESULTS: Compared with relatives of unscreened controls, the risk of nonschizophrenic, nonaffective psychoses was significantly elevated in relatives of probands with schizophrenia, SAD, schizotypal personality disorder, and psychotic AI. No significant ele

Childhood trauma, psychosis liability and social stress reactivity: a virtual reality study.

BACKGROUND: Childhood trauma is associated with higher risk for mental disorders, including psychosis. Heightened sensitivity to social stress may be a mechanism. This virtual reality study tested the effect of childhood trauma on level of paranoid ideations and distress in response to social stress, in interaction with psychosis liability and level of social stress exposure. METHOD: Seventy-five individuals with higher psychosis liability (55 with recent onset psychotic disorder and 20 at ultra-high risk for psychosis) and 95 individuals with lower psychosis liability (42 siblings and 53 con

Psychiatric and Cognitive Symptoms Associated with Niemann-Pick Type C Disease: Neurobiology and Management.

Niemann-Pick disease type C (NPC) is a lysosomal storage disorder that presents with a spectrum of clinical manifestations from infancy and childhood or in early or mid-adulthood. Progressive neurological symptoms including ataxia, dystonia and vertical gaze palsy are a hallmark of the disease, and psychiatric symptoms such as psychosis and mood disorders are common. These latter symptoms often present early in the course of NPC and thus these patients are often diagnosed with a major psychotic or affective disorder before neurological and cognitive signs present and the diagnosis is revised.

[Psychopathology and creativity].

In this paper, a concept has been presented stating that the process of creativity may be connected with psychopathological features such as mood disorders, mainly bipolar, and psychosis-like thought abnormalities. Biographic studies point to a more frequent occurrence of affective disorders in creative subjects and members of their families. There is also data concerning the occurrence of schizophrenia in the families of prominent persons. A number of studies have demonstrated a similarity of patients with bipolar affective illness and members of their families to creative persons, as to incr

[Comorbidity of schizophrenia and bulimic anorexia. A case with forensic implications].

We are reporting about a 35 year old female forensic patient who has a double diagnosis of anorexia nervosa, bingeeating/purging type, and schizophrenia, paranoid type. She repeatedly attacked her therapists and physicians violently. Her aggressive state is a result of her psychotic interpretation of her constant preoccupation with her body image. We discuss problems concerning antipsychotic medication in her case specifically. Furthermore epidemiological aspects of the comorbidity of eating disorders and schizophrenia are considered; the incidence of schizophrenia in eating disorders seems to

Shared and Distinct Time-Space Experiences Along the Psychotic-Affective Continuum.

BACKGROUND: Alterations in lived time and space mark many mental disorders, yet they unfold in different ways. In schizophrenia spectrum disorders (SSD), the core disturbance lies at the microstructural level: fragmented temporal synthesis and disintegration of world, self, and body. In contrast, both mood disorders (MOD) and SSD involve more accessible macro-level shifts, such as acceleration or slowing of the passage of time, fixation on the past, or blockage of the future, difficulties in social space. This overlap raises the question of how such similarities in time and space experiences o

A study of psychotic symptoms in borderline personality disorder.

Patients with borderline personality disorder (BPD) report psychotic symptoms, but it has been questioned whether they are intrinsic to BPD. Thirty patients meeting Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV), criteria for BPD were drawn from a specialist personality disorder service. Exclusion criteria included a preexisting clinical diagnosis of nonaffective psychotic disorder. Participants underwent structured psychiatric interview using the Present State Examination (PSE), lifetime version. Approximately 60% of the patients reported psychotic symptoms unr

Psychosis as a multisystem disorder of aberrant aging.

Psychotic disorders, including schizophrenia and affective psychosis, affect ~3% of the population and typically emerge in early adulthood. Cardiometabolic disease accounts for much of the 20-year life-expectancy gap in psychosis. Evidence indicates potentially causal processes, often seen in aging, act within and beyond the brain, and before the onset of treatment; these include inflammation, metabolic and mitochondrial dysfunction. Here we synthesize evidence and propose a framework that proposes psychosis as a multisystem disorder of accelerated aging, and outline implications for aging-tar

Epidemiological and clinical characterization following a first psychotic episode in major depressive disorder: comparisons with schizophrenia and bipolar I disorder in the Cavan-Monaghan First Episode Psychosis Study (CAMFEPS).

While recent research on psychotic illness has focussed on the nosological, clinical, and biological relationships between schizophrenia and bipolar disorder, little attention has been directed to the most common other psychotic diagnosis, major depressive disorder with psychotic features (MDDP). As this diagnostic category captures the confluence between dimensions of psychotic and affective psychopathology, it is of unappreciated heuristic potential to inform on the nature of psychotic illness. Therefore, the epidemiology and clinical characteristics of MDDP were compared with those of schiz

Acute symbiotic psychosis in a postoperative transsexual.

An unexpected complication of sex-reassignment surgery in a 23-year-old male-to-female transsexual is presented. Having been periodically depressed but never psychotic before surgery, the patient experienced an acute decompensation 3 days after operation. Symptoms of this time-limited illness included hallucinations and wish-fulfilling delusions, but most prominent was the demand to be in continuous contact with other people. Historical data revealed a symbiotic relationship with the grandmother ending precipitously with her death when the patient was 6. It is hypothesized that the unresolved

Types of depression more frequent in bipolar than in unipolar affective illness: results of the Polish DEP-BI study.

BACKGROUND: The aim of the study was to assess the relative frequency of various kinds of depression in patients with bipolar and unipolar affective illness. The study was performed in the framework of the DEP-BI project aimed at assessing the prevalence of bipolar disorders among depressive outpatients treated by psychiatrists in Poland. METHODS: Eight-hundred and eighty patients (237 male, 643 female) participated in the study. The patients were classified into the following diagnostic categories: bipolar affective illness type I, type II, bipolar spectrum disorder and unipolar affective il

The effects of ketogenic metabolic therapy on mental health and metabolic outcomes in schizophrenia and bipolar disorder: a randomized controlled clinical trial protocol.

BACKGROUND: Schizophrenia, schizoaffective disorder, and bipolar affective disorder are debilitating psychiatric conditions characterized by a chronic pattern of emotional, behavioral, and cognitive disturbances. Shared psychopathology includes the pre-eminence of altered affective states, disorders of thoughts, and behavioral control. Additionally, those conditions share epidemiological traits, including significant cardiovascular, metabolic, infectious, and respiratory co-morbidities, resulting in reduced life expectancy of up to 25 years. Nutritional ketosis has been successfully used to tr

Dissociative Amnesia: A Mist Over Psychosis.

This case report highlights the unique presentation of dissociative amnesia masking an underlying brief psychotic disorder, triggered by a very intense psycho-social stressor. Learning points from this report center around the importance of considering psychotic as well as affective disorders alongside a presentation of dissociative amnesia, and not only the expected anxious, post-traumatic or personality-oriented states. Our patient, a 37-year-old gentleman, was brought to our emergency department via police referral. He had gaps in his autobiographical memory that, upon receiving a regular d

Manic depressive psychosis with mental retardation and flexion deformities: a clinical and cytogenetic study.

Five mentally handicapped patients are described in whom a bipolar manic depressive psychosis was associated with flexion deformities, involving principally the fingers. The effect of increasing degrees of retardation on the clinical presentation of the affective psychosis is discussed. Surgical treatment of the flexion deformity brought about considerable improvement in one patient. These five patients were further investigated cytogenetically using high resolution banding techniques. The results obtained were interesting but inconclusive. There would seem to be a definite place for further c

Sexual abuse and psychotic phenomena: a directed acyclic graph analysis of affective symptoms using English national psychiatric survey data.

BACKGROUND: Sexual abuse and bullying are associated with poor mental health in adulthood. We previously established a clear relationship between bullying and symptoms of psychosis. Similarly, we would expect sexual abuse to be linked to the emergence of psychotic symptoms, through effects on negative affect. METHOD: We analysed English data from the Adult Psychiatric Morbidity Surveys, carried out in 2007 (N = 5954) and 2014 (N = 5946), based on representative national samples living in private households. We used probabilistic graphical models represented by directed acyclic graphs (DAGs).

[Reactive depression in patients with slowly progressive schizophrenia].

Under observation there were 71 patients with reactive states developed in the presence of slowly progressing schizophrenia. Three types of the reactive depressions were distinguished: simple depressions following the line of psychogenically provoked endogenous phases; hysteric-type depressions, and reactive depressions with a polymorphous picture approaching schizophrenic reactions and being, in essence, psychogenically provoked schizophrenic episodes. It is shown that the level of the reactive lability and the time course of the reactive depressions is determined to a great measure by the de
