Duration of imipramine therapy and relapse in panic disorder with agoraphobia.

It has been suggested that maintenance treatment of patients who have remitted panic disorder with agoraphobia beyond the six months of acute phase imipramine treatment may decrease the risk of relapse. This study further explores the relationship between relapse and duration of imipramine treatment in this population.Fifty-one patients, all in remission at the end of six months acute phase open trial with imipramine 2.25 mg/kg/day and randomized to double-blind maintenance or placebo substitution, discontinued imipramine treatment eventually and were followed over a 12-month risk period: 27 d…

Recognition bias for safe faces in panic disorder with agoraphobia.

Panic patients with agoraphobia were compared with normal controls on tasks of face recognition. The subjects were presented with 20 photos, and were required to make a judgement of the persons on the photos; shortly afterwards they were unexpectedly presented with a recognition task. In the first study, one task was to judge whether the persons on the photos were critical or accepting: unlike social phobics (Lundh and Ost, 1996b, Behaviour Research and Therapy, 34, 787-794), panic patients showed no bias for critical vs accepting faces on the recognition task. In a secondary study, the task w…

Panic disorder and agoraphobia in consecutively referred children and adolescents.

OBJECTIVE: This report examines the clinical features and correlates of juvenile panic disorder in referred children and adolescents to test specific hypotheses about its relationship with adult panic disorder. METHOD: The sample consisted of consecutively referred children and adolescents (N = 472) comprehensively evaluated with structured diagnostic interviews, cognitive tests, and psychosocial assessments. RESULTS: Panic disorder was identified in 6% and agoraphobia in 15% of psychiatrically referred children and adolescents. Children meeting criteria for panic disorder also frequently me…

Interpretive bias for benign sensations in panic disorder with agoraphobia.

The present study further examines the cognitive model of panic disorder by investigating two questions. The first is whether panic patients misinterpret bodily sensations which are symptoms of either nonanxious states or harmless events. The second is whether panic patients are able to provide benign subsequent explanations for bodily sensations which have initially been interpreted in an anxiety-related manner. Two groups of subjects were used, 15 panic disorder patients with agoraphobia and 15 controls, matched on verbal fluency, age and gender. Compared to controls, patients failed to iden…

Explicit and implicit memory bias in panic disorder with agoraphobia.

Thirty patients with a diagnosis of panic disorder with agoraphobia and 30 normal controls were compared on explicit memory (cued recall) and implicit memory (word stem completion) for positive, neutral, social threat, and physical threat words. The panic patients showed an explicit memory bias, but no implicit memory bias, for physical threat words. The index of explicit memory bias for physical threat words was found to correlate with anxiety sensitivity and degree of agoraphobic fear and avoidance. The index of baseline bias for threat words on the word completion task, on the other hand, c…

[Treatment of panic disorders and agoraphobia. Psychotherapy, psychopharmacotherapy and their combination].

During the last two decades substantial progress has been achieved in the development and evaluation of effective treatment approaches to panic disorder and agoraphobia. Many clinical research studies in this area were begun after the definition of operationalized diagnostic criteria for panic disorder and agoraphobia in the DSM-III in 1980. Different concepts concerning the etiology, pathogenesis and maintaining factors of these disorders are still controversial. At the beginning of this controversy, psychopharmacological treatment was believed to suppress spontaneous panic attacks while beha…

The phenomenology of panic attacks in panic disorder with and without agoraphobia.

The frequency of symptoms during panic attacks and anticipation of the panic consequences were compared in patients with the subtypes of panic disorder (PD). Patients with moderate and severe agoraphobic avoidance reported that they had experienced more symptoms than patients with an uncomplicated PD (without agoraphobia [AG]); they also experienced almost all of the symptoms more frequently, with the difference being significant for a quarter of the examined panic symptoms. Panic patients with moderate and severe AG were also significantly more concerned about the loss of control and social a…

Gender effects and alcohol use in panic disorder with agoraphobia.

Previous studies have found some significant, but weak, gender differences in panic and agoraphobia with females generally being more symptomatic. The present study sought to expand this line of research by examining alcohol use and self-medication in relation to gender differences and measures of psychopathology. Seventy-four male and 162 female patients with panic disorder with agoraphobia were compared. There were some significant, but relatively small, gender differences with females reporting higher levels of phobic avoidance. Males reported significantly more weekly alcohol intake and al…

Reports of childhood incest by adults with panic disorder or agoraphobia.

A convenience sample of 94 members of an agoraphobia self-help group responded to an anonymous survey on their possible histories of childhood incest. 12 (13%) respondents reported such a history. These results are discussed in terms of hypothesized etiologies of panic disorder and agoraphobia.

A prospective study of panic and anxiety in agoraphobia with panic disorder.

The features of panic and anxiety in the natural environment were studied by prospective self-monitoring in 39 patients with chronic agoraphobia and panic disorder. Panics overlapped greatly with anxiety episodes but were more intense. Panics occurred more often in public places than did anxiety episodes, but had otherwise similar symptom profile, time of occurrence, and antecedents. Most panics surged out of a pre-existing plateau of tonic anxiety which lasted most of the day. Spontaneous panics were less frequent than situational panics and occurred more often at home but were otherwise simi…

Prodromal symptoms in panic disorder with agoraphobia: a replication study.

The majority of 20 patients suffering from panic disorder with agoraphobia reported experiencing agoraphobic avoidance, generalized anxiety, and/or hypochondriacal fears and beliefs before the first panic attack. The results replicated those of a previous investigation and are in accordance with an increasing number of studies concerned with prodromal symptoms, epidemiologic surveys, and analysis of mechanisms of change upon treatment.

Sleep studies and neurochemical correlates in panic disorder and agoraphobia.

Although panic disorder is classified by the DSM III among the anxiety disorders, there is evidence from epidemiological and neurochemical studies that links panic to the affective disorders. In addition several of the effective pharmacological treatments suggest, as in the depressive disorders, serotonergic involvement. As part of the evaluation of patients who would take part in the Cross National Collaborative Panic study, polysomnography, the dexamethasone suppression test and platelet imipramine binding and 5HT-uptake were scheduled. 44 patients who met DSM III criteria on the SCID interv

Fading of therapeutic effects of alprazolam in agoraphobia. Case reports.

1. A decrease in therapeutic effects of alprazolam after an initial good response is described in three patients suffering from panic disorder with agoraphobia. 2. The possibility of fading of therapeutic effects of alprazolam (defined as a progressive decrease of therapeutic effects refractory to dosage increase, after non-immediate symptomatic improvement) in such cases is discussed. 3. Fading may not be apparent from large scale controlled studies with alprazolam, since its detection requires a prolonged time of observation and a specific design.

Trazodone in the treatment of panic disorder and agoraphobia with panic attacks.

Eleven patients with panic disorder or agoraphobia with panic attacks completed an 8-week single-blind trial of trazodone (300 mg/day) without concurrent behavioral instructions. The measures of change included ratings of generalized and panic anxiety, phobias, and depression and a behavioral avoidance test, which were administered during a baseline period of placebo administration and at 4 and 8 weeks of the trial. There was significant improvement on all symptom dimensions, which suggests that trazodone may have specific antipanic and antiphobic actions and underscores the importance of sero…
