Histopathological abnormalities of muscle are common in chronic alcoholics and can be classified into the rare acute myopathy, with myonecrosis, and the much more common chronic myopathy, with a selective atrophy of type II muscle fibres (particularly IIb fibres). The patients may often be asymptomatic and, except for florid cases of acute rhabdomyolysis, clinical features do not help to distinguish the two. Serum creatine kinase activity is an insensitive guide to the presence of the muscle abnormalities. These abnormalities cannot be adequately explained by co-existing alcoholic liver diseas

Retrograde axonal transport may play an important role in the feedback regulation of protein synthesis in neuronal perikarya, and the anterograde axonal transport of protein. The "dying-back" neuropathies associated with thiamine deficiency and chronic alcoholism may arise as a consequence of altered axonal transport. We have reported alterations in fast anterograde axonal transport in rats as a result of ethanol exposure or thiamine deficiency. The present studies were undertaken to determine whether retrograde transport was also affected by these experimental treatments. One group of rats wa

BACKGROUND: Diabetic peripheral neuropathy (DPN) is one of the most common microvascular complications of type 2 diabetes mellitus (T2DM) and contributes substantially to morbidity and disability. Early detection and identification of associated factors are essential for preventing complications and improving diabetes care.
OBJECTIVES: To estimate the prevalence of diabetic peripheral neuropathy and identify its independent predictors among participants with T2DM in rural South India.
METHODS: A community-based cross-sectional study was conducted among 390 participants with T2DM in the field

BACKGROUND: Peripheral neuropathies may present with vascular skin lesions, such as livedo racemosa, ulcers, palpable purpura, or necrosis. These rare neurocutaneous associations can be severe and life-threatening.
OBJECTIVE: To determine (1) whether phenotypic correlations exist between neuropathy profiles and vascular skin lesion subtypes, and (2) whether these syndromes share a specific mechanism.
METHODS: We conducted a monocentric retrospective study at Montpellier University Hospital (1993-2022) including patients with peripheral neuropathy and vascular skin lesion(s) occurring as a sy

AIM: To study the normal distributions of mitochondria and voltage gated Na+ channels in the human optic nerve head in order to gain insight into the potential mechanisms of optic nerve dysfunction seen in the inherited optic neuropathies.
METHODS: Five fresh frozen human optic nerves were studied. Longitudinally orientated, serial cryosections of optic nerve head were cut for mitochondrial enzyme histochemistry and immunolabelling for cytochrome c oxidase (COX) subunits and voltage gated Na+ channel subtypes (Na(v) 1.1, 1.2, 1.3, and 1.6).
RESULTS: A high density of voltage gated Na+ channe

Chronic liver disease, both alcoholic and nonalcoholic, has been shown to be associated with autonomic neuropathy, as well as other hemodynamic and circulatory disturbances. In a longitudinal study, the presence of autonomic neuropathy and the severity of liver disease were independent risk factors for mortality. The aim of this study was to determine whether the severity of liver disease correlated with measures of heart rate variability. We studied 21 patients being evaluated for liver transplantation to determine if severity of disease correlated with heart rate variability and compared the

Several infectious pulmonary diseases affect the eye. An understanding of the association between infectious pulmonary and ocular diseases is pivotal to their successful management. We aimed to review the infections affecting both the lungs and the eye. The electronic database PubMed and the search engine Google Scholar were searched for relevant articles. Ocular tuberculosis (TB), usually not associated with clinical evidence of pulmonary TB, can affect almost all the ocular structures. Confirmation of the diagnosis of ocular TB requires demonstration of Mycobacterium tuberculosis in ocular f

Thiamine deficiency (vitamin B1) is common in patients with alcohol dependence. Cognitive impairments may be an early consequence of thiamine deficiency. Wernicke's encephalopathy is underdiagnosed and undertreated. In patients with established Wernicke's encephalopathy, parenteral thiamine 200-500mg three times a day should be given for 3-5 days, followed by oral thiamine 250-1000mg/day. In patients with suspected Wernicke's encephalopathy, parenteral thiamine 250-300mg should be given two times a day for 3-5 days, followed by oral thiamine 250-300mg/day. In patients at high risk of thiamine

Benign Symmetric Lipomatosis (Madelung's disease) is a rare disease, characterized by massive fatty deposits in the neck, the shoulders, and the upper extremities. The deformity is associated with chronic alcohol use, malignant tumors of the upper airways, neuropathy, diabetes mellitus, hyperlipidemia, and other metabolic disorders. Although the deformity is prone to recurrence, surgical removal via lipectomy or liposuction provides the only way of palliation. This paper describes the treatment of a 51-year-old man with a history of alcoholism and liver cirrhosis. He reported masses in his cer

A growing number of therapeutic agents and exogenous toxins are harmful to structure and function of human skeletal muscle. The clinical syndrome encompasses asymptomatic creatine kinase elevation, myalgia, exercise intolerance, muscle paresis and atrophy, and lastly acute rhabdomyolysis. Toxic myopathies are potentially reversible, hence a prompt recognition is particularly helpful for the early diagnosis and in conclusion elimination of a myopathy inducing toxin. Toxic myopathies may be classified as acute or chronic accordingly to the exposition time to a toxin. Main source of an exogenous

Another case of flexion contracture in a 60 year-old patient with hypopituitarism, chronic alcoholism and starvation is reported. The contracture, as in the other cases, involved the lower limbs, but in this particular patient it extended to the abdominal and extensor neck muscles and neurological signs were associated: cerebellar atrophy and peripheral neuropathy of the lower limbs probably due to alcoholism. As in the other reported cases, severe hyponatremia was observed (105 mmol/l) the physiopathology of which is discussed: starvation and glucocorticoid deficiency alone or associated with

Accidental hypothermia is a condition associated with significant morbidity and mortality. A 48-year-old male with a history of alcohol use disorder and optic neuropathy presented to the emergency department after being found unresponsive with an unknown downtime. One hundred four minutes passed from resuscitation, to pre-hospital discovery, until cannulation with extracorporeal membrane oxygenation. Here, a rare case of successful resuscitation of a profoundly hypothermic patient to normal neurologic outcome is presented.

Metronidazole is an antibiotic commonly prescribed for anaerobic and protozoan infections. Despite its good safety profile, this drug frequently causes a series of well-known side effects (nausea and intestinal transit disorders, dysgeusia, headaches, and alcohol intolerance). However, there are few data in the literature, mainly case reports and case series, about the onset of peripheral neuropathy with a generally self-limiting course after drug withdrawal. Thus, we herein describe two cases of peripheral neuropathy due to treatment with metronidazole. A 69-year-old woman treated with a tota

Neuropathic arthropathy (Charcot's arthropathy) is a progressive articular disease associated with a reduced sensorial and protector proprioceptive reflex. Its etiology includes many different conditions such as syringomyelia, traumatic lesion causing medullary deformity, spina bifida, diabetic neuropathy, leprosy neuropathy, neurofibromatosis, amyloid neuropathy, alcohol, and repetitive injection of hydrocortisone into joints, among others. However, the relationship between Charcot's arthropathy and herpetic encephalitis has not yet been described. Herpes encephalitis causes acute and chronic

A case of middle aged male who developed swelling and weakness of muscles in the lower limbs following a heavy binge of alcohol is being reported. He had myoglobinuria and developed acute renal failure for which he was dialyzed. Acute alcoholic myopathy is not a well recognized condition and should be considered in any intoxicated patient who presents with muscle tenderness and weakness.

We have reported a unique example of an organic affective syndrome in a patient who had focal brain lesions, but whose initial neurologic examination was nonfocal. In retrospect, the only clue to an organic disorder was hyperreflexia in this patient with a history of alcohol abuse and clinical signs of a distal sensory neuropathy. This case emphasizes the importance of pursuing a neurologic work-up (including CT scan) in older patients with sudden mood changes, even in the absence of focal neurologic findings.

Background/Objectives: Toxic optic neuropathy (TON) represents a spectrum of optic nerve damage caused by exposure to toxins, including drugs, alcohol, and industrial chemicals. It is characterized by progressive vision loss, dyschromatopsia, and optic nerve pallor and poses a clinical challenge in diagnosis and management due to overlapping features with other optic neuropathies. Non-arteritic anterior ischemic optic neuropathy (NAION), although distinct, shares common pathophysiological mechanisms such as oxidative stress and mitochondrial dysfunction. This review aims to evaluate therapeuti

OBJECT: The resection of glomus jugulare tumors can be challenging because of their inherent vascularity. Preoperative embolization has been advocated as a means of reducing operative times, blood loss, and surgical complications. However, the incidence of cranial neuropathy associated with the embolization of these tumors has not been established. The authors of this study describe their experience with cranial neuropathy following transarterial embolization of glomus jugulare tumors using ethylene vinyl alcohol (Onyx, eV3 Inc.).
METHODS: The authors retrospectively reviewed all cases of glo
