[Complicated sigmoid tumor: perforated amebiasis].

Amoeboma is an inflammatory mass of the colon. It can be an inaugural symptom and thus pose the problem of differential diagnosis with colon cancer. The purpose of this report is to describe the case of a 43-year-old patient who presented with acute abdomen. Physical examination revealed a perforated circumferential mass in the sigmoid colon. Based on a presumptive diagnosis of colonic cancer complicated by perforation, segmental colectomy was performed. Histological examination of the surgical specimen demonstrated colonic amoeboma. The patient was treated using metronidazole. Although rare,

Adherence-blocking vaccine for amebiasis.

The Gal/GalNAc lectin is a candidate vaccine antigen for an amebiasis vaccine due to its mediation of parasite adherence to the human intestine, because partial immunity in humans is associated with a mucosal IgA response against it, and because it is effective as a vaccine against amebic colitis in the murine model. The LecA domain of the Gal/GalNAc lectin contains neutralizing antibody epitopes. LecA contains the active site of the lectin (the carbohydrate recognition domain or "CRD") and has been an effective vaccine antigen in animal models of amebic colitis and liver abscess. Research nee…

Progress towards an amebiasis vaccine.

Amebiasis (infection by Entamoeba histolytica) remains a major health problem in much of the developing world. Morbidity and mortality from amebic dysentery and amebic liver abscess have persisted despite the availability of effective anti-amebic therapy, suggesting a need for alternative measures of disease control. Through the application of recombinant DNA technology, several E. histolytica antigens have now been expressed in prokaryotic systems and tested in animal models as vaccines to prevent invasive amebiasis. In this review, Sam Stanley Jr discusses why a vaccine for amebiasis may be …

Fulminant amebiasis: a clinical evaluation.

Two patients with fulminant amebic colitis with colon perforation and concomitant liver abscess were collected over the last 5 years. One patient underwent emergency laparotomy to treat amebic cecal perforation. Diverted ileostomy saved his life. The ileostomy was successfully reversed 6 months later. The other patient underwent 4 laparotomies with more invasive procedures in less than 1 month due to sequential complications of amebiasis. Colon resection with enterostomy miraculously allowed him to survive. In comparison with the latter, who underwent more aggressive surgery and experienced mo…

[Intestinal amebiasis in Antofagasta, Chile].

In January 1989--December 1991 period we verified 35 cases of symptomatic intestinal amebiasis in Antofagasta, the main city of the North of Chile. Out of the total number of studied patients 23 were adults (77.1% males and 22.9% females) and all of them were permanent residents of Antofagasta. E.histolytica was frequently detected in association with other parasites and/or commensals, and observed alone in only seven cases. The clinical and epidemiological aspects of these 35 cases are discussed.

[Histopathology of experimental hepatic amebiasis].

The method of intrahepatic inoculation was successfully used to establish an experimental model of hepatic amebiasis in golden hamsters for studying its pathological morphology. Three types of macroscopic liver lesions, i.e., isolated abscess, multinodular abscess and ruptured lesion were described. On light microscopy the alteration and the proliferation type lesions were found as distinct pathological characteristics of the early and advanced hepatic amebiasis respectively. The development pattern of hepatic amebiasis and the pathogenicity of Entamoeba histolytica were discussed.

Amebiasis: clinical and laboratory perspectives.

Entamoeba histolytica, the premier intestinal protozoan, has traversed time in its relentless quest for survival in its dichotomous role of parasite and pathogen. Enigmatic in its transition from human intestinal commensal to invader of human tissue, diverse in its pathogenicity for the human host, and intricate in its bacterial interrelationship in the bowel, E. histolytica has become the focal point of intensive investigation in its basic biology underscoring human pathogenicity. This review will focus on facets of cell biology, pathophysiology, clinical, therapeutic, and epidemiologic, corr

[Amebiasis. Surgical treatment in 1989].

Even when the number of patients with invasive amebiasis has decreased, the internist and surgeon must be alert in case that the patient requires an operation. Amebic liver abscess is treated medically; percutaneous evacuation is rarely used and surgical drainage is made when there is not response to medical treatment or there is high risk of abscess rupture. Operation is mandatory when the abscess has ruptured to the abdominal cavity or through the pericardial sac. In fulminant colitis it is necessary to resect the diseased portion of the colon without primary anastomoses. Amebic apendicitis …

Seven patients with amebic liver abscess presenting as pleuropulmonary disease were admitted to hospital initially because of pulmonary symp … the bronchus may provide spontaneous drainage so that only medical therapy is needed. Recovery from amebiasis in all three categories is generally complete. Morbidity and mortality increase with failure to correctly identify amebic infection of the liver as the underlying cause. Because, in new cases, no findings specifically suggest that pleuropulmonary di …

[Acute hepatic amebiasis (author's transl)].

A case report is given of severe extraintestinal hepatic amebiasis without diarrhea. Formation of an hepatic abscess could be avoided by early institution of appropriate antiamebic treatment, following the diagnosis using sonography and specific serological methods.

Surgical intervention in intestinal amebiasis.

Operative intervention in patients with unrecognized and untreated amebomas or acute fulminating amebic colitis may result in severe complications. Nevertheless, certain clinical presentations of intestinal amebiasis require surgical procedures varying from drainage of an abscess to a subtotal colectomy. If, in patients with acute fulminating amebic colitis, the signs or symptoms of intraperitoneal or impending perforation develop, unremitting diarrhea associated with anemia and hypoproteinemia continues, or localized abscsses fail to improve with chemotherapy, operative intervention is indica…
