Inherited factor V deficient neonate with galactosaemia.

OBJECTIVES: Reporting a case of inherited factor V deficiency and galactosemia. METHODS: A neonate was admitted with hematoma, jaundice, splenomegaly, diarrhea, anemia, abdominal ascites and bilateral cataracts that diagnosis of galactosaemia and factor V deficiency was established. RESULTS: Coinheritance of both coagulation disorder and metabolic disorder is very rare episode that was identified in a neonate. CONCLUSION: Our case indicates that in mild bleeding episodes of neonates that imitate of coagulation disorders should be considered promptly by pediatricians.

Extremely Rare Case of Fetal Anemia Due to Mitochondrial Disease Managed with Intrauterine Transfusion.

This report describes a rare case of fetal anemia, confirmed as a mitochondrial disease after birth, treated with intrauterine transfusion (IUT). Although mitochondrial diseases have been described in newborns, research on their prenatal features is lacking. A patient was referred to our institution at 32 gestational weeks owing to fetal hydrops. Fetal anemia was confirmed by cordocentesis. After IUT had been performed three times, the anemia and associated fetal hydrops showed improvement. However, after birth, the neonate had recurrent pancytopenia and lactic acidosis. He was eventually diag

Causes and outcomes of prenatally unexplained fetal anemia.

Despite advances in diagnostic approaches, fetal anemia of unknown etiology continues to be observed in rare cases. This study aimed to assess the incidence of unexplained fetal anemia and to evaluate the associated perinatal outcomes. We conducted an observational retrospective cohort study of all fetuses that underwent fetal blood sampling (FBS) due to an MCA-PSV>1.5 MoM at a tertiary center between 2007 and 2024. Fetuses were included if they had moderate or severe anemia defined as a hemoglobin (Hgb) deviation of more than 20g/L below a gestational age adjusted mean, with a negative anemia

Identification of a De Novoc.1000delA ANK1 mutation associated to hereditary spherocytosis in a neonate with Coombs-negative hemolytic jaundice-case reports and review of the literature.

BACKGROUND: To strengthen the understanding of Hereditary Spherocytosis (HS) and determine the disease-causing mutation present with neonatal jaundice. HS is a hemolytic condition resulting from various erythrocyte membrane defects. Many different mutations result in HS, including mutations in ANK1. CASE PRESENTATION: A term neonate presented at ten hours with severe jaundice requiring exchange transfusion. At two months he was hospitalized due to repeated pallor and anemia requiring blood transfusions. Using next-generation sequencing, we discovered the responsible mutation in the proband bu

KLF1-null neonates display hydrops fetalis and a deranged erythroid transcriptome.

We describe a case of severe neonatal anemia with kernicterus caused by compound heterozygosity for null mutations in KLF1, each inherited from asymptomatic parents. One of the mutations is novel. This is the first described case of a KLF1-null human. The phenotype of severe nonspherocytic hemolytic anemia, jaundice, hepatosplenomegaly, and marked erythroblastosis is more severe than that present in congenital dyserythropoietic anemia type IV as a result of dominant mutations in the second zinc-finger of KLF1. There was a very high level of HbF expression into childhood (>70%), consistent with

Effective combined immunotherapy for severe hemolytic disease of the fetus.

BACKGROUND: Maternal anti-Kell and anti-Rhesus D-alloimmunization cause severe hemolytic disease of the fetus and newborn (HDF) by crossing the placenta, leading to anemia and hydrops fetalis. Early-onset HDF from anti-K and anti-RhD alloimmunization often requires multiple, early intrauterine transfusions (IUTs), with higher complication rates and decreased sensitivity after the first intervention. Mortality remains high. CASE REPORT: We report the successful treatment of three pregnant women with high-titer red cell alloantibodies-two with anti-K and one with anti-RhD-during the first trime

An explanation of the pathophysiology of adverse neurodevelopmental outcomes in iron deficiency.

Iron deficiency (ID) is a major public health problem worldwide among children aged 0-12 months. Several factors seem to contribute to the iron-deficient state in infancy, including insufficient antenatal and neonatal iron supplementation, exclusive breastfeeding, and early umbilical cord clamping after birth. The most concerning complications of ID, except for anemia, are related to altered long-term neurodevelopment. Clinical studies have shown a negative impact of ID anemia on fetal and neonatal behavior including impairments of motor maturity, autonomic response, memory/learning, and mood.

Optimal predelivery hemoglobin to reduce transfusion and adverse perinatal outcomes.

BACKGROUND: Maternal anemia has been associated with poor obstetrical outcomes; however, the optimal hemoglobin level for reducing blood transfusion at delivery has not been well-defined. OBJECTIVE: This study aimed to measure the association of maternal anemia immediately before delivery with peripartum transfusion and other adverse perinatal outcomes. We also sought to identify the optimal hemoglobin level for predicting transfusion. STUDY DESIGN: This was a retrospective cohort study of patients who had hemoglobin or hematocrit collected before delivery of live, nonanomalous neonates at ≥

A Novel ANK1 Mutation in a Neonatal Hereditary Spherocytosis Case: Diagnostic Challenges and Familial Genetic Analysis.

Hereditary spherocytosis (HS) is a congenital disease in which erythrocyte membranes are abnormal, with ANK1 defects as the main cause. The diagnosis of neonatal HS is difficult due to poor phenotypic specificity. Therefore, a detailed inquiry into family history may be helpful for diagnosis. Here, we describe a familial case of HS caused by a novel mutation in ANK1. The proband is a premature infant of Chinese Han ethnicity characterized by progressive aggravation of anemia and jaundice. The disease was caused by a frameshift mutation (c.3392delT/p.Leu1131Argfs*15) of ANK1 that was identified

Management of neonatal Rh disease.

Dramatic improvements have been made in the management of Rh disease. Anti-D immune globulin has reduced the incidence of Rh sensitization. Intrauterine transfusions have become routine to treat fetal anemia. Once an affected infant is born, several recent improvements in neonatal care have aided in the treatment of hyperbilirubinemia. These include improved phototherapy, such as fiberoptic delivery systems, and intravenous immunoglobulin. Experience with heme oxygenase inhibitors is accumulating, and they may prove efficacious in Rh disease. Double-volume (and perhaps single-volume) exchange

Maternal diabetes and the role of neonatal reticulocyte hemoglobin content as a biomarker of iron status in the perinatal period.

AIMS: We aimed to evaluate the effects of maternal diabetes on neonatal iron status, measuring erythrocyte indices including hemoglobin, hematocrit, reticulocytes, mean corpuscular volume (MCV), percent (%) hypochromia, ferritin, and additionally mean reticulocyte hemoglobin content (MCHr) as an early marker of iron deficiency, and examine the association between neonatal MCHr, red cell indices, and ferritin. MATERIALS AND METHODS: We conducted a hospital-based prospective cohort study in a tertiary neonatal unit of a University Hospital from 2018 to 2020. We enrolled 126 maternal-infant pair

Neonatal babesiosis: case report and review of the literature.

A case of transfusion-associated neonatal babesiosis is presented. Jaundice, hepatosplenomegaly, anemia and conjugated hyperbilirubinemia developed in this preterm infant. The diagnosis was eventually made by blood smear, serology and polymerase chain reaction. The patient was treated with clindamycin and quinine and made a favorable recovery. Of neonatal babesiosis reported in the literature, 9 other cases are reviewed, including 6 that were transfusion-associated, 2 congenital and 2 tick transmitted.

[Familial lymphohistiocytosis. Anatomopathological study of 2 cases detected neonatally].

Two brothers presented with hemophagocytic reticulosis with neonatal onset. Early clinical and biological symptoms included pallor, hepatosplenomegaly and anemia, thrombocytopenia. Evolution was lethal in both cases, at 5 months and 13 days of age, respectively. Diagnosis was confirmed in both by the pathologic findings: diffuse lymphohistiocytic cellular proliferation with hemophagocytosis and atrophy of the lymphoid tissue. The diagnostic difficulties of this disease in the neonatal period are emphasized.

Sinusoidal heart rate pattern and fetal distress secondary to severe anemia.

A case of sinusoidal fetal heart rate (FHR) pattern with fetal anemia is described. The etiology of the pattern appears to have been fetal anemia from an umbilical cord knot, abruptio placentae and fetal asphyxia. The poor neonatal outcome in our case and a review of the literature suggest that a sinusoidal pattern is an ominous sign demanding complete evaluation of fetal well-being and often prompt obstetric intervention. The use of scalp capillary hematocrit is suggested.

Neonatal adrenal hemorrhage.

The neonatal adrenal gland is vulnerable to traumatic hemorrhage. With the correct interpretation of clinical, laboratory and radiographic data the diagnosis can be made non-operatively. We review 6 cases of acute neonatal hemorrhage. Trauma during or shortly after birth occurred in 4 cases and all 6 newborns had jaundice and anemia. Operative intervention rarely is necessary either for the diagnosis or treatment of intracapsular adrenal hemorrhage in the newborn.

Hereditary Spherocytosis in the Neonatal Period: A Case Report.

Hereditary spherocytosis (HS) is the third most common yet most frequently underrecognized, congenitally acquired hemolytic disease of the neonate. Hereditary spherocytosis is caused by a defect of one or more erythrocyte membrane proteins, which leads to an increased rate of destruction of circulating red blood cells. The HS spectrum of symptoms is varied from asymptomatic to intrauterine hydrops. Diagnostic tests range from a complete blood count (CBC) analysis to deoxyribonucleic acid (DNA) sequencing. Management in the neonatal period focuses primarily on associated comorbidities, includin
