Anesthetic management in an infant with tetra-amelia syndrome with congenital maxillomandibular fusion: A case report.

A 10-month-old girl who had tetra-amelia syndrome and congenital maxillomandibular fusion (syngnathia) was scheduled for the surgical fusion separation. Anesthetic management for this case was considerably challenging. Standard monitoring was still applied to the patient's extremities. IV access was suspected to be difficult but firmly needed before intubation to provide resuscitation during an emergency. Connecting anesthetic circuit with nasopharyngeal airway was the preferred technique due to its benefits such as maintaining spontaneous ventilation, providing inhaled anesthetic, as well as

Clinical and economic factors important to anaesthetic choice for day-case surgery.

Clinical and economic factors that are important to consider when selecting anaesthesia for day-case surgery can differ from those for inpatient anaesthesia. Patients undergoing day-case surgery tend to be healthier and have shorter durations of surgery. They expect less anxiety before surgery, amnesia for the surgical experience, a rapid return to normal (normal mentation with minimal pain and nausea) after surgery, and lower expenses. However, the latter 2 expectations can conflict; older generic drugs have lower acquisition costs but often impose longer recovery times. Longer recovery perio

Improving the efficiency of sevoflurane delivery during general anaesthesia by educating and motivating anaesthetists to utilise the Volatile Efficiency Ratio.

Volatile anaesthetic agents such as sevoflurane contribute to greenhouse gas emissions, and selecting low fresh gas flows on anaesthetic machines minimises their waste. Facilitating improvements in sevoflurane use requires the education, motivation, and standardised evaluation of anaesthetists. There is currently no standard of practice related to the efficiency of anaesthetic gas delivery per case. We conducted a multi-component study termed 'Low With The Flow' (LWTF) to directly address these requirements by educating and motivating anaesthetists to reduce fresh gas flow and thereby sevoflur

Isoflurane-associated penile erection in two bats: a Savi's pipistrelle (Hypsugo savii) and a Kuhl's pipistrelle (Pipistrellus kuhlii).

This report describes two cases of persistent penile erection in vespertilionid bats (Hypsugo savii and Pipistrellus kuhlii) during anaesthesia with isoflurane. The bats, presenting with wing fractures, were anaesthetised setting the vaporiser at 5% isoflurane for induction and 2% for maintenance. Both subjects developed penile erection upon induction of anaesthesia, which persisted for the duration of the procedures, lasting approximately 20 minutes. The condition resolved spontaneously after isoflurane discontinuation in both cases. No complications were observed and recovery was uneventful.

ISOFLURANE RESPIRATORY ANESTHETIC INDEX IN CHICKENS (GALLUS GALLUS DOMESTICUS).

Respiratory depression from isoflurane seems to be greater in birds than in mammals. Isoflurane respiratory anesthetic index (AI) has only been evaluated in ducks (Anas platyrhynchos), which indeed showed a lower AI compared to mammals, but the isoflurane AI for other avian species is not known. The aim of this study was to evaluate the isoflurane AI in chickens (Gallus gallus domesticus). Six adult hens were anesthetized with isoflurane for determination of the minimum anesthetic concentration (MAC) using the bracketing method. During a second anesthetic event, the isoflurane AI was determine

High initial concentration versus low initial concentration sevoflurane for inhalational induction of anaesthesia.

BACKGROUND: Sevoflurane induction for general anaesthesia has been reported to be safe, reliable and well accepted by patients. Sevoflurane induction uses either low or high initial concentrations. The low initial concentration technique involves initially administering a low concentration of sevoflurane and gradually increasing the concentration of the dose until the patient is anaesthetized. The high initial concentration technique involves administering high concentrations from the beginning, then continuing with those high doses until the patient is anaesthetized. This review was originall

Electroencephalographic Density Spectral Array monitoring in pediatric anesthesia: clinical background and practical applications.

Depth of hypnosis monitoring using numerical index based processed electroencephalography is part of the standard anesthesia equipment in many operating rooms. Unfortunately, this method has its limitations, especially in pediatric patients. Therefore, there is an increasing interest in raw electroencephalography parameters. Electroencephalographic density spectral array (DSA) monitoring is a real-time technique that can be used in all age groups and with all common anesthetics. We present a practice-oriented introduction to the principle of DSA monitoring in pediatric anesthesia for pediatric

Thiopental to desflurane - an anaesthetic journey. Where are we going next?

Development targets in anaesthetic pharmacology have evolved from minimizing harm caused by unwanted effects through an era in which rapid onset and offset of drug effect were prioritised. Today's anaesthetists have access to a library of effective drugs whose characteristics offer controllable hypnosis, analgesia and paralysis with manageable off-target effects. The availability of these agents at generic prices inhibits commercial interest and this is reflected in the limited number of current anaesthetic drug development projects. Recently, questions around neonatal neurotoxicity, delirium

Anesthetic considerations and surgical caveats for awake airway surgery.

The evolution of novel techniques for the treatment of laryngeal pathology has led to a significant expansion of the role of diagnostic assessment and the range of laryngeal procedures performed. These procedures typically benefit from an anesthetic approach that diverges from a standard general endotracheal or laryngeal mask airway-based inhalational anesthetic. The shared airway, need for intraoperative assessment of vocal cord function, risk of airway fire, and desire for rapid emergence and discharge are all important factors. In this article the authors undertake a collaborative anesthesi

Comparison of Isoflurane, Ketamine-Dexmedetomidine, and Ketamine-Xylazine for General Anesthesia during Oral Procedures in Rice Rats (Oryzomys palustris).

Rice rats (Oryzomys palustris) are an unconventional laboratory species that has been used to study photoperiodicity, periodontitis, and osteonecrosis of the jaw. Interventional procedures that require anesthesia, including oral procedures, are sometimes necessary in preclinical settings. The use of anesthetics including isoflurane and ketamine combined with α2-adrenoreceptor agonists, such as dexmedetomidine and xylazine, is well-established for laboratory rodents. However, their effects have been studied only modestly in rice rats. The aims of this study were to 1) determine the safety and c

General anesthesia/general anesthetics: Journal of Anesthesia symposium 2025.

Key interventions for anesthesia patients include administering drugs, infusing fluids and blood, and providing artificial ventilation. As general anesthetics are always administered to patients undergoing general anesthesia, anesthetists need to be familiar with the fundamentals and the latest issues of general anesthesia/anesthetics. The Journal of Anesthesia symposium 2025 aimed to discuss fundamental topics and environmental issues related to general anesthesia and anesthetics. In this symposium, we had following topics: (1) patient outcomes of general anesthesia observed outside the opera

Glutathione depletion following inhalation anesthesia.

Glutathione depletion following inhalation of halogenated anesthetics was investigated as a possible mechanism of toxic reactions associated with anesthesia. Concentrations of reduced glutathione were measured in the blood, liver, lung and kidney of the mouse after anesthesia with enflurane, fluroxene, halothane, isoflurane, methoxyflurane, or trichloroethylene. The anesthetic had no effect on glutathione concentrations in tissues except when fluroxene was used. After two hours of fluroxene anesthesia, glutathione in liver, lung, kidney, and blood was depleted by 93, 85, 85, and 61 per cent, r

[In vitro effects of anesthetics on hydrogen peroxide generating system of the thyroid gland].

Effects of anesthetics on thyroidal hydrogen peroxide generating system was studied using porcine thyroid follicles. This system is indispensable in thyroid hormone synthesis and seems also to play an important role in regulation of thyroid hormone release. The results indicated that H2O2 generation was controlled by Ca2+ ion mobilization and protein kinase-C linked signal transduction system. It was also demonstrated that thiopental, halothane, and enflurane had inhibitory effects on H2O2 generation system in thyroid follicle cells, and 50% inhibitory concentrations (IC50) of each anesthetic

A method for measuring the molecular ratio of inhalation to exhalation and effect of inspired oxygen levels on oxygen consumption.

Using a new method for measuring the molecular ratio (R) of inhalation to exhalation, we investigated the effect of high fraction of inspired oxygen (FIO2) on oxygen consumption (VO2), carbon dioxide generation (VCO2), and respiratory quotient (RQ) in mechanically ventilated rats. Twelve rats were equally assigned into two groups by anesthetics: intravenous midazolam/fentanyl vs. inhaled isoflurane. R, VO2, VCO2, and RQ were measured at FIO2 0.3 or 1.0. R error was ± 0.003. R was 1.0099 ± 0.0023 with isoflurane and 1.0074 ± 0.0018 with midazolam/fentanyl. R was 1.0081 ± 0.0017 at an FIO2 of 0.

[Localization and function of the brainstem neuronal mechanism for respiratory control].

Recently, the neural mechanism of respiratory control in the brainstem has been extensively analyzed mainly in vitro. A neuronal group in the ventrolateral medulla, the ventral respiratory group (VRG), is important in respiratory rhythm and pattern generation. A small region in the rostral VRG, the pre-Bötzinger Complex (pre-BötC), is the kernel of respiratory rhythmogenesis. A novel region ventrolateral to the facial nucleus, the para-facial respiratory group (pFRG), was found and has been considered to also generate respiratory rhythm. These two oscillators, pre-BötC and pFRG, are coupled an
