Two patients sustained a subarachnoid haemorrhage secondary to radiation-induced vasculopathy and associated aneurysm. A 65-year-old woman was treated by radiotherapy for a stereotactically biopsied left parieto-temporal astrocytoma. Three-and-a-half years later, she suffered a subarachnoid haemorrhage secondary to a giant aneurysm arising from the middle cerebral artery bifurcation. A 44-year-old man sustained a subarachnoid haemorrhage 21 years after receiving whole brain irradiation for a large left parietal angioma. Angiography following his subarachnoid haemorrhage revealed a distal right…

A girl who had just turned one year old was transported to our hospital because of an episode of general convulsive seizure. Neurological examinations demonstrated a comatose consciousness level, moderate right hemiparesis and conjugate deviation to the right. CT scan on admission revealed an intracerebral hematoma in the left medial temporal lobe associated with intraventricular hemorrhage in the lateral and third ventricles. MRI also showed the hematoma with evidence of a dilated venous system. Angiography demonstrated a huge venous sac fed by the left posterior choroidal artery and left ant

A case of chylous ascites occurring after elective abdominal aortic aneurysm repair is described. The patient was initially managed conservatively with hyperalimentation and intermittent drainage of the retroperitoneal collection, but, this proved inadequate and a peritoneo-venous shunt was inserted. Although the shunt appeared to work well the patient finally died of septicaemia.

A 61-year-old female developed subarachnoid hemorrhage after trans-sphenoidal surgery for Rathke's cleft cyst. Neuroradiological examination revealed a large aneurysm at the C1 portion of the right internal carotid artery. Autopsy revealed marked proliferation of aspergillus hyphae in the wall of the aneurysm. A review of previously reported cases of fungal aneurysm proposes two developmental processes. Aneurysms secondary to fungal meningitis tend to be large in size and located in the major cerebral artery trunk, but aneurysms following fungal sepsis tend to be small and in peripheral branch…

Between January 1983 and March 1988 25 patients with aneurysms of the ascending aorta were operated, out of these only 15 (56%) electively. Acute incompetence (10) were the main indications for operation. Poststenotic aneurysms (2), mycotic (2) and saccular sclerotic (1) aneurysms have also been observed. A rare case of a ruptured ascending aorta due to histologically and serologically proven Brucellosis is presented. Valved conduits were used in 16 patients to reconstruct the aortic root, whereas the remaining patients had different types of supra-coronary reconstruction, i.e. resection with …

Thirteen infants were found to have atrial septal aneurysms (ASAs) on two-dimensional (2D) echocardiography and their characteristics and natural course have been studied by 2D Doppler echocardiography. Atrial septal aneurysm was accompanied by atrial septal defect (ASD) or other cardiac malformations in 12 of 13 patients, and as an isolated anomaly in one patient. Spontaneous closure of ASD by aneurysmal formation of the interatrial septum was observed in 10 patients. The aneurysms of all 13 patients showed a moderate reduction in size, 14 to 25 months (mean 18 months), after these aneurysms …

In a 23 year old female patient, hospitalized at the internal ward for precollapse condition in tachyarrhythmia seizure with signs of the Turner's syndrome and a corresponding chromosomal finding, a radiographic examination of thorax revealed, beside a spine deformity with a marked scoliosis, a spindle-like aneurysmatic dilatation of aortic arc. The duplication of its contour indicated suspected beginning dissection. The further course did not confirm it, though. The possibility of excluding atherosclerotic and luetic etiology of the aneurysm makes it possible to consider the Erdheim's cystic …

The author reviews his personal experience in the surgical treatment of 325 patients presenting intracranial aneurysms. The patients were operated at Cristo Redentor Hospital between January 1979 and December 1986. Pre and post surgical management of sub-arachnoid haemorrhage and its complications is described. Internal carotid aneurysms were 122; anterior carotid artery, 100; middle cerebral artery, 69; vertebro-basilar system, 4; and multiple aneurysms, 30. 321 of those aneurysms were approached directly. 177 patients Grade I were surgically treated, with a mortality rate of 5.08%; 78 in Gra…

45 popliteal aneurysms were surgically treated in 35 patients. In 18 cases surgery was necessitated by emergency conditions: acute arterial occlusion due to thrombosis and (or) embolism (acute critical ischaemia) in 17 cases; rupture of an aneurysm in one case. Five major amputations could not be avoided. Elective surgical intervention in 27 cases, on the other hand, only resulted in the amputation of one leg. Given the high incidence of serious complications such as thrombosis and embolism in popliteal aneurysm and the reduced success rate of surgery in acute critical ischaemia, surgical corr…

The trigeminal artery is the most common persistent carotid-basilar anastomotic channel observed in adult life, and its occurrence probably represents a defect in cerebrovascular development. It can be associated with other congenital abnormalities such as cerebral aneurysms, but only rarely do aneurysms of the primitive trigeminal artery itself arise. The authors present a new case of an aneurysm arising directly from the primitive trigeminal artery and discuss its significance and treatment.

An infant with symptoms of congestive heart failure at 6 months of age was found by cardiac catheterization at the age of 1 year to have a fusiform aneurysm of the ductus arteriosus as well as discrete stenosis of the left pulmonary artery and a secundum atrial septal defect. The aneurysm was resected, and a pericardial patch angioplasty corrected the peripheral pulmonary stenosis. The atrial septal defect was closed in a separate procedure. To our knowledge, this case is one of four ductal aneurysms reported in infants that have been diagnosed before operation and successfully treated. The pa…

The risk of late epilepsy was analyzed in a consecutive series of 177 patients operated on for supratentorial aneurysms. Late seizures occurred in 25 patients (14%); the seizures were recurrent in 21 patients (12%). Most seizures were partial, secondary generalized, or generalized tonic-clonic (72%). The mean latency between the operation and seizures was 8.4 months (range, 1 to 24 months), and in only 2 patients was the interval more than 12 months. The most important risk factors were preoperative and postoperative complications. Only 2.5% of the 81 Grade I patients developed epilepsy, compa…

A 15-year-old boy had a history of a functional systolic murmur with systolic and diastolic murmurs and radiological evidence of left ventricular hypertrophy. The angiocardiogram demonstrated an aneurysm of the membranous septum, bicuspid aortic valve, dilatation of the left subclavian artery and left common carotid artery. Echocardiography showed a holosystolic prolapse of the mitral valve with mitral insufficiency and a bicuspid aortic valve with insignificant incompetence of the aortic valve. Classical TM mode echocardiography did not show an aneurysm of the membranous septum.

Interatrial septal aneurysm is a rare abnormality and can now be diagnosed by echocardiography. We report the case of a 52 year old woman in whom this condition was diagnosed after an embolic cerebrovascular accident. M mode recordings showed a linear echo in the left atrial cavity in early and mid systole. The aneurysm was directly visualised by 2D echo as a hemispherical bulge in the mid portion of the interatrial septum, which was mobile and had a to-and-fro motion between the two atria in relation to the different phases of the cardiac cycle. The diagnosis was confirmed by angiography, and…

A 90 year old man developed the rare lesion of an aneurysm of the gluteal artery, provoked by an injury in a patient with atheromatous multiple aneurysm disease. The inflammatory and permanent nature of the sciatic pain, the history of a recent increase in size of the buttocks, as well as signs of a deglobulization suggest the need for urgent arteriographic exploration for precise definition of the arteries supplying the aneurysm. Surgical treatment is by primary ligature of the hypogastric artery associated with a direct approach in order to perform endoaneurysmorhaphy.

A rare case is reported of an hepatic artery aneurysm developing 5 months after cholecystectomy. This presented as massive haemobilia as a result of the aneurysm communicating with the cystic duct stump. The diagnosis was delayed owing to the complicating features of Waldenström's macroglobulinaemia and jejunal diverticulosis.

A case of bilateral ureteral obstruction associated with an abdominal aortic aneurysm is presented. The increasing frequency of this association suggests investigation of the upper urinary tract in patients with an abdominal aortic aneurysm and abdominal ultrasound in patients with bilateral ureteral obstruction.
