Acute Coronary Syndromes: Unstable Angina and Non-ST Elevation Myocardial Infarction.

Non-ST elevation acute coronary syndromes (NSTE-ACS) encompass the clinical entities of unstable angina and non-ST elevation myocardial infarction. Several advances have occurred over the past decade, including the emergence of new antiplatelet and antithrombotic therapies and novel treatment strategies, leading to marked improvements in mortality. However, there has also been an increased incidence in NSTE-ACS as a result of the use of high-sensitivity troponins and the increase in cardiovascular risk factors. This article provides a focused update on contemporary management strategies pertai

Unstable angina clue to coronary artery muscle bridge: a case report.

A 40-year-old man was admitted with the complaint of rest anginal episodes since few hours prior to admission. His BP was 120/80 mm Hg. ECG revealed no evidence of acute ischaemia. Coronary angiography revealed 80% luminal narrowing by systolic compression in the mid segment of the left anterior descending coronary artery best seen in the left anterior oblique cranial view. He improved symptomatically with beta-blockers, antiplatelets, statins and nitrates and the case was diagnosed as unstable angina due to coronary artery muscle bridge.

Coronary-subclavian steal syndrome: an usual cause of refractory unstable angina.

A 73 year old woman, with previous history of coronary artery bypass grafting, was admitted for refractory unstable angina. The angiography revealed a significant stenosis of the ostium of the left subclavian artery. The patient underwent successful stenting of the subclavian artery and remained symptom free until hospital discharge a few days later.

Pro-inflammatory cytokines may induce late preconditioning in unstable angina patients.

Late preconditioning can be induced by a wide variety of stimulus, including non-pharmacological and pharmacological. Thus, late preconditioning is a universal response of the heart to stress and it requires the simultaneous activation of multiple stress-responsive genes. Recently, compelling evidence has evolved that the up-regulation of pro-inflammatory cytokines plays an important role in induction of the late phase of ischemic preconditioning in rodent models. However, the role of cytokines in induction of late preconditioning in humans has not been explored. Patients with unstable coronar

Mycotic aortocoronary saphenous vein graft aneurysm presenting with unstable angina pectoris.

We report the case of a 60-year-old man with a history of coronary bypass surgery 20 years prior who had a fever, chest pain, and a mediastinal mass develop after a complicated postoperative course of abdominal aortic aneurysm resection. A mycotic aneurysm of the saphenous vein graft to his left anterior descending coronary artery was diagnosed based on blood culture results and visualization of the aneurysm before resection. A summary of the saphenous vein graft aneurysm and pseudoaneurysm cause, diagnosis, and management is detailed.

Vasodilator stress echocardiography for risk stratification of medically stabilized unstable angina.

AIMS: The aims of this study were to assess the safety, feasibility and prognostic value of dipyridamole-atropine stress echo in patients with medically stabilized unstable angina. METHODS: The initial population consisted of 173 patients consecutively admitted at two different Coronary Care Units with class IIIB unstable angina. Of these, 56 were excluded: five had poor acoustic window, 24 did not stabilize with medical therapy and underwent urgent coronary angiography, 26 evolved in non-Q wave myocardial infarction and one patient died. The remaining 117 patients underwent dipyridamole-atro

[Heart rate variability in unstable angina during 'day-to-day' monitoring].

Spectral analysis of heart rate variability was used for assessment of background autonomic activity in patients with unstable angina pectoris. There were weak linear and strong nonlinear links between autonomic activity and frequency of angina pectoris during 'day-to-day' monitoring.

"Traditional" medical therapy for unstable angina. How important? How to use?

Unstable angina comprises a heterogeneous population of patients who present with a wide spectrum of underlying pathophysiology. The traditional treatment of these patients is based on both evidenced-based medicine as well as clinical experience. Despite the large population of patients admitted with this diagnosis, the scientific literature regarding its treatment is scarce. Therefore, the management of patients with unstable angina relies heavily on the clinical skills of the physician. One of the most important steps in this process involves risk stratification, especially in the current en

[Natural history of unstable angina. Observations on 157 cases (author's transl)].

157 cases affected with "unstable angina" and hospitalized were observed over a period of from 8 to 24 months (average observation time: 16 1/2 months). The patients were treated with: nitroderivates, beta blocking drugs (when not contra-indicated); treatment of side affects (hypertension; arrhythmias, decompensation, associated pathology, correction of risk factors of coronary heart disease). 9 cases were lost and 148 were studied for the course of the illness. 10.6% died from cardiopathy (2.8% through sudden death; 7.4% from myocardial infarction); there was a 12.1% total incidence of myocar

The Material Basis and Mechanism of Xuefu Zhuyu Decoction in Treating Stable Angina Pectoris and Unstable Angina Pectoris.

METHODS: Firstly, we used a network proximity approach to calculate and compare the effectiveness of the formula with that of Western drugs for each type of angina, including all targets and intersecting targets, from a topological perspective. Secondly, we compared the mechanisms of action of the two angina pectoris at three levels and five aspects, including conventional and modular analysis approaches. Thirdly, based on the unique functions of each angina in the complex heterogeneous network, we designed a reverse process for finding the material basis using dynamic, static, and enriched it

Comparative Effectiveness of Routine Invasive Coronary Angiography for Managing Unstable Angina.

BACKGROUND: Non-ST-segment elevation acute coronary syndromes include unstable angina and non-ST-segment elevation myocardial infarction. Most randomized controlled trials of routine versus selective invasive coronary angiography have high rates of crossover from control to intervention groups and do not include subgroup analysis for unstable angina. Consequently, no clear, specific recommendations exist regarding the use of angiography in unstable angina. OBJECTIVE: To assess the effect of angiography on mortality in unstable angina, incorporating the results of additional cardiac procedures
