Radionuclide and angiographic assessment of pulmonary perfusion after Fontan procedure: comparative interim outcomes.

BACKGROUND: Right-side heart function is essentially externalized during extracardiac total cavopulmonary connection. The Fontan procedure has a certain impact on pulmonary hemoperfusion and may explain various postsurgical complications. In this study, alterations of pulmonary perfusion in patients undergoing the Fontan procedure were analyzed at the 5-year postoperative mark by radionuclide imaging and angiocardiography, and results of both methods were compared. METHODS: For 43 post-Fontan patients, perfusion ratios of each lung segment were calculated based on radionuclide imaging data. T…

Surgical treatment of atrial myxomas: a report of 20 cases.

Twenty atrial myxomas were resected in 20 patients with the use of cardiopulmonary bypass, from the beginning of July 1966 through the end o … ion; other symptoms were associated with systemic embolization. The 1st 4 patients were tested with angiocardiography alone, and 3 of these were misdiagnosed. The last 16 were tested by angiocardiography, M-mode echocardiography, and 2-dimensional echocardiography, alone or in various combinations, and there were no further misdiagnoses. In our experience, 2-dimens …

Guidelines for Clinical Use of Cardiac Radionuclide Imaging, December 1986. A report of the American College of Cardiology/American Heart Association Task Force on Assessment of Cardiovascular Procedures (Subcommittee on Nuclear Imaging).

This report describes the nuclear cardiology procedures available for use as diagnostic techniques in patients with definite or suspected cardiovascular disease. The usefulness of myocardial imaging, radionuclide angiocardiography and other radionuclide cardiovascular imaging techniques is classified within specific disease states. The clinical utility of each technique is graded from I to IV, depending on the clinical importance of the technique (I = most important; IV = not indicated). A grade of V is given for methods now considered to be in their research phase. The usefulness of these met…

Congenital absence of the pulmonary valve leaflets associated with complete transposition of the great arteries.

We describe the clinical and surgical findings of a patient with absence of the pulmonary valve leaflets and complete transposition of the great arteries. The main clinical features were a loud, single second heart sound, a loud pansystolic murmur over the entire precordial area, but no diastolic murmurs were heard. The electrocardiogram suggested right ventricular hypertrophy and right bundle branch block. The roentgenographic study showed cardiomegaly with dilatation of both branches of the pulmonary artery. Cardiac catheterization and angiocardiography showed systemic arterial desaturation,…

[Corrective surgery of double-outlet right ventricle and subaortic ventricular defect. Report of 12 cases surgically treated (author's transl)].

Experience with corrective surgery in 23 pts with double outlet right ventricle (DORV) and subaortic VSD is presented. The overall hospital mortality was 17%. Patients were divided into two groups: Group I includes seven cases without pulmonic stenosis and Group II sixteen cases with pulmonic stenosis. Six of Group II required a sistemic to pulmonary artery shunt earlier in life. Anatomic and clinical aspects in both groups are described as well as indication for surgery. The importance of angiocardiography for a precise diagnosis is emphasized. Problems related to management and complications…

Infradiaphragmatic total anomalous pulmonary venous connection to portal vein. Diagnostic implications of echocardiography.

A case of total anomalous pulmonary venous connection to the portal vein is described. The diagnosis was suspected clinically, supported by the echocardiogram, and confirmed by cardiac catheterisation, angiocardiography, and contrast echocardiography. An echo-free space lying behind the left atrium initially was thought to represent the common pulmonary vein. However, contrast echocardiography showed that this space was not the anomalous vein but probably an artefact. This paper shows that the origins of intracardiac echoes cannot always be assumed from a simple comparison of echocardiography …

Successful surgical repair of aortico-left ventricular tunnel in an infant.

Aortico-left ventricular tunnel is a rare congenital malformation in which a perivalvular tunnel produces free communication between the aortic root and left ventricle. The hemodynamic consequences of this lesion are similar to those of aortic valve incompetence. Infants with this condition have clinical findings that should suggest it as a diagnostic possibility, and aortic root angiocardiography can establish the diagnosis. Early severe congestive heart failure is common in infants with this malformation, and in such patients prompt surgical correction of the deformity is recommended. This p…

[Parietal fibroplastic endocarditis].

Parietal fibroplastic endocarditis is commonly observed in two main diseases: Löffler' endocarditis and Davies' endomyocardial fibrosis. End … ency, adiastolic syndrome. Two-dimensional echocardiography has improved the diagnosis; ventricular angiocardiography remains the reference examination. Prognosis has been largely improved these recent past years by surgical endocardiectomy and valvular conservative surgery among children.

A case of berry syndrome type 2B.

We present a case of a 34-day-old baby boy with congestive heart failure and differential cyanosis. Based on echocardiography and confirmed with computed tomography angiography, the following abnormalities were detected: aortopulmonary window, aortic origin of the right pulmonary artery, patent ductus arteriosus, and hypoplasia/coarctation of the aorta. No other congenital abnormalities were detected, and a diagnosis of Berry Syndrome type 2B was made. No preoperative cardiac catheterization or angiocardiography was needed. The patient underwent complete surgical repair of the aortopulmonary w…

Congenital unilateral pulmonary venous atresia with pulmonary veno-occlusive disease in contralateral lung: an unusual association.

A patient presenting with the rare association of congenital unilateral pulmonary venous atresia in one lung and pulmonary veno-occlusive disease in the other is described. The patient first presented at the age of 3 1/2 years with anemia, hemoptysis, and pulmonary hypertension. After cardiac catheterization and angiocardiography, a diagnosis of atresia of the left pulmonary veins was made for which left pneumonectomy was done. Four years later the patient presented with right-sided congestive failure and radiologic evidence of right-sided pulmonary edema from which death resulted. At autopsy,…

[Arterioventricular disorders in the transposition of the anterior aorta. 2 cases: with the aorta on the right and with the aorta on the left].

We show two cases of transposition of the great arteries with arterioventricular discordance and aorta at both sides (right and left). They were diagnosed by angiocardiography and verified afterwards in the anatomical analysis of the specimens. We discuss the necessary steps taken for the diagnosis in which we followed De la Cruz's rules. We emphasize that this anatomical group should be distinguished from the group of arterioventricular concordance. Both groups have the same connection. Against the opinions of other authors, we think that this connection signifies only the same physiology in …

[Large patent ductus arteriosus and interventricular communication associated with congenital absence of the pulmonary valve].

A case of pulmonary valve agenesis with ventricular septal defect and large patent ductus arteriosus is reported in a neonate. Heart failure occurred at 10 days of life; clinical examination showed continuous murmur. Echocardiography and angiocardiography visualized a major dilatation of the pulmonary artery. At cardiac catheterization there was an important left to right shunt through a large ductus, and pulmonary hypertension. There was also marked pulmonic regurgitation. Because of respiratory distress, ligation of ductus arteriosus was performed at 5 weeks of life. Five years later the pat…

[Adult pulmonary valvular stenosis, major stenosing right ventricular hypertrophy and tricuspid insufficiency. Apropos of 2 cases].

2 cases of severe pulmonary valvular stenosis in two young adults aged 21 and 29 years respectively are presented. Despite the degree of stenosis (4 mm jet), the right ventricular pressures did not exceed systemic pressures because of severe tricuspid regurgitation. This dominated the clinical and anatomical findings, causing aneurysmal dilatation of the right atrium and displacement of the tricuspid valve to the left. The diagnosis of Ebstein's anomaly was raised in one case but excluded by echocardiography and angiocardiography. In spite of the severity of the symptoms and peripheral clinica…

[Frequency and time of diagnosis of congenital heart defects in an out-patient clinic (author's transl)].

The frequency of different forms of congenital heart disease was obtained from among a group of 1066 patients seen in a paediatric-cardiological out-patient clinic, the diagnosis being confirmed by cardiac catheterisation and (or) angiocardiography. Also recorded and analysed was the time the various defects, subsequently confirmed by other methods, were first diagnosed in an out-patient clinic. It became clear that cyanotic lesions and acyanotic ones with a large left to right shunt as well as additional pulmonary stenosis were diagnosed earlier than atrial septal defect, persistent ductus ar…
