Antibiotic prophylaxis in cesarean section causing anaphylaxis and intrauterine fetal death.

Intrauterine fetal death and maternal shock occurred as a result of a type-1 hypersensitivity reaction following antibiotic prophylaxis in a cesarean section. Amniotic fluid embolism may mimic the condition. The ability to diagnose and treat such an event as early as possible is necessary in all maternity centers. The selection of antibiotic regimen and the type of anesthesia should be individualized depending upon the existing facilities and the patient's profile, especially in a resource-scarce developing country.

Antibiotic prophylaxis with azithromycin or penicillin for childhood-onset neuropsychiatric disorders.

BACKGROUND: The acronym PANDAS (pediatric autoimmune neuropsychiatric disorders associated with streptococcal infections) describes a subgroup of children with obsessive-compulsive disorder and/or tic disorder that experience symptom exacerbations following streptococcal infections. We hypothesized that the prevention of streptococcal infections among children in the PANDAS subgroup would decrease neuropsychiatric symptom exacerbations. METHODS: Twenty-three subjects with PANDAS were enrolled in a double blind, randomized controlled trial. Antibiotic prophylaxis with penicillin or azithromyci

Ceftriaxone versus ampicillin/cloxacillin as antibiotic prophylaxis in elective caesarean section.

We carried out a prospective, randomized, controlled clinical trial to evaluate the clinical efficacy of ceftriaxone and ampicillin/cloxacillin prophylaxis in decreasing the frequency of post-caesarean section infection-related morbidity. Two hundred patients randomly received either ceftriaxone (single dose) or ampicillin/cloxacillin (3 doses) intravenously at induction of anaesthesia. There was no statistical difference in incidence of endometritis (P = 0.34), wound infection (P = 0.44), or other febrile morbidity (P = 0.5). Eleven babies had a low Apgar score (< 8) in the ceftriaxone group

[Assessment of antibiotic treatment effectiveness for prophylaxis of postoperative wound infections].

Uniform prevention of postoperative infections has been introduced in the second half of 1998. Patients who underwent lung tissue resection were treated with the 2nd generation cephalosporin on the day of operation and every 8 hours within next 24 hours. Patients operated on trachea and oesophagus additionally received metronidazol. Those prevention methods were applied to 465 patients. Infections of postoperative wounds were observed only in 1.1% of patients, compared to 3.8% of patients in whom such infections were found before the beginning of the prophylactic regime.

Group B streptococcal disease in the era of intrapartum antibiotic prophylaxis.

BACKGROUND: Group B streptococcal infections are a leading cause of neonatal mortality, and they also affect pregnant women and the elderly. Many cases of the disease in newborns can be prevented by the administration of prophylactic intrapartum antibiotics. In the 1990s, prevention efforts increased. In 1996, consensus guidelines recommended use of either a risk-based or a screening-based approach to identify candidates for intrapartum antibiotics. To assess the effects of the preventive efforts, we analyzed trends in the incidence of group B streptococcal disease from 1993 to 1998. METHODS:

Intrapartum antibiotic prophylaxis increases the incidence of gram-negative neonatal sepsis.

OBJECTIVE: To investigate the influence of the increased use of intrapartum chemoprophylaxis on the incidence of vertically transmitted neonatal sepsis. METHODS: Multiple institutional databases were queried for the number of cases in which intrapartum antibiotics were used, the obstetric risk factors that were present, and the number of resultant cases of neonatal sepsis that occurred for deliveries from 1992 through 1997. Intrapartum antibiotic use was compared between the first and fourth quarter of 1997. Comparisons were made between the years 1992-1996 and 1997 for the incidence of the v

[Perioperative antibiotic prophylaxis in orthodontic bone operations of the facial skull].

Intraoral orthognathic surgical procedures are clean-contaminated operations because of the facultative pathogenic flora of the oral cavity. Without antibiotic prophylaxis in this kind of operation a postoperative wound infection can be expected in 20-31% of cases. Therefore, a retrospective analysis of the clinical course of 545 patients with various dentofacial deformities was performed to evaluate the importance of perioperative antibiotic prophylaxis. The total rate of wound infections was 2.8% and lower than rates published in other comparable studies without antibiotic prophylaxis. The r

Antibiotic prophylaxis in biliary tract surgery--current practice in The Netherlands.

A questionnaire about the current practice of antibiotic prophylaxis in biliary tract surgery was sent to the chairmen of the departments of surgery of 175 hospitals in The Netherlands. The Dutch hospitals are classified into three categories depending on the type of residency in general surgery. Overall, 80 per cent replied. Antibiotic prophylaxis in elective cholecystectomy is given in 76 per cent of the clinics (100/132), and single-dose prophylaxis is employed in 28 per cent of the clinics (28/100). In patients with acute cholecystitis, emergency surgery is the treatment of choice in 108 h

Antibiotic prophylaxis in patients with rheumatic heart disease and prosthetic devices.

Prophylaxis is defined as: "measures designed to preserve health and prevent the spread of disease" (18). Two implicit parts to such a definition are first, that what is being prevented causes disease, and second, that the measure(s) proposed prevents this disease. We will briefly review the evidence that patients with rheumatic heart disease and/or prosthetic cardiac devices are at increased risk of morbidity and mortality from infection and further that antibiotics reduce this morbidity and mortality. Antibiotic prophylaxis is undertaken in patients with rheumatic heart disease and in those

Bacteriuria After Diagnostic Flexible Cystoscopy Without Antibiotic Prophylaxis: A Prospective Study.

INTRODUCTION AND HYPOTHESIS: Flexible cystoscopy is well tolerated by women. Reprocessing of cystoscopes requires sterilization or, at least, high-level disinfection (HLD). This study aims to assess the incidence of bacteriuria after flexible cystoscopy using ortho-phthalaldehyde HLD. METHODS: This is a prospective study. Seventy-eight women undergoing diagnostic outpatient flexible cystoscopy were included. Exclusion criteria were microbiologically confirmed or acute urinary tract infection, significant urethral stricture, and bleeding disorders. Cystoscopes were reprocessed using ortho-phth

Guideline Concordance of ChatGPT-Generated Responses on Antibiotic Prophylaxis in Endodontics.

Antibiotic prophylaxis decision-making in endodontics represents a core competency in dental education, requiring the integration of systemic risk assessment, guideline interpretation, and antimicrobial stewardship principles. Although artificial intelligence (AI) tools such as ChatGPT are increasingly accessed by students and clinicians for clinical guidance, their educational reliability in teaching evidence-based antibiotic prophylaxis remains unclear. This descriptive study evaluated the accuracy and guideline concordance of ChatGPT-5 responses to predefined clinical scenarios in endodonti

Evolution toward early stent removal and reduced antibiotic prophylaxis post-cystectomy.

BACKGROUND: The ideal duration of ureteral stents in cystectomy is unknown, with trends toward earlier removal or foregoing them altogether. To reduce the duration of indwelling stents and antibiotics, we modified the cystectomy pathway at our institution from ~2 weeks of stenting with 30 days of antibiotic prophylaxis to stent removal before discharge with prophylaxis ending one day after stent removal. We evaluated rates of urinary tract infection (UTI) and urine leak in cystectomy patients after these changes. METHODS: We performed a retrospective review of patients undergoing cystectomy w

Perception and application of pre-operative surgical antibiotic prophylaxis by physicians.

Surgical site infections (SSIs) are the most common and prevalent complications occurring post-operatively leading to additional costs to the health care system. Hence, medical interns, general practitioners, surgical residents and surgeons who meet the inclusion criteria were included in this cross-sectional study. Surgical antimicrobial prophylaxis (SAP) was recorded using a structured questionnaire. Data shows that senior surgeons, scored highest and of the 21 surgical residents, 25 medical interns and 6 GPs the performances varied with overall averages of 84.37%, 76.56% and 34.37%, respect

Extended-course antibiotic prophylaxis in lower limb amputation: randomized clinical trial.

BACKGROUND: Lower limb amputation (LLA) is typically described as 'clean surgery', but surgical-site infection (SSI) rates are high and there is significant variation in antibiotic prophylaxis practice. This study aimed to determine whether an extended period of antibiotic prophylaxis (5-day course) is superior to a short course (24 h) in preventing SSI in patients undergoing minor and major LLA. METHODS: Patients undergoing minor and major amputation from a single vascular unit were recruited and randomized to receive either a 5-day or a 24-h prophylactic course of antibiotics. Patients with
