[Diagnosis, epidemiology and comorbidity of anxiety disorders].

Fears and apprehensions which significantly impair a person's everyday life and which cannot be attributed to an organic brain disturbance or to other mental disorders or physical problems are grouped together in the class of "anxiety disorders". Three types of anxiety disorders can be distinguished on the basis of clinical picture and the course of development of symptoms: phobias, panic disorder and generalized anxiety disorder. Recent epidemiological studies indicate that anxiety disorders are one of the most common kinds of mental disorder. More than a fourth of the persons surveyed in a g

Peripheral-type benzodiazepine receptors in anxiety disorders.

Peripheral benzodiazepine receptors (pBDZr) were analyzed in lymphocyte membranes from patients with anxiety disorders (generalized anxiety disorder (GAD), n = 15; panic disorder (PD), n = 10; obsessive-compulsive disorder (OCD), n = 18), other mental disorders (n = 40) and 50 healthy controls, by the specific binding of 3H-PK11195. The number of binding sites (Bmax) was significantly decreased in groups with both GAD and OCD as compared with age-matched controls, by 45% and 25% respectively, whereas the binding affinity (Kd) was the same in all disorder and control groups. Conversely, no chan

The role of personality in anxiety disorders.

It is widely believed that personality disorders and/or traits (PDT) have significant impact on the phenomenology, severity and consequently the treatment of anxiety disorders (AD). Specific PDT's are thought to be characteristic of certain types of AD's. However, little experimental data support these assumptions. The interpretation of the few comorbidity and outcome studies investigating the role of PDT's in anxiety is problematic from both theoretical and methodological points of view. The authors review what is known about the co-occurrence of PDT's and some of the AD's. Particular attenti

Mitral valve prolapse and the anxiety disorders.

Confusing and contradictory results have emerged from studies of the relationship between anxiety disorders and mitral valve prolapse (MVP), a commonly occurring heart defect that has been associated with chest pain, palpitations, tachycardia, and arrhythmias. Patients with anxiety disorders, particularly panic attacks, appear to have an increased prevalence of MVP compared with control groups or the general population, although most individuals with MVP are asymptomatic. MVP does not appear to distinguish a subgroup of patients with panic disorder on the basis of vulnerability to panic attack

Locus of control in chronic anxiety disorders.

The relationship between psychopathology and the personality trait, locus of control, was examined in 116 adult outpatients diagnosed as having chronic anxiety disorder. Measures consisted of state and trait scales, a measure of social adjustment, and a measure of disturbance during childhood. Patients with an external locus of control were more depressed, had higher levels of state anxiety, and exhibited more indecisiveness, fatigue and agoraphobia than those with an internal locus of control. Externally oriented patients also scored higher on neuroticism and trait anxiety and scored lower on

[Social Anxiety Disorder - an Update].

It can happen that one behaves in a supposedly embarrassing or insufficient manner; however, short-term feelings such as shame or self-doubt usually remain without significance. However, if there is excessive and persistent fear or significant avoidance behaviour, it may be a case of social anxiety disorder (SAS). This article provides an overview of the current status of the aetiology, diagnosis and therapy of SAS.

Interventions for generalized anxiety disorder.

PURPOSE OF REVIEW: To provide an overview of recently published work on anxiety, focusing on generalized anxiety disorder (GAD) and its treatment. RECENT FINDINGS: Self-reported anxiety symptoms were highly prevalent during the COVID-19 global pandemic in both the general population and in selected groups. There remains divided opinion about whether internet-based cognitive behavioural therapy (CBT) is noninferior to face-to-face CBT for GAD. A systematic review of drug treatment for GAD showed efficacy for selective serotonin reuptake inhibitors (SNRIs), agomelatine, and quetiapine. There ma

[The pathophysiology and diagnosis of anxiety disorder].

In addition to genetic factors, the role of epigenetic and other environmental factors in the promotion of anxiety disorder has attracted much attention in psychiatric research. When stress is encountered in the environment, the hypothalamus-pituitary adrenal system (HPA system) is activated and cortisol is secreted. CRHR gene function is closely related to this response. As a result of haplotype analysis of CRHR genes in depression and panic disorder patients, it was found that genetic polymorphism of CRHR1 and CRHR2 was related to both disorders. It is reported that abused children are more

Hydroxyzine for generalised anxiety disorder.

BACKGROUND: Generalised anxiety disorder (GAD) is a common chronic long-term psychiatric disorder, particularly frequent in primary care. There are several treatment options available, both non-pharmacological (i.e. cognitive behavioral therapy) and pharmacological. Among the pharmacological interventions, antidepressants, buspirone and benzodiazepines (BDZs) have been studied in GAD. Hydroxyzine is an anti-histamine medication which has been used in the treatment of anxiety. OBJECTIVES: 1. To determine the efficacy of hydroxyzine in comparison with placebo or any other active agent in allevi

Anxiety and outcome in bipolar disorder.

OBJECTIVE: Important differences exist between bipolar disorder with and without comorbid anxiety, but little is known about the long-term prognostic significance of coexisting anxiety in bipolar disorder. The authors sought to identify the anxiety features most predictive of subsequent affective morbidity and to evaluate the persistence of the prognostic relationship. METHOD: Probands with bipolar I or II disorder from the National Institute of Mental Health Collaborative Depression Study were followed prospectively for a mean of 17.4 years (SD=8.4) and were characterized according to variou

The treatment of social anxiety disorder.

We review the available treatments for social anxiety disorder, focusing primarily on psychotherapeutic interventions for adults, but also giving briefer summaries of pharmacological treatments and treatments for children and adolescents. The most well-researched psychosocial treatments for social anxiety disorder are cognitive-behavioral therapies (CBTs), and meta-analyses indicate that all forms of CBT appear likely to provide some benefit for adults. In addition, there are several pharmacological treatments with demonstrated efficacy, and cognitive-behavioral interventions have some demonst

Antidepressants for generalized anxiety disorder.

BACKGROUND: Pharmacological treatments have been successfully used to treat Generalized Anxiety Disorder (GAD). The mainstay for the pharmacological treatment of GAD in past decades has been the use of benzodiazepine and non benzodiazepine anxiolytics. Data emerging over the last two decades have shown that antidepressants may be equally effective to anxiolytics for treating GAD. The use of antidepressants for treating GAD may be advantageous, due to the fact that GAD presents a high co morbidity ratio with major depressive disorder (62%) and dysthymia (37%). OBJECTIVES: To assess the efficac

Antidepressants in social anxiety disorder.

Social anxiety disorder (SAD) is a marked and persistent fear of doing almost everything in front of people due to concerns about being judge by others. An up-to-date review is needed in order to reach a practical judgement of all psychopharmacological data. Case reports, open and double-blind trials with SAD were described and commented upon from a clinical point of view. The MEDLINE system was searched from 1975 to 2001. The references from the selected papers were also used as a source. MAOIs (fenelzine, tranylcypromine), reversible monoamino oxidase-A inhibitors (moclobemide, brofaromine),

Extreme anxiety/panic disorder.

A 45 year old man is referred to a specialist psychiatric centre. For 3 years he has been experiencing worsening impending fear of death and anxiety which he describes as 'panic attacks'. During these episodes he often experiences palpitations and becomes sweaty. These episodes have not been relieved despite several courses of electroconvulsive therapy. On examination, the man is alert and orientated. He has a normal blood pressure of 125/75 mmHg and electrocardiogram reveals a pulse rate of 102 beats per minute. A psychiatrist orders a number of routine tests to rule out organic disease.

Panic disorder: the ultimate anxiety.

Panic disorder, an intense exacerbation of anxiety accompanied by a variety of physical symptoms, is twice as common among women as among men. Onset is bimodal (teens/20 s and mid-30 s/40 s), 50% of cases are accompanied by agoraphobia, and the etiology is probably multifactorial. Treatment in the primary care setting includes pharmacologic (selective serotonin reuptake inhibitors) and cognitive-behavioral intervention. Medication is initiated at low doses, and the drug-evaluation trial is of 6 weeks duration. Psychiatric referral is helpful in nonresponders and in those with comorbid psychiat

The nature of social anxiety disorder.

The essential feature of social anxiety disorder (social phobia) is a fear of scrutiny by other people in social or performance situations. The level of anxiety experienced by the person with social anxiety disorder is excessive, and results in substantial impairment in the sufferer's social, family, and professional life. Three distinct subtypes of the disorder have been identified: generalized social anxiety disorder, in which the individual fears a multitude of social situations; nongeneralized social anxiety disorder, in which only 2 or 3 situations are feared; and public-speaking phobia.

[Anxiety disorder and Freud's disease].

OBJECTIVE: The author examined Freud's chest pains and arrhythmia beginning in late 1893 according to the new available data and modern psychiatry. METHOD: Published studies and recent findings were reviewed. The major works of Freud were also considered. Among the issues examined are clinical features, comorbidity, boundaries with others disorders. RESULTS: The findings of this review provided support for the dual diagnosis of panic disorder without agoraphobia and nicotine dependence. CONCLUSIONS: Freud's scientific learning was wide-ranging and his scientific ambition vast. During this p
