Utilization of Preoperative Endoscopic Airway Examination Guiding Difficult Airway Management in an Unknown Cause of Mucositis: A Case Report.

Assessing the extent of pathology in difficult airways and choosing the optimal airway management strategy in such cases can be a challenge for clinicians. Preoperative endoscopic airway examination (PEAE) is helpful in evaluating a challenging airway and formulating an airway plan in stable patients. A 52-year-old male scheduled for esophagogastroduodenoscopy (EGD) and biopsy presented with dysphagia, aphonia, mucositis, mucosal bleeding, and impaired mouth opening from pain. We were unable to complete the airway exam and were concerned about possible airway edema. PEAE was easily performed i…

[Lance-Adams syndrome in patient with anoxic encephalopathy in the course of bronchial asthma].

Lance-Adams syndrome, described in 1963, is caused by anoxia of central nervous system, generally in the course of primary respiratory failu … After successful cardiopulmonary resuscitation action myoclonus developed with cerebellar syndrome, aphonia, dysphagia and generalized convulsive seizures of tonic-clonic type. Electroencephalography showed polyspikes and complex of polyspikes-slow wave, synchronized with myoclonus. CT of the brain was normal. Action myoclonus responded appropriately to s …

[A case of bilateral upper medial medullary infarction].

A clinical case of bilateral upper medial medullary infarction was reported. A 61-year-old woman was admitted to our hospital because of numbness of trunk and bilateral upper and lower limbs, aphonia and left-hemiparesis, which progressed to quadriplegia. Facial movements were intact. Her tongue was not fully protruded and deviated to the right side. Impairment of the position sense was noted in bilateral lower limbs. Respiratory failure was not observed. A brain MRI revealed a high-intensity area on T2-weighted imaging in the upper medulla oblongata. The lesion involved the medial medulla obl…

Airway foreign bodies: a diagnostic challenge.

Foreign bodies can become lodged anywhere in the air passages, depending on their size, shape, and makeup. Symptoms of laryngeal foreign body inhalation can vary greatly but usually include one or more of the following: hoarseness, croupy cough, stridor, wheezing, dyspnea, cyanosis, hemoptysis, aphonia, odynophagia, or a subjective feeling of the presence of a foreign substance. Foreign body inhalation occurs most often in children and the elderly. The symptoms of bronchial foreign body inhalation are very similar to those of laryngeal foreign body inhalation. Usually, after the initial expres…

Clinical aspects of spasmodic dysphonia.

The clinical features of 12 patients with spasmodic dysphonia are described. In 11 patients, the voice was strained, harsh, tight, and tremu … nt, who had a familial tremor, the voice was characterised by marked breathiness, with intermittent aphonia. The disorder is probably due to a focal dystonia of the laryngeal musculature, and this would be consistent with the type of neurological disorders that were associated with it in our cases. Symptomatic benefit follows the therapeutic division of o …

Bilateral subthalamic nucleus stimulation in a parkinsonian patient with preoperative deficits in speech and cognition: persistent improvement in mobility but increased dependency: a case study.

We report a patient with advanced Parkinson's disease, including severe and frequent off periods with freezing of gait, moderate dysphonia, and some cognitive impairment, who underwent bilateral subthalamic nucleus (STN) stimulation. The patient was followed for 1 year after surgery, showing persistent good mobility without off periods and without freezing, which reverted completely when stopping the stimulation. There was deterioration of cognition as well as increased aphonia and drooling, all of which remained when the stimulation was turned off. The striking improvement in motor symptoms f…

Laryngotracheal paratubal air envelope: a useful sign in voice and airway reconstructive surgery.

Aphonia, dysphonia, and laryngotracheal stenosis with upper airway compromise are diagnosed with increasing frequency. Endoscopic surgical reconstructive techniques accompanied by tracheal T-tube stenting successfully restore voice, airway function, and structure. Timing of tracheal T-tube removal remains controversial, particularly when the tip of the upper arm of the optimally sized T-tube is placed in a critical supraglottic position. The appearance of an air envelope, manifested as linear air densities between the outside wall of the T tube and the inside wall of the larynx and trachea, ha…

Functional stridor diagnosed by the anaesthetist.

While stridor is an ominous sign implying severe airway stenosis, not all stridor has an organic aetiology. We present two cases of functional stridor in which the diagnosis was made by the anaesthetist. As experts in the management of difficult airways, anaesthetists should be aware of this clinical entity. Recurrent episodes present as aphonia, dysphonia, dyspnoea, apnoea or unconsciousness. Stridor is usually inspiratory. Flow volume loops show a pattern of variable extrathoracic obstruction with diminished peak inspiratory flow. Awake fibreoptic laryngobronchoscopy reveals normal airway an…

Antrochoanal polyp--a rare paediatric emergency.

An antrochoanal polyp, a common clinical entity, with a rare presentation is being reported. A 12 year old boy was brought to Accident and Emergency department with an unusual sudden presentation of polypoidal mass filling the oral cavity up to his incisors as a result of which he could not swallow and speak. Prior to this episode he had no complaints of the disease. An emergency removal was planned and only during the operation could it be diagnosed as a antrochoanal polyp because of its pedicle in the lateral wall of the nose, which was confirmed by histopathological examination.
