GNAS is frequently mutated in both low-grade and high-grade disseminated appendiceal mucinous neoplasms but does not affect survival.

We analyzed a series of 55 disseminated appendiceal mucinous neoplasms treated at our institution for GNAS and KRAS mutations in an attempt to correlate mutation status with clinicopathological findings and patient survival. GNAS mutations (p.R201H, c.602G>A and p.R201C, and c.602C>T) were identified in 17 (31%) of 55 of disseminated mucinous neoplasms and were found in 8 (35%) of 23 low-grade mucinous neoplasms, 7 (37%) of 19 high-grade mucinous adenocarcinomas lacking a signet ring cell component, and 2 (15%) of 13 high-grade mucinous adenocarcinomas with a signet ring cell component. A CT-based radiomics nomogram involving the cystic fluid area for differentiating appendiceal mucinous neoplasms from appendicitis with intraluminal fluid.

OBJECTIVE: To develop and validate a radiomics nomogram based on computed tomography (CT) to distinguish appendiceal mucinous neoplasms (AMNs) from appendicitis with intraluminal fluid (AWIF). METHOD: A total of 211 patients from two medical institutions were retrospectively analysed, of which 109 were pathologically confirmed as having appendicitis with concomitant CT signs of intraluminal fluid and 102 as having AMN. All patients were randomly assigned to a training (147 patients) or validation cohort (64 patients) at a 7:3 ratio. Radiomics features of the cystic fluid area of the appendice

Gross mucinous multinodular appearance aids in the identification of ovarian metastases in low-grade appendiceal mucinous neoplasms during intraoperative consultation.

Ovarian metastases of low-grade appendiceal mucinous neoplasms (LAMNs) show grossly abundant nodular mucous cells, with a gross mucinous multinodular appearance and a histological resemblance to primary ovarian mucinous tumors (POMTs). This study aimed to elucidate the utility of gross features including the gross mucinous multinodular appearance and available clinical information at the time of intraoperative consultation, in distinguishing the ovarian metastases of LAMNs from POMTs or the ovarian metastases of colorectal cancer (CRC). In total, 776 patients with primary ovarian tumor and 68

Synchronous Low-grade Appendiceal Mucinous Neoplasm and Primary Peritoneal Low-grade Serous Carcinoma: A First Description of These 2 Neoplasms Presenting Together as Suspected Peritoneal Carcinomatosis.

Low-grade appendiceal mucinous neoplasm is a neoplasm typically of appendiceal origin, which is characterized by diffuse peritoneal involvement by pools of mucin with mucinous epithelium lacking high-grade cytologic atypia, and clinically presents as suspected peritoneal carcinomatosis. A similar clinical presentation can sometimes be seen with disseminated low-grade serous carcinomas of the peritoneum, fallopian tubes, or ovaries; however, this neoplasm is histologically characterized by tubal-type epithelium and invasive or confluent growth. In this case report, we describe a patient present

A new score for predicting incidental appendiceal neoplasms in patients aged ≥40 years with acute appendicitis: a multicenter retrospective cohort study.

BACKGROUND: This multicenter retrospective cohort study aimed to develop a novel five-parameter scoring model to predict incidental appendiceal neoplasia in patients aged ≥40 years diagnosed with acute appendicitis. Previous literature has reported a significant increase in the risk of neoplasia, particularly after age 40, and this cut-off value was used as the basis for our study design. METHODS: A multicenter retrospective cohort analysis was conducted across six tertiary hospitals between January 2019 and December 2024. Adult patients aged ≥40 years who underwent appendectomy with preopera

Preoperative CT detection of appendiceal neoplasms in suspected acute appendicitis using a simple diameter-and-fecalith criterion: multicenter derivation and validation.

OBJECTIVES: Appendiceal neoplasms (ANs) may mimic acute appendicitis (AA), making preoperative recognition difficult in emergency settings. We aimed to derive and externally validate a simple non-contrast CT criterion combining appendiceal diameter and fecalith obstruction status for differentiating AN from AA. METHODS: This multicenter retrospective case-control study included adults who underwent appendectomy at four tertiary hospitals from January 2013 to June 2025. Histopathology after centralized re-review served as the reference standard. In the derivation cohort, receiver operating cha…

Survival benefits with EPIC in addition to HIPEC for low grade appendiceal neoplasms with pseudomyxoma peritonei: a propensity score matched study.

OBJECTIVES: Appendiceal cancer is a rare malignancy, occurring in roughly 1.2 per 100,000 per year. Low grade appendiceal neoplasams (LAMN) in particular can lead to pseudomyxoma peritonei (PMP), and respond poorly to systemic chemotherapy. Standard treatment includes cytoreduction surgery (CRS) with addition of heated intraoperative peritoneal chemotherapy (HIPEC). Several centres include early postoperative intraperitoneal chemotherapy (EPIC) however; the literature is mixed on the benefits. We aim to examine the benefits of additional EPIC through a propensity-matched analysis. METHODS: Pa

Laparoscopic resection of a low-grade appendiceal mucinous neoplasm in a patient with autosomal dominant polycystic kidney disease: a case report.

INTRODUCTION AND IMPORTANCE: Appendiceal mucinous neoplasms (AMNs) are rare, and preoperative diagnosis can be challenging, particularly when other abdominal cystic diseases coexist. Autosomal dominant polycystic kidney disease (ADPKD), which is characterized by multiple cystic formations, can complicate both differential diagnosis and surgical planning. CASE PRESENTATION: A 77-year-old man with ADPKD and chronic renal failure was referred to our hospital following a positive fecal occult blood test during an evaluation for anemia. Colonoscopy revealed a large submucosal tumor-like elevation

Low-Grade Appendiceal Mucinous Neoplasm (LAMN) Presenting Acutely: A Case Report and Concise Review of Surgical Decision-Making.

The optimal surgical management of Low-Grade Appendiceal Mucinous Neoplasm (LAMN) remains nuanced, requiring a balance between recurrence risk and operative morbidity. Although right hemicolectomy is frequently recommended, accumulating evidence suggests that simple appendectomy is adequate in well-defined circumstances. We report the case of a 52-year-old man with common cardiac comorbidities who presented with symptoms resembling acute appendicitis and was found to have a 10 cm LAMN. Given the intraoperative findings an intact appendix, a healthy appendiceal base, and absence of extra-append

Two cases of low-grade appendiceal mucinous neoplasm (LAMN) mimicking ovarian tumors successfully treated with laparoscopic surgery.

We report two cases of low-grade appendiceal mucinous neoplasm (LAMN) mimicking ovarian tumors that were treated with laparoscopic surgery. Case 1: A 57-year-old woman was referred to our hospital because of a pelvic mass noted during physical examination. Imaging revealed an oval, cystic, 81×39-mm mass diagnosed as a right ovarian tumor, and surgery was performed. Intraoperative findings revealed an appendiceal tumor, and laparoscopic appendectomy was conducted. The pathological diagnosis was LAMN.Case 2: A 76-year-old woman was referred to our hospital because of a pelvic mass noted during p

Bilateral borderline ovarian Brenner tumor coexisting with low-grade appendiceal mucinous neoplasm in a perimenopausal woman: a case report.

Borderline ovarian Brenner tumor (BOT-B) accounts for less than 2% of all ovarian neoplasms. It is typically unilateral and large, with non-specific imaging features. Synchronous extra-ovarian neoplasia is rarely documented. A 47-year-old gravida 2, para 2 woman presented with intermenstrual bleeding and lower abdominal discomfort. Transvaginal ultrasound and pelvic magnetic resonance imaging (MRI) revealed a 9 cm right adnexal complex mass without ascites or elevated serum tumor markers. Intraoperative findings identified bilateral, firm, lobulated ovarian tumors (right: 5 cm; left: 7 cm) and

Ten-Year Outcome of a Randomized Trial: Cytoreduction and HIPEC with Mitomycin C Versus Oxaliplatin for Appendiceal Neoplasm with Peritoneal Dissemination.

BACKGROUND: Appendiceal cancer is a rare disease that has proven difficult to study in prospectively. Our initial report of this trial showed minor hematologic toxicity with both mitomycin C and oxaliplatin and similar 3-year survival. We now report an update of the first prospective randomized trial for appendiceal cancer with 10-year follow up. PATIENTS AND METHODS: Patients with mucinous appendiceal neoplasms and evidence of peritoneal dissemination were enrolled in the Multicenter Randomized Trial to evaluating HIPEC for 120 min with oxaliplatin (200 mg/M2) or mitomycin C (40 mg). An uncommon multimodal management approach for low-grade appendiceal mucinous neoplasm with pseudomyxoma peritonei and mucinous borderline ovarian tumor.

INTRODUCTION AND IMPORTANCE: Low-grade appendiceal mucinous neoplasm (LAMN) is a rare epithelial tumor characterized by mucin production and potential dissemination into the peritoneal cavity, resulting in pseudomyxoma peritonei (PMP). The coexistence of LAMN with a mucinous borderline ovarian tumor (MBOT) represents a complex and exceedingly rare clinical scenario. Unlike the main introduction, this abstract focuses on the clinical and therapeutic significance rather than epidemiology. There are no standardized treatment protocols for synchronous LAMN with MBOT; hence, individualized manageme

A rare case of a Low Grade Appendiceal Mucinous Neoplasm in a 46-year-old Middle Eastern female - A case report.

INTRODUCTION AND IMPORTANCE: Mucinous cell neoplasia are rare and conceivably fatal causes of surgical abdomen. Adult manifestations can be obscure and ill-defined, yielding misdiagnoses and wasting precious time and effort to reach the correct one. Clinical awareness ought to be present once presented with a surgical abdomen in such a patient because this neoplasm may present with misleading presentations which mask the actual diagnosis and masquerade as a different one, and in turn, may result in performing a dissimilar treatment intervention. Rapid learnt surgical judgments must be taken an
