[Sulpiride induced dystonia in a patient with Tourette syndrome. A stroke-mimicking presentation].

We present a case of a 23-year-old male Caucasian patient admitted to the emergency department because of an acute onset of difficulty of articulation, weakness of the left arm, throat- and neck pain. An emergency CT & MRI of the brain showed no abnormalities. The Patient had started visiting a new neurologist three weeks before admission and received Sulpiride against Tourette syndrome (TS) in a rapid escalation manner over a short period. Sulpiride induced dystonia and other neurological symptoms that were clinically masked by dystonic and clonic tics of the known TS. 5 mg Biperiden (anticho

Speech Therapy for Children with Cleft Lip and Palate Using a Community-Based Speech Therapy Model with Speech Assistants.

OBJECTIVE: Evaluate the speech services using a Community-Based Speech Therapy model by trained speech assistants (SAs) on the improvement of articulation in cleft palate children. MATERIAL AND METHOD: Seventeen children with repaired cleft palates who lived in Chiang Rai and Phayao provinces were registered to the camp. They received speech therapy with a 4-day intensive camp and five follow-up camps at Chiang Rai's The Young Men's Christian Association (YMCA). Eight speech assistants (SAs) were trained to correct articulation errors with specific modeling by the speech-language pathologists

Developmental language and speech disability.

Speech disabilities (articulation deficits) and language disorders--expressive (vocabulary) receptive (language comprehension) are not uncommon in children. An overview of these along with a global description of the impairment of communication as well as clinical characteristics of language developmental disorders are presented in this article. The diagnostic tables, which are applied in the European and Anglo-American speech areas, ICD-10 and DSM-IV, have been explained and compared. Because of their strengths and weaknesses an alternative classification of language and speech developmental

Speech Sounds Production, Narrative Skills, and Verbal Memory of Children with 22q11.2 Microdeletion.

22q11.2 deletion syndrome (22q11.2DS), the most frequent microdeletion syndrome in humans, is related to a high risk of developing neurodevelopmental disorders. About 95% of patients with 22q11.2DS have speech and language impairments. Global articulation, story generation, and verbal memory tests were applied to compare articulatory characteristics of speech sounds, spontaneous language abilities, and immediate verbal memory between four groups of Serbian-speaking children: patients with 22q11.2DS, children with clinical presentation of 22q11.2DS that do not have the microdeletion, children w

Deep learning applications in telerehabilitation speech therapy scenarios.

Nowadays, many application scenarios benefit from automatic speech recognition (ASR) technology. Within the field of speech therapy, in some cases ASR is exploited in the treatment of dysarthria with the aim of supporting articulation output. However, in presence of atypical speech, standard ASR approaches do not provide any reliable result in terms of voice recognition due to main issues, including: (i) the extreme intra and inter-speakers variability of the speech in presence of speech impairments, such as dysarthria; (ii) the absence of dedicated corpora containing voice samples from users

Temporal dissociation of early lexical access and articulation using a delayed naming task--an FMRI study.

Neuroimaging studies of overt speech hold an important practical advantage allowing monitoring of subject performance, particularly valuable in disorders like aphasia. However, speech production is not a monotonic process but a complex sequence of stages. Levelt and colleagues have described these as roughly corresponding to two originally independent systems--conceptual and sensorimotor--that are linked in the formulation and expression of spoken language. In the initial stages a word is chosen to match a concept (lexical selection); in the later stages the sound and motor patterns are encode

Whole Genome Analysis in Consanguineous Families Reveals New Loci for Speech Sound Disorder (SSD).

Speech is the most common means of communication in humans. Any defect in accurate speech production ability results in the development of speech sound disorder (SSD), a condition that can significantly impair an individual's academic performance, social interactions, and relationships with peers and adults. This study investigated the genetic basis of SSD in three Pakistani families. We performed family-based genome-wide parametric linkage analysis and homozygosity mapping in three consanguineous families with SSD from the Punjab province of Pakistan. The Test for Assessment of Articulation a

Automatic vocal tract landmark localization from midsagittal MRI data.

The various speech sounds of a language are obtained by varying the shape and position of the articulators surrounding the vocal tract. Analyzing their variations is crucial for understanding speech production, diagnosing speech disorders and planning therapy. Identifying key anatomical landmarks of these structures on medical images is a pre-requisite for any quantitative analysis and the rising amount of data generated in the field calls for an automatic solution. The challenge lies in the high inter- and intra-speaker variability, the mutual interaction between the articulators and the mode

[Speech improvement with velopharyngoplasty in patients with lip-jaw-palate clefts].

BACKGROUND: Despite successful closure of the hard and soft palate and intensive speech therapy a velopharyngeal insufficiency is not completely avoidable in each case of cleft palate. An improvement by velopharyngoplasty should be possible. PATIENTS AND METHODS: Two hundred and ninety patients suffering from cleft palate were examined before and one year after velopharyngoplasty according to Sanvenero-Rosselli. Four main symptoms of the affected speech were assessed: changes of the resonance, inappropriate nasal air emission, articulary disorders (deviations from articulation areas), and non

[Speech rate of compensatory articulation in patients with cleft lip and palate].

Performance analysis of 2D and 3D image features for computer-assisted speech diagnosis of dental sibilants in Polish children.

BACKGROUND AND OBJECTIVE: Sigmatism is a speech disorder concerning sibilants, and its diagnosis affects many Polish children of preschool age. The success of therapy often depends on early and accurate diagnosis. This paper presents research findings on using 2D and 3D (time-related) visual features to analyze the place of articulation, sibilance (the character of a gap between teeth that allows the articulation of sibilant sounds), and tongue positioning in four of twelve Polish sibilants:/s/,/z/,/ʦ/, and/dz/. METHODS: A dedicated data acquisition system captured the stereovision stream dur

The supplementary motor region and speech emission.

Speech disorders occur with left supplementary motor region (SMR) lesions. Right SMR lesions probably produce speech disorders only when the right cerebral hemisphere is speech-dominant. Paroxysmal SMR dysfunction produces both uncontrollable vocalization and speech arrest. Slowly evolving SMR lesions produce reduced and labored speech output with word-finding difficulties. Suddenly acquired SMR lesions produce marked speech suppression and aphonia; during recovery nonpropositional "automatic" speech may be initiated easily, even involuntarily, while initiation of propositional speech is still

A physiological analysis of articulatory dysfunction in dysarthric speakers following severe closed-head injury.

A physiological analysis of the lip and tongue function of a group of 18 severely closed-head injured (CHI) subjects was performed using lip and tongue force transduction systems. Lip and tongue strength, endurance and rate of repetitive movements were assessed on four non-speech tasks: maximum strength, sustained maximum strength, repetition of 10 maximum strength compressions, and maximum repetition rate. The values recorded were compared to those achieved by a group of 18 non-neurologically impaired subjects, matched for age and sex, who served as the controls. The results of the study indi

Pattern-based approaches to phonological therapy.

Pattern-based approaches to phonological disorders emerged in the 1970s and 1980s after researchers realized that phonological "rules" used to describe speech patterns of adults could be used in the analysis of children's speech. In this article, a pattern-based approach to assessment and treatment of phonological disorders is described, based on identification and treatment of phonological processes. Means of identifying and quantifying phonological processes are provided and assessment tests are described. Principles underlying the use of phonological processes in intervention programs are p

Explaining speech production deficits in poor readers.

The purpose of the present study was to further examine speech production abilities of young poor readers. Fourteen poor readers and 14 age-matched nondisabled subjects were taught to produce four novel, multisyllabic nonsense words. A recognition task was part of the training procedure. Retention of the words was also probed. The poor readers took significantly longer than the nondisabled children to produce three of the four words. The recognition data indicated that encoding limitations, rather than speech production limitations, were primarily responsible for the longer acquisition times.
