Previous studies have found that subjects diagnosed with verbal auditory agnosia (VAA) from bilateral brain lesions may experience difficulties at the prephonemic level of acoustic processing. In this case study, we administered a series of speech and nonspeech discrimination tests to an individual with unilateral VAA as a result of left-temporal-lobe damage. In order to evaluate the frequency selectivity of the 500 Hz FFR we applied narrow-band masking at closely spaced center frequencies in the range 0.25 to 4 kHZ. The results show clearly that it is the apical part of the cochlea that generates the 500 Hz FFR, but the area of the basilar membrane that contributes is rather broad. It was evident that some of the masking effect that was observed in the Vertex-derived response stemmed from as yet obsure brain stem mechanisms. We report the case of a patient who developed severe left auditory neglect following a cerebral vascular accident with infarction of areas in the frontal, temporal, and parietal lobes of the right hemisphere. The severity of neglect was documented using pure tones, speech, and environmental stimuli, administered binaurally and dichotically. PURPOSE OF REVIEW: In recent years, parents and schools have spent millions of pounds and dollars on commercial programs that claim to treat children's developmental disorders by training their auditory processing.
