Subclavian and axillary vessel anatomy: a prospective observational ultrasound study.

PURPOSE: The primary objective of this study was to define the ultrasound-derived anatomy of the axillary/subclavian vessels. As a secondary … tients with a body mass index as high as 46.4 kg·m-2. The mean (standard deviation) diameter of the axillary vein was 1.2 (0.3) cm on the right side and 1.1 (0.2) cm on the left side. The dimensions of the axillary vein were larger on the right side in 69% of patients. The vein was located directly over the artery in the mid-clavicular view in 67% of the patien …

Axillary grafts for difficult hemodialysis access.

PURPOSE: This study describes the largest reported experience to date with axillary artery-to-axillary vein or axillary artery-to-jugular vein polytetrafluoroethylene bridge fistulas for hemodialysis access. The purpose of the study was to determine the incidence of complications and the durability of the access to better determine the role of this procedure in the dialysis access algorithm. METHODS: A single center's experience over a period of 5 years was retrospectively reviewed. RESULTS: Twenty-six axillary grafts were placed in 24 patients. All but one were used for dialysis. At the tim…

Secondary arteriovenous malformation due to subclavian vein occlusion.

An 80-year-old man underwent rectal resection and insertion of a central venous catheter through the left subclavian vein 16 years earlier. Following surgery, he developed edema of his left upper limb that became exacerbated and infected. Computed tomography showed occlusion of the subclavian vein and multiple arteriovenous shunts from the branches of the axillary artery to the venous sac of the axillary vein. Angiography confirmed numerous shunts between the branches of the axillary artery and vein and dilated collateral veins. Embolization of the venous sac was performed using coils, alcohol…

Surgical landmarks for identification of the thoracodorsal pedicle during axillary dissection in carcinoma breast patients: a prospective observational study.

BACKGROUND: Axillary lymph node dissection (ALND) plays a pivotal role in breast cancer surgery, serving both staging and locoregional contr … the pectoralis minor muscle were measured using vernier calipers. The clock-face orientation of the axillary vein was also employed to describe the relative position of the lateral thoracic vein (LTV) and TDP. RESULTS: In all 30 patients, the lateral thoracic vein (LTV) was consistently identified at the anteroinferior aspect of the axillary vein (9 o'clock po …

The use of confirming x-rays to verify tip position for peripherally inserted catheters.

The tips of long-line peripherally inserted catheters may be positioned in the superior vena cava (SVC), subclavian vein, or axillary vein. Based on current standards of practice, catheters with tips placed in the SVC are central lines that require a confirming x-ray; catheters with tips terminating in the subclavian or axillary vein are noncentral and do not require confirming x-rays. Several authors follow this standard; others, however, suggest that radiographic examination should be performed on catheters with tips lying in locations other than SVC unless numerous specific criteria are met…

Unusual variability of the superficial venous system of the upper limb and its consequences for deep venous system.

In our report we would like to present a variation of the two main superficial veins of the upper limb. In 21 (88%) explored upper limbs, th … stead of connecting with the second one, it gave rise to an independent second vein - the accessory axillary vein. This uniqueness was based on that, independently from deep venous system, the superficial one formed their own net. The accessory axillary vein and the second brachial vein were completely formed by two main superficial veins: the basilic and cepha …

Phleborheography of the upper extremity.

Phleborheography is a well-established technique for the noninvasive diagnosis of deep venous thrombosis of the lower extremity. For ten years we have used phleborheography in the diagnosis of axillary and subclavian venous thrombosis. We developed modifications in technique and interpretation that are necessary for accurate application of phleborheography to the upper extremity. When compared with venography, the sensitivity and specificity of this technique approached 90%. Therefore, phleborheography appears to be a useful screening examination for deep venous thrombosis of the upper extremi

Refractory axillary venous spasm during permanent pacemaker implantation.

BACKGROUND: Vascular spasm is well known to occur in the arterial system. Central venous spasm during pacemaker implantation is uncommon wit … ial vein, which showed good-sized axillary and subclavian veins. We attempted to cannulate the left axillary vein with a 16G needle using Seldinger technique, but the axillary vein could not be cannulated despite multiple attempts. We gave incremental boluses of intravenous nitroglycerine, despite that the left axillary vein could not be cannulated. Repeat intr …

First rib and venous anomalies - Anatomical challenges for transvenous implantation of cardiac electronic devices.

Atypical anatomy may be encountered unexpectedly and undiagnosed in clinical practice, and this is especially important during the performance of interventional procedures such as transvenous implantation of cardiac electronic devices. The body of the first rib can be absent. If this not noticed, pneumo-/haemothroax may be induced during subclavian vein puncture as the needle may enter the first intercostal space rather than the costo-clavicular angle. The cephalic vein may pursue a supraclavicular course, the axillary vein may drain into an intercostal vein rather than the axillary vein, and …

Layered relationship of Langer's axillary arch with the axillary neurovascular bundle: a cadaveric case report.

BACKGROUND: Langer's axillary arch (LAA) is a common muscular variant in the axilla, yet detailed descriptions of its layered and level-depe … layer. More superiorly, the tendinous portion crossed the brachial vein near its junction with the axillary vein; notably, the brachial vein was interposed between the tendinous LAA and the region where the lateral and medial cords of the brachial plexus converged, after which the LAA passed over the surface of the lateral cord before insertion. CONCLUSIONS: …

Deep venous thrombosis of the upper extremity: diagnosis and treatment.

Deep vein thrombosis of the upper extremity was long thought to be a benign disease, rarely complicated by pulmonary embolism and associated with minimal long-term morbidity. More recent observations have demonstrated however, that a significant number of patients will continue to have disabling symptoms after treatment with conservative measures and standard anticoagulation therapy, and that pulmonary embolism can occur in the course of the disease. Because of its significant morbidity and increasing incidence, an aggressive emergency department approach to diagnosis and early consideration o
