Inflammatory and malignant thickening of the bile duct wall can be assessed without difficulty. We present a case of a 70-year old American Indian who had a previous cholecystectomy for symptomatic cholelithiasis and who presented, one year later, with progressive painless jaundice and a dilated common bile duct. Work-up revealed an adenocarcinoma of the extrahepatic bile duct. In this article we discuss the common presentation, work-up, and treatment of carcinoma of the bile ducts.

Papillomatosis of the biliary ducts is exceptional. It is defined by the presence of multiple, benign, papillary type, epithelial tumors on the choledochus and hepatic ducts, and can also effect the gallbladder and intrahepatic bile ducts. It courses with a tendency to recurrence and secondary degeneration, and its prognosis is uncertain and sometimes grave. After examining the literature on the subject, all admissions to the Pavia University Department of Surgical Pathology from 1-1-1974 to 31-12-1985 belonging to the IIIrd age and suffering from non-neoplastic and neoplastic bile way pathology have been reviewed. The diagnosis and treatment of 18 patients with carcinoma of the extrahepatic bile duct are reported. All patients were worked up by USG, PTC or ERC before operation. Tumor was resected in 3 patients, palliative internal or external drainage of the biliary tract was performed in 10, 1 was explored and 4 received PTCD treatment. One of the patients with the tumor resected has survived for 2 years. Mean survival time after the operation was 5.6 months for patients treated by drainage. The authors believe that PTC and ERC play the most important role in diagnosing carcinoma of the biliary tract. …

Carcinoma of the extrahepatic bile ducts is relatively uncommon. Although these tumors usually grow slowly, the patient's prognosis is poor. This article reviews the records of patients with a diagnosis of cancer of the extrahepatic biliary tract treated surgically at Scott and White Memorial Hospital from 1960 to 1984. The pertinent history, physical findings, laboratory data, type of operation, adjuvant therapy, and survival were collected and recorded.

The study of bile composition in thalassemic patients revealed a lithogenic bile even without gallstones. The lithogenic index was higher than in a group of colelithiasic non hemolytic patients. Seventeen patients have completed a double-blind cross-over study of hydrotalcite against placebo in the treatment of bile vomiting after surgery for peptic ulcer. Overall there was no significant difference between the two treatments, with 9 patients improving on hydrotalcite and 5 on placebo. The original operation had been peformed less than 3 years before the study in 9 patients; in this subgroup there was an improvement on hydrotalcite treatment in 8 patients but in only 1 on placebo.
