A 43-year-old man with liver cirrhosis received transjugular intrahepatic portosystemic shunt (TIPS) for the treatment of recurrent variceal … bile duct. After the successful TIPS revision with the placement of an additional stent-graft, the biliary fistula and common bile duct were no more delineated. We herein report a rare case with an obvious visualization of biliary-TIPS fistula associated with obstruction of TIPS shunt on the tractogram and recanalization with an additional stent-graft.

Mirizzi syndrome type II is a form of obstructive jaundice caused by a stone impacted in the gallbladder neck or the cystic duct that impinges on the common hepatic duct with a cholecysto-choledochal fistula. Preoperative recognition is necessary to prevent injury to the common duct during surgery. We present a patient with an operative diagnosis of type II Mirizzi syndrome, which was not originally indicated in the preoperative work-up; in particular endoscopic retrograde cholangiopancreatography showed stenosis of the middle third of the hepatic duct along with markedly elevated serum CA19-9

INTRODUCTION: A bronchobiliary fistula (BBF) is a rare pathological communication between the biliary tract and the bronchial tree. It is most often associated with hepatic tumours, infections or trauma. The diagnosis can be challenging; management strategies vary and include conservative, surgical and endoscopic approaches.
CASE DESCRIPTION: A 61-year-old woman with metastatic colorectal cancer to the liver -managed with hepatic resections and ablation - presented with progressive dyspnoea and a productive cough. She had a recent history of pyogenic liver abscess treated with antibiotics. A …

BACKGROUND: Biliary fistulas typically occur as surgical complications after laparoscopic cholecystectomy, liver transplantation, or partial liver resection.
AIMS: This study aimed to evaluate the efficacy of the endoscopic treatment of biliary fistulae secondary to liver transplantation compared to that of other etiologies.
METHODS: A retrospective study of 25 patients undergoing endoscopic retrograde cholangiopancreatography for biliary fistula from 2015 to 2021 was conducted at the Endoscospy Unit of Walter Cantídio University Hospital. Clinical characteristics and endoscopic success rate…

Although laparoscopic cholecystectomy is considered to be the gold standard for treatment for symptomatic cholelithiasis, it is associated with an increased risk of biliary and vascular injury compared to the traditional technique. Massive hemobilia is a rare but potentially life-threatening cause of upper gastrointestinal hemorrhage. Arterio-biliary fistula is an uncommon cause of hemobilia. We describe a case of cystic artery pseudo-aneurysm causing arterio-biliary fistula and presenting as severe melaena and cholangitis that occurred 7 months after laparoscopic cholecystectomy. Gastroduoden…

A total of 1293 patients who underwent ERCP between 1980 and 1990 were reviewed. A parapapillary choledochoduodenal fistula (PCDF) was noted in 42 patients (3.2%). In order to study the frequency of PCDF, the patients were divided into groups of age of ten years. As a result, PCDF was formed to increase with advance in age. Thus the dispose is regarded as a "later on" disease. Thirty-one of these patients were included in the definite PCDF group, in which the distance between the papillar and fistulous opening was confirmed by cannulation or choledochography, and 11 in the questionable PCDF gr

Hepatic arterioportal fistulas (HAPFs) are aberrant shunts between the portal venous and the hepatic arterial systems leading to portal hypertension (PH). Acquired post-biopsy HAPFs are rare and most of them resolve spontaneously with no clinical manifestations. If symptomatic, they should be treated, and endovascular embolization represents the standard of care. We present the first case of an acquired high-flow post-biopsy HAPF causing clinically significant PH and PH-related bleeding in a 26-year-old female patient affected by early-stage primary biliary cholangitis (PBC). The HAPF was safe

We report a case in which a spontaneous choledochoduodenal fistula occurred after biliary covered self-expanding metal stent (SEMS) placement and a late transfistula migration of the stent in a patient with malignant distal biliary obstruction. A partially covered WallFlex biliary stent (Boston Scientific) was appropriately implanted in the common bile duct. Subsequently the patient received chemotherapy with gemcitabine. After 7 months of the SEMS insertion, the patient presented with frequent vomiting. Abdominal computed tomography revealed the obstruction of the duodenal descending part and

Background and Objectives: Pancreatoduodenectomy (PD) is the primary treatment for patients with resectable, non-metastatic pancreatic adenocarcinoma and periampullary tumors. Although surgical methods and perioperative management have improved, the procedure still carries a high risk of complications, with postoperative pancreatic fistula (POPF) being the most significant. This study focuses on identifying current risk factors for POPF after PD in a single HPB center. Materials and Methods: We retrospectively analyzed prospectively collected data from patients undergoing PD in our department

BACKGROUND AND AIMS: Gastrogastric fistula (GGF) is a known rare adverse event after gastric bypass surgery. Management of refractory GGF, either surgical or endoscopic, is associated with poor long-term results. There is limited evidence on the successful management of refractory GGF with endoscopy.
METHODS: A 62-year-old female patient status post Roux-en-Y gastric bypass presented with symptoms of aspiration. She not was responsive to repeated endoscopic and laparoscopic methods for fistula closure, which made her unfit for surgical intervention. We decided to proceed with an endoscopic at
