The effects of primary thoracic blast injury and morphine on the response to haemorrhage in the anaesthetised rat.

Primary thoracic blast injury causes a triad of bradycardia, hypotension and apnoea mediated in part via a vagal reflex. Blast casualties may also suffer blood loss, and the response to progressive simple haemorrhage is biphasic: an initial tachycardia followed by a vagally mediated reflex bradycardia which can be attenuated by micro opioid agonists. The aims of this study were to determine the effects of thoracic blast injury on the response to subsequent haemorrhage, and the effects of morphine, administered after blast, on the response to blood loss. Male Wistar rats, terminally anaesthetis

Reflex nature of the cardiorespiratory response to primary thoracic blast injury in the anaesthetised rat.

Blast injuries represent a problem for civilian and military populations. Primary thoracic blast injury causes a triad of bradycardia, hypotension and apnoea. The objective of this study was to investigate the reflex nature of this response and its modulation by vagotomy or administration of atropine. The study was conducted on terminally anaesthetised (alphadolone/alphaxalone, 18-24 mg x kg x h(-1), I.V.) male Wistar rats randomly allocated to the groups indicated below. Blast injuries were produced with compressed air while sham blast involved the sound of a blast only. Primary blast injury …

Perforation of the terminal ileum induced by blast injury: delayed diagnosis or delayed perforation?

Blast injuries are rare, and although blast-induced perforations of the bowel have been described in the past, the entity of a delayed perforation caused by an evolving injury has not been reported. We report three men injured by the explosion of a terrorist bombing in open air. They suffered primary blast injuries, which resulted in isolated perforations of the terminal ileum. They were operated at different times after the blast event. The resected specimens were examined under light microscopy. One patient was operated immediately, and had three perforations in the terminal ileum. In the ot…

Chest wall velocity as a predictor of nonauditory blast injury in a complex wave environment.

Previous blast injury prediction criteria have been based on exposure to classic Friedlander or ideal blast waves. An ideal waveform is characterized by an instantaneous rise to a peak overpressure that decays exponentially to ambient pressure followed by a negative phase. The prediction criteria did not address injuries resulting from exposure to complex blast waves. It was difficult to establish a simple relationship between the two because complex blast waves typically consist of multiple shocks with variable frequency content and intensity that may be superimposed on a slow rising quasista

Pattern of injury in those dying from traumatic amputation caused by bomb blast.

Traumatic amputation of limbs caused by bomb blast carries a high risk of mortality. This paper describes 73 amputations in 34 deaths from bomb blast in Northern Ireland. The principal aim was to determine the sites of traumatic amputation to provide a biophysical basis for the development of protective measures. Few amputations were through joints; nearly all were through the bone shafts. The most common site in the tibia was the upper third. The distribution of femoral sites resulting from car bombs differed from that characterizing other types of explosion. For car bombs the principal site

[Effectiveness of applying the bioacoustic correction method in therapy of phantom pain syndrome in amputees after mine-blast injuries].

UNLABELLED: Phantom pain syndrome (PPS) has attracted the attention of scientists and physicians for many years. In recent decades, the number of patients with PPS has increased due to the growing number of victims of local conflicts, natural disasters, and industrial accidents, as well as the rise in occlusive vascular diseases of the lower extremities. Given the pathogenesis of PPS, treatment for this condition is aimed at suppressing the pathological activity of peripheral pacemakers and hyperexcitable neuronal aggregates and activating the antinociceptive system. Bioacoustic correction, an

Identification of Serum Biomarkers for Blast-induced Traumatic Brain Injuries: Low vs. High-intensity Exposure in a Rat Model.

BACKGROUND/AIM: Traumatic brain injury (TBI), particularly from blast exposures, is a growing global health concern due to increasing international conflicts and terrorist attacks. Blast exposure involves whole-body impact; however, the specific effects on the brain remain inadequately defined. This study aimed to identify serum-based protein biomarkers that could serve as non-invasive indicators for early detection and prediction of long-term outcomes of blast-induced TBI (bTBI), and to assess other organ-specific responses. Focus was given to proteins associated with inflammation, mitochondr

Blast injuries found on the exhumed remains of Polish postwar partisans killed by the Polish security service in 1946.

Exhumed, badly decomposed human remains constitute very difficult research material. It requires a great deal of caution and critical judgment to analyze the nature of injuries, especially those resulting from an explosion. Scientific publications in the field of traumatology concerning injuries caused by the action of explosives mainly focus on the effects of contemporary terrorist or military activities. The results of this research regard mainly clinical medicine and the organization of healthcare. This article discusses the problems and solutions of determining the presence of injuries cau

Reduction in Temporary and Permanent Audiological Injury Through Internal Jugular Vein Compression in a Rodent Blast Injury Model.

HYPOTHESIS: Internal jugular vein (IJV) compression influences not only intracranial but also intracochlear physiology and has demonstrated preclinical effectiveness in reducing acute audiological injury in a rodent blast model. However, the long-term effects in this model are unknown. BACKGROUND: Blast wave-induced audiological injury from an improvised explosive device is a leading cause of morbidity among service members in theater but there are limitations to the current protective measures. METHODS: For this study, we exposed 20 Sprague Dawley rats to a 16.8 ± 0.3 PSI (195.3 dB SPL) rig

Successful treatment of burn and visceral injury combined with full-thickness loss of the abdominal wall after blast injury.

A 29-year-old man was admitted to our institution 10 days after he had undergone an urgent exploratory laparotomy at a local army hospital after a terrorist bombing attack. On admission, deep second-degree and third-degree burns involving 25% of the upper and lower extremities were present, together with a 25 x 10-cm abdominal full-thickness blast injury defect on the left side, an infected eviscerated midline incision, and a colostomy on the right side of the abdomen. The patient underwent a second laparotomy, at which time the intraabdominal abscess was drained, and the abdominal cavity was

Temporal differential effects of post-injury alcohol consumption in a mouse model of blast-induced traumatic brain injury.

Traumatic brain injury is a prevalent condition that affects millions worldwide with no clear understanding or effective therapeutic management available. Military soldiers have a high risk of exposure to blast-induced traumatic brain injury (bTBI). Furthermore, alcohol drinking is common in this population, and studies have shown that post-TBI alcohol exposure can result in memory loss. Hence, it is possible that alcohol could contribute to the overall pathological outcome of brain trauma. However, such a possibility has not been explored in detail. Here, we combined a mild bTBI (mbTBI) model

Primary Blast-Induced Traumatic Brain Injury as a Risk Factor for (Cerebro)vascular Disorder: Clinical Manifestations, Blast Physics, Biomechanics, Pathobiology, and Critical Gaps.

Exposure to blast waves without kinetic, penetrating, thermal, or toxic components causes a distinct form of traumatic brain injury, termed primary blast-induced TBI (pbTBI). Clinical manifestations of pbTBI span a wide spectrum, ranging from life-threatening intracranial hemorrhage, hyperemia, and delayed cerebral edema to mild and transient neurological symptoms without detectable structural abnormalities on routine imaging. At the mild end of the spectrum, symptoms after a single exposure may resolve quickly, yet repeated exposures-even at very low levels, termed "subconcussive"-can develop

High Lifetime Blast Exposure Using the Blast Exposure Threshold Survey Is Associated With Worse Warfighter Brain Health Following Mild Traumatic Brain Injury.

The purpose of this study was to extend previous research by examining the relationship between lifetime blast exposure and neurobehavioral functioning after mild TBI (MTBI) by (a) using a comprehensive measure of lifetime blast exposure, and (b) controlling for the influence of post-traumatic stress disorder (PTSD). Participants were 103 United States service members and veterans (SMVs) with a medically documented diagnosis of MTBI, recruited from three military treatment facilities (74.8%) and community-based recruitment initiatives (25.2%, e.g., social media, flyers). Participants completed

Safety and Feasibility of Early Aquatic Physical Therapy for Military Survivors of Gunshot Wounds and Blast Injuries: A Case Series.

This study evaluated the safety and feasibility of early aquatic physical therapy (APT) for gunshot wounds (GSWs) and/or blast injuries (BIs). Early aquatic physical therapy (eAPT) is defined as "therapy conducted within the first month after injury, considering the injury type, setting, and environment." APT facilitates the restoration of mobility and function after illness or injury. Although APT is helpful for patients with mobility issues, muscle weakness, and pain, its early use in high-functioning adults recovering from ballistic trauma remains underexplored. Usually, multidisciplinary r…

Metformin mitigates gas explosion-induced blast lung injuries through AMPK-mediated energy metabolism and NOX2-related oxidation pathway in rats.

Gas explosions are a recurrent event in coal mining that cause severe pulmonary damage due to shock waves, and there is currently no effective targeted treatment. To illustrate the mechanism of gas explosion-induced lung injury and to explore strategies for blast lung injury (BLI) treatment, the present study used a BLI rat model and supplementation with metformin (MET), an AMP-activated protein kinase (AMPK) activator, at a dose of 10 mg/kg body weight by intraperitoneal injection. Protein expression levels were detected by western blotting. Significantly decreased expression of phosphorylate
