Drug Delivery Challenges and Current Progress in Nanocarrier-Based Ocular Therapeutic System.

Drug instillation via a topical route is preferred since it is desirable and convenient due to the noninvasive and easy drug access to different segments of the eye for the treatment of ocular ailments. The low dose, rapid onset of action, low or no toxicity to the local tissues, and constrained systemic outreach are more prevalent in this route. The majority of ophthalmic preparations in the market are available as conventional eye drops, which rendered <5% of a drug instilled in the eye. The poor drug availability in ocular tissue may be attributed to the physiological barriers associated wi

Control of subretinal fluid: experimental and clinical studies.

Experimental work shows that subretinal fluid is removed both by active transport across the retinal pigment epithelium (RPE) and by passive hydrostatic and oncotic forces that work most effectively when the RPE barrier has been damaged. The retina will stay attached whether or not the RPE is intact--but retinal function requires the RPE barrier and thus active transport is the primary mechanism of subretinal fluid control. RPE fluid transport is normally limited by the retina (which resists water flow from the vitreous) but can be quite powerful when a reservoir of subretinal fluid is present

Animal modeling of cystoid macular edema.

Primate models of cystoid macular edema (CME) were developed along four sets of presumed primary causative factors: 1) disruption of the blood-retinal barrier at the retinal vasculature and the retinal pigment epithelium; 2) ischemic tissue injury with cyst formation; 3) intraocular events frequently observed in patients with CME, such as inflammation, ocular hypotony, and vitreous traction; and 4) the probable additive effect on the macula by systemic diseases such as diabetes mellitus and systemic hypertension. While each experiment produced some aspects of CME in the human, none gave the co

Autosomal dominant vitreoretinochoroidopathy (ADVIRC).

We report the second family recognised to have autosomal dominant vitreoretinochoroidopathy. The clinical features were (1) autosomal dominant inheritance; (2) peripheral, coarse pigmentary degeneration of the fundus for 360 degrees, with a relatively discrete posterior border in the equatorial region (this finding may be pathognomonic); (3) superficial punctate yellowish-white opacities in the retina; (4) various vascular abnormalities; (5) breakdown of the blood-retinal barrier; (6) retinal neovascularisation; (7) vitreous abnormalities; and (8) choroidal atrophy. Visual reduction was mainly

Retina-Brain Axis in Postoperative Delirium: Mechanistic Links, Single-Cell/Omics Evidence, and Translational Opportunities.

Postoperative Delirium (POD) is a common, mechanistically heterogeneous neuropsychiatric syndrome characterized by acute neuroinflammation and neurovascular dysfunction. Despite its clinical significance, the lack of accessible biomarkers to monitor Central Nervous System (CNS) vulnerability remains a major barrier to effective risk stratification. The retina, as an embryological extension of the CNS, shares highly conserved neurovascular and immune architectures with the brain, including blood-barrier parallels and glial-mediated neurovascular coupling. This biological continuity suggests tha

Disruption of the hippocampal and hypothalamic blood-brain barrier in a diet-induced obese model of type II diabetes: prevention and treatment by the mitochondrial carbonic anhydrase inhibitor, topiramate.

BACKGROUND: Type II diabetes is a vascular risk factor for cognitive impairment and increased risk of dementia. Disruption of the blood-retinal barrier (BRB) and blood-brain barrier (BBB) are hallmarks of subsequent retinal edema and central nervous system dysfunction. However, the mechanisms by which diet or metabolic syndrome induces dysfunction are not understood. A proposed mechanism is an increase in reactive oxygen species (ROS) and oxidative stress. Inhibition of mitochondrial carbonic anhydrase (mCA) decreases ROS and oxidative stress. In this study, topiramate, a mCA inhibitor, was ex

Blood-retina barrier failure and vision loss in neuron-specific degeneration.

Changes in neuronal activity alter blood flow to match energy demand with the supply of oxygen and nutrients. This functional hyperemia is maintained by interactions between neurons, vascular cells, and glia. However, how changing neuronal activity prevalent at the onset of neurodegenerative disease affects neurovascular elements is unclear. Here, in mice with photoreceptor degeneration, a model of neuron-specific dysfunction, we combined assessment of visual function, neurovascular unit structure, and the blood-retina barrier permeability. We found that the rod loss paralleled remodeling of t

Chen's Jinshui Pills ameliorate diabetic retinopathy in mice and are associated with intestinal barrier protection and anti-inflammatory effects.

ETHNOPHARMACOLOGICAL RELEVANCE: Chen's Jinshui Pills (CJSP), a traditional Chinese medicinal formula, is commonly used to treat ophthalmic disorders. However, its therapeutic effects and mechanisms in diabetic retinopathy (DR) require further validation and exploration. AIM OF THE STUDY: In this study, we investigated the protective effects of CJSP on DR and its association with intestinal homeostasis. MATERIALS AND METHODS: A type 2 diabetic mouse model was established and treated with CJSP. Metabolic parameters and retinal pathology were evaluated. Mass spectrometry and network pharmacolog

Emerging roles of migrasomes in blood-tissue barrier integrity and intercellular signaling.

Migrasomes are a newly recognized class of migration-dependent organelles that arise along retraction fibers of migrating cells and are released into the extracellular milieu. Structurally and biogenetically distinct from extracellular vesicles, migrasomes function as signaling hubs that mediate localized transfer of bioactive cargo and coordinate intercellular communication. Accumulating evidence indicates that migrasomes participate in diverse biological processes, including developmental patterning, immune regulation, and tumor progression. Blood-tissue barriers (including the blood-brain,

Human Vascular Pericytes and Cytomegalovirus Pathobiology.

Pericytes are multipotent cells of the vascular system with cytoplasmic extensions proximal to endothelial cells that occur along the abluminal surface of the endothelium. The interactions between endothelial cells and pericytes are essential for proper microvascular formation, development, stabilization, and maintenance. Pericytes are essential for the regulation of paracellular flow between cells, transendothelial fluid transport, angiogenesis, and vascular immunosurveillance. They also influence the chemical composition of the surrounding microenvironment to protect endothelial cells from p

An Aggregation-Induced Polymerization Poly(Disulfide)-Drug Nanoplatform for Autoimmune Uveitis Therapy via Inhibiting the cGAS-STING Pathway.

Autoimmune uveitis is a sight-threatening inflammatory disease in which the cGAS-STING signaling pathway exacerbates inflammation by recognizing excessive cell-free DNA (cfDNA). However, conventional cfDNA scavengers have limited blood-retinal barrier penetration, and monotherapies cannot control the complex inflammatory network. Herein, we developed a cationic poly(disulfide)-drug nanoplatform (LA/DexP), which self-assembled via salt bridge interactions and aggregation-induced polymerization between a guanidyl-functionalized lipoic acid derivative (LA) and dexamethasone phosphate (DexP). This

Nanozyme eye drops for retinal barrier penetration and vasculopathy repair.

Retinal neovascularization diseases cause vision impairment due to abnormal blood vessel growth in the retina. Current treatments, including repeated intraocular anti-vascular endothelial growth factor injections, are invasive and often lead to discomfort and complicated hemorrhages. Here, we developed a noninvasive nanozyme eye drop capable of penetrating the fundus to eliminate reactive oxygen species (ROS) and thereby inhibit neovascularization. The nanozyme eye drops consist of liposomes formed by fluorinated and arginine-glycine-aspartic acid-modified phospholipids, which enhance the pene

Suprachoroidal and Subretinal Injections of AAV Using Transscleral Microneedles for Retinal Gene Delivery in Nonhuman Primates.

Retinal gene therapy using adeno-associated viruses (AAVs) is constrained by the mode of viral vector delivery. Intravitreal AAV injections are impeded by the internal limiting membrane barrier, while subretinal injections require invasive surgery and produce a limited region of therapeutic effect. In this study, we introduce a novel mode of ocular gene delivery in rhesus macaques using transscleral microneedles to inject AAV8 into the subretinal or suprachoroidal space, a potential space between the choroid and scleral wall of the eye. Using in vivo imaging, we found that suprachoroidal AAV8

Granzyme B in age-related macular degeneration: mechanistic insights and therapeutic perspectives.

Granzyme B (GzmB), beyond its established role in corneal and conjunctival disease, is increasingly recognized in the retinal and choroidal distribution of several pathologies. GzmB targets tight-junction proteins (e.g., ZO-1, JAM-A) and extracellular proteins such as fibronectin and laminin within the retinal pigment epithelium (RPE) and Bruch's membrane, thereby disrupting the outer blood-retinal barrier and contributing to the pathogenesis of age-related macular degeneration (AMD) and Macular neovascularization (MNV). This mini review highlights recent mechanistic insights into how GzmB dri

Long-Term Oral Administration of Salidroside Alleviates Diabetic Retinopathy in db/db Mice.

Diabetic retinopathy (DR), a microvascular complication of diabetes mellitus, is the leading cause of vision loss in the working-age population worldwide. Unfortunately, current clinical treatments cannot completely prevent the occurrence and development of DR. Salidroside (Sal) is a medicinal supplement that has antioxidative and cytoprotective properties. This study aimed to investigate the therapeutic effect of Sal on DR. Briefly, Sal treatment was applied to wide-type mice and db/db mice (a widely used diabetic mice) at 25 mg/kg by oral gavage once daily from 8 weeks to 20 weeks. Mice's bo

Gain-of-Function Variant in STAT3 and Retinal Macular Edema: Insights into the IL-6 R/JAK/STAT3 Pathway in Retinal Pigment Epithelium.

PURPOSE: The IL-6 R/JAK/STAT3 signaling pathway is implicated in non-infectious uveitis-associated macular edema, and its inhibition using monoclonal anti-IL-6 receptor therapy has been associated with clinical improvement. However, the underlying pathogenic mechanisms remain unclear. In this study, we investigated the pathological effects of a STAT3 gain-of-function (GOF) variant p.L387R identified in a family with immune dysregulation syndrome and severe retinal vasculitis and macular edema. METHODS: Primary retinal pigment epithelial (RPE) cells were transduced with STAT3 wildtype (WT), th

A Glucose-Responsive Hydrogel Inhibits Primary and Secondary BRB Injury for Retinal Microenvironment Remodeling in Diabetic Retinopathy.

Current diabetic retinopathy (DR) treatment involves blood glucose regulation combined with laser photocoagulation or intravitreal injection of vascular endothelial growth factor (VEGF) antibodies. However, due to the complex pathogenesis and cross-interference of multiple biochemical pathways, these interventions cannot block disease progression. Recognizing the critical role of the retinal microenvironment (RME) in DR, it is hypothesized that reshaping the RME by simultaneously inhibiting primary and secondary blood-retinal barrier (BRB) injury can attenuate DR. For this, a glucose-responsiv

Orchestrated leukocyte recruitment to immune-privileged sites: absolute barriers versus educational gates.

Complex barriers separate immune-privileged tissues from the circulation. Here, we propose that cell entry to immune-privileged sites through barriers composed of tight junction-interconnected endothelium is associated with destructive inflammation, whereas border structures comprised of fenestrated vasculature enveloped by tightly regulated epithelium serve as active and selective immune-skewing gates in the steady state. Based on emerging knowledge of the central nervous system and information from other immune-privileged sites, we propose that these sites are endowed either with absolute en
