[Internal fixation of meta- and diaphyseal intercalary bone defects after tumour resection with intramedullary nailing and porous polymethylmetacrylate (PMMA) spacer].

BACKGROUND: Radical resection of bone tumours localized in the meta- or diaphysis of long bones frequently results in segmental defects. Several stabilization techniques with preservation of the adjacent joint have been published, but the failure rate appears to be considerable. The objective of this study is the experimental and clinical testing of a new technique which combines unreamed nailing with support of the defect by a porous polymethylmetacrylate (PMMA) spacer. METHODS: For spacer preparation, PMMA spheres were adhered to a cylindrical spacer (Ø 32 mm) with interconnective porosity.

Novel anchor-type proximal femoral nail for the improvement of bone-fixation integrity in treating intertrochanteric fractures: an experimental and computational characterization study.

This study introduces a novel anchor-type proximal femoral nail (AT-PFN) to improve the bone-fixation integrity over the standard screw-type nail (SST-PFN). Quasi-static incremental cyclic load test was performed to investigate load-displacement, cumulative deformation energy, time-strain, and backbone curves. The finite element analysis (FEA) was implemented to identify the stress and strain distributions. Additionally, non-destructive dynamic tests were conducted, and the measurements were processed using statistical pattern recognition, based on vector autoregression and principal component

Solid locked intramedullary nailing for expeditious return of bone-setting-induced abnormal fracture union victims to work in South-western Nigeria.

Wage earning in low- and middle-income countries (LMICs) is predominantly through physical labour. Consequently, limb-related disabilities caused by abnormal fracture unions (AFUs) preclude gainful employment and perpetuate the cycle of poverty. Many AFUs result from traditional bone-setting (TBS), a pervasive treatment for long bone fractures in LMICs. The objective of this study was to accentuate the expediency of solid locked intramedullary nail in the early restoration of victims of TBS-induced abnormal fracture unions (AFUs) to their pre-injury functioning, including work. One hundred AFU

Exchanging reamed nailing versus augmentative compression plating with autogenous bone grafting for aseptic femoral shaft nonunion: a retrospective cohort study.

OBJECTIVE: The purpose of this study was to compare the outcomes of exchanging reamed nailing (ERN) and augmentative compression plating (ACP) with autogenous bone grafting (BG) for the treatment of aseptic femoral shaft nonunion secondary to the treatment of intramedullary nailing (IMN). METHODS: A multicenter retrospective study was performed for 178 patients (180 cases) of aseptic femoral shaft nonunion secondary to first treatment of IMN. All cases were fixed with either ERN (n=87) or ACP (n=93). In the ERN group, 42 cases (48.3%) were nonisthmal nonunions and 45 (51.7%) were isthmal nonu

Fractures of the neck of the fifth metacarpal bone, treated by percutaneous intramedullary nailing: surgical technique, radiological and clinical results study (28 cases).

This study report the results in 28 patients affected by closed fractures of the neck of the fifth metacarpal bone (boxer's fracture), treated with percutaneous elastic intramedullary nailing using a single wire, to verify the effectiveness of this surgical treatment. We reviewed the results of 28 patients treated with A single Kirschner wire (K-wire) pre-bent in a lazy-S fashion with a mild bend at approximately 5 millimeters, The K-wire is inserted blunt end first in an antegrade manner and the fracture reduced as the wire is passed across the fracture site The wire is usually removed with p

Retrospective study on the outcomes of Fassier-Duval nailing and osteotomy for the treatment of long bone fractures or deformities in the lower extremities in children with osteogenesis imperfecta.

BACKGROUND: Osteogenesis imperfecta (OI) is a rare hereditary disorder characterized by bone fragility and deformity, frequently requiring surgical intervention to restore limb alignment and stability. Traditional intramedullary fixation devices are limited in growing children. The Fassier-Duval (FD) nail, designed to elongate with growth, offers potential advantages when combined with corrective osteotomy. This study retrospectively evaluated the clinical outcomes of FD nail fixation with osteotomy for long bone fractures and deformities in children with OI. METHODS: We retrospectively revie

Treatment of Infection After Tibial Intramedullary Nailing With Bone Distraction Combined With Free Anterolateral Thigh Flap for Concomitant Soft-tissue Defect.

A 57-year-old man presented with a tibial shaft fracture treated with tibial intramedullary nail fixation and plate and screw fixation for fibular fracture. Two weeks postoperatively, the patient developed skin sloughing with exposed bone and metal at the fracture site. Three weeks postoperatively, a physical examination revealed swelling of the left lateral malleolus and lower leg, a skin defect of approximately 3 × 5 cm2 at the lower third of the left tibia with an exposed fracture site and hardware in between the fracture fragments. The patient underwent debridement, removal of the intramed

Optimal blocking screw placement for retrograde IM nail fixation of distal femur fracture: a standardized biomechanical study of "osteoporotic" synthetic bone.

PURPOSE: Intramedullary (IM) nail use for metaphyseal fracture management is problematic in long bones like the femur because the larger medullary cavity allows increased fracture motion and potentially increased implant failure and malunion/nonunion risk; Achieving effective distal femur fracture fixation is even more difficult in osteoporotic bone. Blocking screws to control IM nail movement are known to be effective for tibia fracture management. Few studies have evaluated IM nail and blocking screw use efficacy for distal femur fracture fixation in osteoporotic bone. METHODS: This study u

Early Conversion of External Fixation to Interlocked Nailing in Open Fractures of Both Bone Leg Assisted with Vacuum Closure (VAC) - Final Outcome.

INTRODUCTION: Management of compound grade III fractures of both bone leg includes external stabilization for long period, followed by various soft tissue coverage procedures. Primary interlocking of tibia had been also done with variable results. External fixation for long time without any bone loss often leads to infected nonunion, loss of reduction, pin tract infection and failure of fixation, primary interlocking in compound grade III fractures had shown high medullary infection rate. We managed all cases of compound grade III A/B fractures with primary external fixation, simultaneous woun

A novel approach to juxta-articular aggressive and recurrent giant cell tumours: resection arthrodesis using bone transport over an intramedullary nail.

Aggressive juxta-articular giant cell tumours of the lower limbs occurring in young patients are a challenge to the average orthopaedic surgeon. Although it is the treatment of choice for these tumours, wide resection creates a problem for the reconstruction of large bone gaps. We describe our results after resection arthrodesis of such tumours using the technique of bone transport over a long intramedullary nail in 27 patients. This is the first and largest study of its kind in the management of giant cell tumours in the literature. All our patients fared well with this mode of treatment, and

Does a bone deformity of the distal phalanx undergo remodeling after removal of a congenital ectopic nail?: A case with periodic radiographic follow-up.

BACKGROUND: Congenital ectopic nails are rare and are characterized by the presence of nail-like tissue mainly at the tip of a finger or toe. OBJECTIVE: Although the accompanying bone deformity might undergo remodeling after removal of the ectopic nail, it remains unknown whether complete bone remodeling can be eventually achieved, and whether such remodeling is necessary to improve the final cosmetic appearance of the concerned fingertip. METHODS AND RESULTS: Follow-up with periodic radiographic examination for 1 year after surgery in a child with congenital ectopic nail revealed no bone re

A Fracture Healing Odyssey: Kinematic Comparison of Unions and Nonunions in Human Lower-Extremity Long Bones Treated with Intramedullary Nailing: A Retrospective Cohort Study.

BACKGROUND: Fracture nonunion remains a major clinical challenge. Although radiographic scores such as RUST (Radiographic Union Score for Tibial fractures) and mRUST (the modified version) are widely used to assess healing, there has been limited investigation into their longitudinal use. This study evaluated whether a log-logistic statistical model of radiographic healing scores over time could quantify differences between fractures that healed and those that went on to nonunion. METHODS: This retrospective cohort analysis added serial radiographs of 49 femoral shaft nonunions to radiographs

Femoral nailing associated with bone marrow emboli in pigs induced a specific increase in blood IL-6 and broad inflammatory responses in the heart and lungs.

INTRODUCTION: Bone marrow embolization may complicate orthopedic surgery, potentially causing fat embolism syndrome. The inflammatory potential of bone marrow emboli is unclear. We aimed to investigate the inflammatory response to femoral intramedullary nailing, specifically the systemic inflammatory effects in plasma, and local tissue responses. Additionally, the plasma response was compared to that following intravenous injection of autologous bone marrow. METHODS: Twelve pigs underwent femoral nailing (previously shown to have fat emboli in lung and heart), four received intravenous bone m
