Massive cerebral swelling immediately after cranioplasty, a fatal and unpredictable complication: report of 4 cases.

Cranioplasty after decompressive craniectomy (DC) is associated with increased morbidity, but the reported mortality rate is low. Recently, some authors have reported a rare unexplained complication of sudden death in association with massive cerebral edema immediately after cranioplasty. The author reports on 4 patients who underwent cranioplasty after DC between January 2005 and August 2010 at his department and died because of massive cerebral edema immediately after uneventful surgery and anesthesia. All 4 of the new cases reported involved young male patients who underwent decompressive h

Central nervous system mucormycosis in a patient with hematological malignancy: A case report and review of the literature.

Invasive mucormycosis is a refractory fungal infection. Central nervous system (CNS) mucormycosis is a rare complication caused by infiltration from the paranasal sinuses or hematogenous dissemination. Here, we present a case of a brain abscess, due to mucormycosis, diagnosed using burr craniotomy. A 25-year-old Japanese woman with relapsed-refractory acute lymphoblastic leukemia underwent cord blood transplantation (CBT). The patient experienced prolonged and profound neutropenia, and oral voriconazole was administered as primary antifungal prophylaxis. The patient received a conditioning reg

Development and evaluation of a craniocerebral model with tactile-realistic feature and intracranial pressure for neurosurgical training.

OBJECTIVE: In this article, a craniocerebral model is introduced for neurosurgical training, which is patient-specific, tactile-realistic, and with adjustable intracranial pressure. METHODS: The patient-specific feature is achieved by modeling from CT scans and magnetic resonance images (MRI). The brain tissue model is built by the hydrogel casting technique, while scalp, skull, vasculature, and lateral ventricles are all-in-one fabricated by three-dimensional (3D) printing. A closed-loop system is integrated to monitor and control the intracranial pressure. 3D measurements, mechanical tests,

Repetitive Computed Tomography Perfusion for Detection of Cerebral Vasospasm-Related Hypoperfusion in Aneurysmal Subarachnoid Hemorrhage.

BACKGROUND: Delayed cerebral infarction after aneurysmal subarachnoid hemorrhage (aSAH) still remains the leading cause of disability in patients that survive the initial ictus. It has been shown that computed tomography perfusion (CTP) imaging can detect hypoperfused brain areas. The aim of this study was to evaluate if a single acute CTP examination at time of neurologic deterioration is sufficient or if an additional baseline CTP increases diagnostic accuracy. METHODS: Retrospective analysis of acute and baseline (within 24 hours after aneurysm treatment) CTP examinations of patients with

An unusual presentation of a large lower extremity abscess caused by Aggregatibacter aphrophilus: A case report.

INTRODUCTION: Aggregatibacter aphrophilus (A. aphrophilus) is a rare cause of severe infections, including periodontitis, brain abscess, and endocarditis. While typically oral commensals, its involvement in extremity soft tissue abscesses is uncommon, posing diagnostic challenges and often mimicking tumors. This report describes a rare A. aphrophilus infection presenting as a large, spontaneously draining lower extremity abscess without trauma history. PRESENTATION OF CASE: A 63-year-old male with untreated diabetes presented with posterior knee pain. Initial MRI suggested a biceps femoris te

Evidence for bulk flow of brain interstitial fluid: significance for physiology and pathology.

This review surveys evidence for the flow of brain interstitial fluid (ISF) via preferential pathways through the brain, and its relation to cerebrospinal fluid (CSF). Studies over >100 years have raised several controversial points, not all of them resolved. Recent studies have usefully combined a histological and a mathematical approach. Taken together the evidence indicates an ISF bulk flow rate of 0.1-0.3 microl min(-1) g(-1) in rat brain along preferential pathways especially perivascular spaces and axon tracts. The main source of this fluid is likely to be the brain capillary endothelium

Management of residual dysplastic vessels after cerebral arteriovenous malformation resection: implications for postoperative angiography.

OBJECTIVE: The verification of surgical resection of cerebral arteriovenous malformations (AVMs) relies on angiography. Abnormal vasculature often is identified after removal of the AVM. Differentiation of dysplastic feeding vessels that resemble the neovascularity of moyamoya disease, as distinct from residual AVM, is crucial for preserving critical brain areas. We review a large experience with immediate postoperative angiography after AVM resection and discuss the implications for management of abnormal dysplastic vessels discovered after AVM resection. METHODS: Beginning in 1992, 86 conse

Postoperative Diabetes Insipidus Mimicking Radiological Findings and Symptoms of Intracranial Hypotension: A Case Report.

Endocrine disturbances such as diabetes insipidus (DI) and syndrome of inappropriate antidiuretic hormone secretion (SIADH) are recognized complications of craniopharyngioma surgery, which occur due to damage to structures that produce or store antidiuretic hormone (ADH). Intracranial hypotension is a clinical syndrome that presents with headache and typical radiological features and can occur due to a leak of cerebral spinal fluid (CSF) in operations that involve the opening of the arachnoid (e.g., craniopharyngioma surgery). We describe a patient presenting with headache, radiological eviden

Very Large Metastases to the Brain: Retrospective Study on Outcomes of Surgical Management.

BACKGROUND: The incidence of brain metastases is rising. To our knowledge, no published study focuses exclusively on brain metastases larger than 4 cm. We present our surgical outcomes for patients with brain metastases larger than 4 cm. METHODS: This is a retrospective chart review of inpatient data at our institution from January 2006 to September 2015. Primary end points included overall survival, progression-free survival, and local recurrence rate. RESULTS: Sixty-one patients had a total of 67 brain metastases larger than 4 cm: 52 were supratentorial and 15 were infratentorial. Forty-th

Risk factors for postoperative seizures in patients with chronic subdural haematomas.

Postoperative seizures are a frequently occurring yet not well-understood complication in patients undergoing surgical treatment of chronic subdural haematomas (cSDHs). Therefore, we investigated surgical and non-surgical risk factors that are commonly considered causal in provoking epileptic seizures, paying special attention to the intracranial course of the subdural drain (SDD) and the configuration of the haematoma. Data of patients with a cSDH, that were treated at our neurosurgical department between 2008 and 2014 were analysed. Patients suffering from severe pre-existing conditions and

Oscillation analysis as a supporting element toward safe and reliable lumbar catheter intracranial pressure monitoring.

OBJECTIVE: Lumbar drainage of cerebrospinal fluid for treatment of refractory increased intracranial pressure (ICP) is associated with the risk of infratentorial herniation, but real-time biomarkers for signaling herniation at bedside are lacking. Here, the authors tested whether an alteration of pulsatile waveform conduction across the level of the foramen magnum could serve as an indicator of insufficient hydrostatic communication and impending herniation. METHODS: This prospective observational cohort study included patients with severe acute brain injury who underwent continuous external

Cerebrospinal Fluid Diversion for Refractory Intracranial Hypertension in Traumatic Brain Injury: A Single Center Experience.

BACKGROUND: Diversion of cerebrospinal fluid (CSF) is a common neurosurgical procedure for control of intracranial pressure (ICP) in the acute phase after traumatic brain injury (TBI), where medical management is insufficient. CSF can be drained via an external ventricular drain (EVD) or, in selected patients, via a lumbar (external lumbar drain [ELD]) drainage catheter. Considerable variability exists in neurosurgical practice on their use. METHODS: A retrospective service evaluation was completed for patients receiving CSF diversion for ICP control after TBI, from April 2015 to August 2021.

Brain stem arteriovenous malformation with large venous varix.

The case of a patient with arteriovenous malformation (AVM) with a venous varix that occurred at the medulla oblongata is reported. The AVM bled twice in a short period. The AVM was fed by the small ramus, possibly the anterior spinal artery, originating from the right distal vertebral artery and draining into a large venous varix and then into the venous systems surrounding the brain stem. The obstruction of the feeding artery was performed, and the varix subsequently disappeared. The remaining abnormal vasculature was then treated with stereotactic radiosurgery. The treatment of an AVM at th

[The reactivity of the major vessels of the human brain based on the data of transcranial dopplerography].

Clinico-physiological studies of 426 healthy subjects revealed normal parameters of the blood flow linear velocity along the extra- and intracranial portions of major cerebral vessels as determined by means of transcranial dopplerography. Normal parameters of the systolic-diastolic coefficient, the reactivity of major vessels and their pial draining network were studied in respect to their responses to functional loads of physical and chemical character which enabled one to determine functional possibilities of the cerebral circulation system. The ability of the dopplerographic technique was a

Persistent left superior vena cava communicating with the left atrium through a systemic-pulmonary venous malformation.

A 14 year old white girl who presented with a brain abscess was discovered to have a left pulmonary vascular malformation on a chest roentgenogram. Angiograms revealed a left superior vena cava that drained into a venous malformation within the left lung, then communicated with the left atrium by way of the left superior pulmonary vein. The right superior vena cava was functionally absent and was anatomically an atretic cord. There was mild systemic arterial hemoglobin desaturation, but no evidence of cyanosis. The embryology, physiology and surgical repair of this rare lesion and the complica

Urinoma: an unusual case of emesis in a brain-injured child.

The evaluation of acute emesis in a multiple-trauma paediatric patient with severe traumatic brain injury can present a difficult but not unusual problem in the rehabilitation unit. The differential diagnosis will be discussed in a paediatric case in which emesis was related to a urinoma, secondary to ureteropelvic disruption, compressing the duodenum. We present a case of a 6-year-old Caucasian girl with severe brain injury who developed emesis while in rehabilitation. An X-ray of the abdomen revealed a shadow over the right psoas. Subsequently mild compression of the duodenum was diagnosed b

Myelin antigen capture in the CNS by B cells expressing EBV latent membrane protein 1 leads to demyelinating lesion formation.

The efficacy of B cell depletion therapies, and their association with Epstein-Barr virus (EBV), implicate B cells in the pathogenesis of multiple sclerosis (MS). In mice, we observed that viral infections induce infiltration of B cells into the brain, independent of phenotype and specificity, and that myelin-reactive B cells then capture antigens directly from parenchyma. Trafficking of these antigen-loaded B cells to draining lymph nodes was not observed, and without T cell help, antigen-capturing B cells die rapidly. CD40L signaling or EBV latent membrane protein 1 (LMP1) can override this

A Case of Cerebellar Hippocampal and Basal Nuclei Transient Edema With Restricted Diffusion Syndrome With Poor Clinical Outcome.

Cerebellar hippocampal and basal nuclei transient edema with restricted diffusion (CHANTER) syndrome is a specific pattern of restricted diffusion in the hippocampi and cerebellum identified on brain imaging by clinicians in patients who present with altered mental status in the context of substance intoxication. These patients developed obstructive hydrocephalus a couple of days into their hospitalization that required therapy with osmotic agents and/or surgical interventions (i.e., drains and decompressive craniectomy). In prior cases published, many of the patients had good recovery. The ca

How Does Severe Acute Respiratory Syndrome-Coronavirus-2 Affect the Brain and Its Implications for the Vaccines Currently in Use.

This mini-review focuses on the mechanisms of how severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2) affects the brain, with an emphasis on the role of the spike protein in patients with neurological symptoms. Following infection, patients with a history of neurological complications may be at a higher risk of developing long-term neurological conditions associated with the α-synuclein prion, such as Parkinson's disease and Lewy body dementia. Compelling evidence has been published to indicate that the spike protein, which is derived from SARS-CoV-2 and generated from the vaccines cu
