Reversible tacrolimus-induced neurotoxicity isolated to the brain stem.

Diplopia, nystagmus, visual hallucinations, and internuclear ophthalmoplegia developed in a 30-year-old woman 84 days after she received a matched, unrelated bone marrow transplant for chronic myeloid leukemia. A regimen of tacrolimus had been administered since the transplantation was performed. MR imaging revealed bilaterally symmetric regions of signal abnormality with abnormal contrast enhancement in the brain stem. No supratentorial abnormality was present. Tacrolimus therapy was discontinued, and the symptoms resolved. MR imaging that was performed 10 days after tacrolimus was discontinu

Brain stem neural mechanisms for vocalization in decerebrate cats.

In order to characterize the brain stem circuitry that produces vocalization, the activities of brain stem respiratory neurons were recorded extracellularly during vocalization induced by electrical stimulation of the periaqueductal gray in decerebrate cats. After the onset of stimulation, the respiratory rhythm ceases, and a preparatory inspiration is induced. Following this initial inspiration, vocalization characterized by increased activities of the intrinsic laryngeal adductor and the major expiratory muscles is induced. During vocalization, most of the dorsal respiratory group inspirator

Brain stem dysfunction in spasmodic torticollis : an electrophysiological study.

Continuous or intermittent deviation of head seen in spasmodic torticollis (ST) may result in abnormalities of blink reflex or brain stem auditory evoked potentials (BAEP). The present study was undertaken on 10 patients of ST and an equal number ofage and sex matched healthy controls. BAEP showed no difference in the two groups. R1 and contralateral R2 of blink reflex were similar in the patients and the controls. R2 contralateral to torticollis was also similar to controls but R2 ipsilateral to torticollis was prolonged in ST patients (32.28??1.91 msec) as compared to controls (30.96??1.53 m

[Focal dystonia and tremor secondary to brain stem tuberculoma].

Brainstem tuberculoma is exceptionally observed. We report a 44 year-old immunocompetent man with proven diagnosis of miliary tuberculosis (TBC) who developed a complex neurological syndrome characterized by right ophtalmoplegia, left-sided hemiparesis and hemihypoesthesia and a gross ipsilateral postural and action tremor with hand dystonia. A ponto-mesencephalic mass was detected by CT and MRI studies of the brain. Clinical, bacteriological and neuroimaging studies allowed to suspect a ponto-mesencephalic tuberculoma. Long-term therapy with anti-TBC drugs and steroids was started, achieving

Surgically resected brain stem cavernous angioma in an infant.

An infantile case of surgically resected brain stem cavernous angioma is presented. This 21-month-old boy was admitted to our department after repeated severe pontine hemorrhage. A deeplying lesion with hematoma was successfully removed via the IV ventricle approach. Few infantile cases of this lesion have been presented previously, and the approaches to brain stem cavernous angiomas in infants are discussed.

Ischemic and hemorrhagic brain stem lesions mimicking diabetic ophthalmoplegia.

Two patients with diabetes mellitus, one of them with an isolated third cranial nerve palsy and the other with an isolated sixth cranial nerve palsy, are presented. MRI investigations including diffusion-weighted MRI revealed a small ischemic brain stem lesion in the former and a small hemorrhagic brain stem lesion in the latter. In the former case wallerian degeneration of the nerve fascicle within the mesencephalon was also detected. These cases indicate that vascular accidents of the brain stem may masquerade as fascicular or infranuclear disturbance of the oculomotor or abducens nerve; the

Primary brain stem lesions caused by closed head injuries.

Traumatic lesions of the brain stem are of two types: primary, which are considered to be caused at the moment of impact, and secondary, associated with supratentorial mass lesion. Of the 239 patients with a serious head injury who showed a severe disturbance of consciousness upon admission and who had CT scan carried out immediately, 21 cases were considered to have a primary brain stem lesion with initial CT scan. A primary brain stem lesion was found in 21 of 239 (8.8%) of patients with serious head injury. Their injuries were caused primarily by traffic accidents. Sixteen of the 21 cases s

[Evoked potentials in diagnosis of ischemic brain stem lesions].

We present an overview on the impact of evoked potentials in diagnosis of ischemic brainstem lesions. Brainstem auditory evoked potentials and somatosensory evoked potentials depict abnormalities, whereas visual evoked potentials are normal in most cases. In patients with basilar artery thrombosis and primary pontine hemorrhage these evoked potentials are mostly abnormal and can indicate the location of the lesion. Furthermore, they are of prognostic value. Whether evoked potentials are abnormal in strokes with branch occlusion of the basilar artery, depends on the location of the infarction.

Auditory brain stem response monitoring during middle ear surgery.

Conductive hearing loss can be determined in the course of middle ear surgery by sound probe stimulation and simultaneous recording of auditory brain stem responses. Mechanical properties of the ossicular chain, usually judged subjectively by the surgeon's visual and tactile senses, can be quantified objectively. Thus, disarticulation of ossicles can be localized precisely, and fixation of the head of the malleus can be differentiated from stapes fixation. Moreover, the function of the reconstructed ossicular chain or prosthesis may be ascertained.

Computer-directed stereotactic biopsy of intrinsic brain stem lesions.

Despite advances in the neuro-imaging of the brain stem, an accurate diagnosis of intrinsic lesions in this region requires tissue sampling and histological verification. We have performed a series of computer-directed stereotactic procedures in 12 patients with intrinsic brain stem lesions. A positive diagnosis was obtained in 11 cases and therapeutic intervention was possible in four. There was no operative mortality. Because of the importance of an accurate diagnosis in order to avoid inappropriate therapy, together with the relative safety of the technique, computer-directed stereotactic b

Eye-movement disorders in brain-stem and cerebellar stroke.

Vertebrobasilar strokes can yield varied disturbances of eye movements, by affecting specific centers and pathways contained in the brain stem and cerebellum. Unique disorders combining supranuclear, nuclear, and infranuclear syndromes may occur. Some eye-movement abnormalities are useful localizing signs (eg, gaze palsies, rotatory nystagmus, and ipsilateral saccadic bias), but many others are not. The use of techniques such as magnetic resonance imaging may provide new insights in clinicotopographic correlations in patients with good recovery, in the absence of pathologic verification.

Brain stem and cerebellar deficits in eye movement control.

Recent experimental studies allow more precise functional-anatomical correlations in patients with ocular motor disorders. The abducens nucleus contains two groups of cells that together subserve ipsilateral horizontal conjugate gaze: Motoneurons with axons that innervate the lateral rectus and internuclear neurons with axons that cross the midline, ascend in the medial longitudinal fasciculus (MLF) and impinge upon medial rectus motoneurons. The pontine paramedian reticular formation (PPRF) contains burst neurons for horizontal saccades. The rostral interstitial nucleus of the MLF (riMLF) in

Measurement of auditory brain stem potentials in Bell's palsy.

Auditory brain stem potentials (ABP) were recorded in 27 patients with Bell's palsy during the early phase of the disease and 1-3 months later. Pathological findings were observed in 6 patients, the dominant features being prolonged brain stem transmission time and diminished amplitude of the second wave. The extension of the pathological process in the so called idiopathic peripheral facial paralysis appears to vary. The observation of brain stem involvement in some patients with Bell's palsy may have important clinical implications.
