Diffuse lymphoplasmacytic bronchiolitis in cartilage-hair hypoplasia.

Three children with cartilage-hair hypoplasia presented with chronic obstructive symptoms and bronchiolar wall thickening on high-resolution computed tomography scanning. In all children, surgical lung biopsy demonstrated diffuse dilated lymphoplasmacytic bronchiolitis. The bronchiolar wall was infiltrated by a lymphocyte sheath with plasma cell differentiation and dispersed secondary follicles. Clarithromycin substantially improved respiratory symptoms and pulmonary function, allowing children to return home.

Respiratory syncytial virus bronchiolitis: prevention and treatment.

BACKGROUND: Acute respiratory illness due to respiratory syncytial virus is one of the most common causes of hospitalization in very young children worldwide. The virus was discovered half a century ago, yet the underlying mechanisms of the disease are not fully understood and treatment remains supportive. METHODS: This review article discusses therapeutic and preventive strategies, past, present and future, in the battle against respiratory syncytial virus. CONCLUSION: Prevention of severe respiratory syncytial virus infection in high-risk children can be achieved by the administration of s

Bronchiolitis: the new American Academy of Pediatrics guidelines.

Respiratory Syncytial Virus (RSV) is the leading cause of hospitalization for lower respiratory tract infections (LRTI) in young children worldwide. RSV is an important cause of LRTI in infants and a variety of factors, such as gender, age at admission, gestational age, birth weight, and exposure to tobacco smoke and breast feeding may affect the prevalence of RSV-related disease and, possibly, the risk of developing asthma-like symptoms during the school years. AAP in 2006 formulated the new guidelines on diagnosis and management of bronchiolitis, giving recommendations on different condition

[A case of mycoplasmal pneumonia with bronchiolitis treated with steroids].

A 37-year-old woman admitted elsewhere for a high fever, dry cough, stridor, and dyspnea was found in chest radiography and computed tomography on admission to have a thickened bronchial wall with centrilobular nodules in both lower lobes and skipped consolidations in the lower and middle lobe of the right lung. She had been diagnosed with mycoplasmal pneumonia because of high Mycoplasma pneumoniae antibody titer, so clarithromycin (CAM) was administrated. She was referred to us due to hypoxia with obstructive impairment in the pulmonary function test. Ventilation/perfusion radioisotope in the

Nasal lavage leukotrienes in infants with RSV bronchiolitis.

Respiratory syncytial virus (RSV) bronchiolitis is a very common infection in infants and, after the acute phase, a number of patients develop a reactive airway disease that lasts for years. Although the pathogenesis of the lung damage after RSV bronchiolitis is still largely unknown, previous studies suggest that leukotrienes may play an active part in it. The aim of this study was to measure leukotriene levels in the nasal lavage fluid (NLF) collected in infants during RSV bronchiolitis and 1 month later. Cysteinyl leukotrienes (Cys-LTs) and leukotriene B(4) (LTB(4)) were measured in the NLF

[Non transplant-related constrictive bronchiolitis in adults].

INTRODUCTION: The term bronchiolitis refers to inflammatory disorders of the bronchioles. Constrictive bronchiolitis is the type most frequently encountered. STATE OF THE ART/PERSPECTIVES: The main clinical manifestations include the development of exertional dyspnoea and fixed airflow obstruction. Chest x-ray findings are usually unhelpful, but CT scanning may reveal a mosaic pattern on expiration. Peripheral micronodules are less frequently seen. The causes of constrictive bronchiolitis are numerous. The diagnosis may be clear from the clinical context when a causative event or predisposing…

[A case of follicular bronchiolitis preceding rheumatoid arthritis].

A 52-year-old woman was admitted to Yame General Hospital because of persistent cough, wheeze, and shortness of breath at age 48. Chest X-ray and computed tomography (CT) showed bilateral centrilobular shadows. Pulmonary function test revealed obstructive dysfunction. She also had chronic sinusitis. Initially, diffuse panbronchiolitis was diagnosed and she was given macrolides, but no improvement was observed. Thus video-assisted thoracoscopic lung biopsy (VATS) was performed in order to establish a definitive diagnosis. Histopathological findings were compatible with a diagnosis of follicular

[Bronchiolitis obliterans organizing pneumonia following radiotherapy].

INTRODUCTION: Pulmonary complications of radiotherapy are rare, but bronchiolitis obliterans organizing pneumonia (BOOP) is observed in 2.5% of cases. It can develop after radiation treatment of breast cancer as well as, more rarely, lung cancer, sarcoma, Hodgkin's disease or malignant thymoma. CASE: Ten months after radiotherapy for breast cancer, a 52 year-old woman developed migratory alveolar opacities outside the radiation field. Their improvement with corticosteroid treatment led to the diagnosis of BOOP. DISCUSSION: BOOP, which resembles infectious pneumonia, can develop 2-7 months af

Bronchiolitis obliterans organising pneumonia: a treatable condition.

Bronchiolitis obliterans organising peumonia BOO) is an uncommon inflammatory lung condition involving the terminal bronchioles and alveoli, which is responsive to treatment with corticosteroids. Patients usually present with dyspnoea, cough and fever. Two cases are described here; both had haemoptysis and were initially treated as community acquired pneumonia. Diagnosis was made on lung biopsy and there was rapid resolution after a course of prednisolone.

Bronchiolitis poses significant public health burden.

Bronchiolitis is the leading cause of infant morbidity, and hospitalization rates are rising. The effect of this disease is not limited to the acute illness episode. Approximately 40% to 50% of children diagnosed with bronchiolitis suffer from subsequent wheezing and airway reactivity or asthma. Attempts to address the burden of this disease via vaccine development have been largely unsuccessful, and treatment is purely supportive rather than curative. As such, primary prevention is paramount. If outdoor air pollution exacerbates this disease, as has been found for other pediatric respiratory

[The effectiveness of montelukast in recurrent post-bronchiolitis wheezing].

Some studies showed increased levels of leukotrienes in the nasopharyngeal secretions in children with wheezing secondary to viral infections, leading to therapeutical attempts with leukotriene receptor inhibitors in infants and small children with recurrent wheezing. This is a double blind randomized study for evaluating the efficacy of montelukast in infants and small children. In the study group (20 children) treated with montelukast, 5 mg/day for 3 months, the frequency of bronchial obstruction episodes in the 6 months following the start of therapy was significantly lower (p = 0.001) than

Cytokines in severe respiratory syncytial virus bronchiolitis.

Bronchiolitis caused by respiratory syncytial virus (RSV) infection is an important cause of severe lung disease in infants, and increasing evidence suggests that it is immunologically mediated. Experiments in mice suggest that this may be due to differential T-cell activation producing either type 1 or type 2 cytokines. We investigated this hypothesis in man by studying 24 infants ventilated with severe RSV bronchiolitis and by measuring messenger RNA (mRNA) for interleukin-4 (IL-4) and interferon-gamma (IFN-gamma), by polymerase chain reaction, in nasopharyngeal aspirates (NPAs) and bronchoa

Follicular bronchiolitis associated with Legionella pneumophilia infection.

An 8-month-old girl with respiratory distress and stridor was admitted to our hospital. Two days later, she died of respiratory insufficiency due to pneumonia. Autopsy confirmed the presence of follicular bronchiolitis (FBB) in both lungs. After consideration of her clinical course, we focused on three pathogens: Legionella pneumophilia, Pneumocystis carinii, and Mycobacterium tuberculosis. Only Legionella pneumophilia was detected by both immunohistochemistry and polymerase chain reaction.

Severe bronchiolitis in acute Mycoplasma pneumoniae infection.

We report on a 17-year-old patient with severe bronchiolitis due to Mycoplasma pneumoniae infection. Despite an early 10-day course of clarithromycin, she developed progressive dyspnea, cough, fever, and severe obstructive ventilatory impairment. Sixteen days after onset of the disease a severe hemolytic anemia developed with only cold agglutinins positive at serologic screening. Thoracoscopic lung biopsy revealed diffuse bronchiolitis with suppurative intrabronchiolar inflammation, lymphohistiocytic "cuffing" of the bronchioli, and foam cell aggregates within neighboring alveoli. The infiltra

Systemic corticosteroids in infant bronchiolitis: A meta-analysis.

OBJECTIVE: To determine whether corticosteroids are efficacious in treating bronchiolitis in hospitalized infants. METHODS: Online bibliographic databases (Medline, Embase, and Cochrane Clinical Trials Registry) were searched for: 1) bronchiolitis or respiratory syncytial virus, and 2) corticosteroid or glucocorticoid or steroidal antiinflammatory agents or adrenal cortex hormones. Reference lists from all selected articles were also examined. Randomized, placebo-controlled trials of systemic corticosteroids in treatment of infants hospitalized with bronchiolitis were selected by 2 investigat

[Hospitalization for bronchiolitis: a risk factor for recurrent wheezing].

This longitudinal study evaluated the role of hospitalization for acute bronchiolitis as a risk factor for recurrent wheezing. Participants were children from a 1993 cohort in a southern Brazilian city. Hierarchical multivariate analysis showed that previous hospitalization for bronchiolitis was the most important risk factor for recurrent wheezing, with an odds ratio of 4.9. This strong association is consistent with a casual relationship hypothesis.

The impact of cytolytic therapy on bronchiolitis obliterans syndrome.

BACKGROUND: Bronchiolitis obliterans syndrome (BOS) is the major cause of morbidity and death after lung transplantation. Therapy has focused on augmented immunosuppression with a variety of agents. Although transient responses are often achieved, sustained remission has been unusual. The outcome of cytolytic therapy for BOS at our center has been analyzed and is reported. METHODS: Between July 1988 and July 1994, 233 patients underwent lung transplantation at Barnes-Jewish Hospital. Among 207 recipients (88.8%) who survived more than 3 months, 81 recipients (39%) had development of BOS; 48 o
