Corrective osteotomy of the malunited distal radius fracture: use of periosteal bed may reduce the extent of postoperative graft resorption.

The aim of this study was to compare osteotomy for malunited distal radius fracture with embedment of a corticospongious graft into a periosteal flap of the recipient bone (test) with the standard procedure (control) with respect to graft resorption. A retrospective assessor-blind analysis of consecutive patients (test: n = 19, control: n = 30) was performed. Ulnar tilt, palmar tilt and capitate-ulna distance were assessed from radiographs taken before, two to four days after and over three months after the surgery to determine loss of correction achieved by the surgery and estimate graft reso

The benefit of magnetic resonance imaging for patients with posttraumatic radial wrist tenderness.

PURPOSE: To describe fractures revealed by magnetic resonance imaging (MRI) in a series of skeletally mature patients with radial wrist pain after an acute injury and clinically suspected to have a scaphoid fracture. Additionally, we attempted to assess the diagnostic value of radiographs and computed tomography (CT) in patients with scaphoid and other carpal fractures verified by MRI. METHODS: We conducted the study prospectively over a 3-year period on skeletally mature patients presenting at our emergency department with tenderness on the radial side of the wrist after an injury. A total o

A Unique Presentation of Scaphocapitate Syndrome With Extrusion Into the Carpal Tunnel: A Case Report.

BACKGROUND: Scaphocapitate syndrome is a rare variety of perilunate instability, described as a trans-scaphoid, trans-capitate fracture, with rotation of the capitate head either 90° or 180°. METHODS: We present a unique case of scaphocapitate syndrome in which the rotated proximal capitate fragment expelled into the carpal canal. RESULTS: The capitate head was extricated from the carpal tunnel via the volar approach, and was anatomically aligned and fixed through the dorsal approach using two 2.0 mm headless compression screws. The scaphoid fracture was then also reduced through the dorsal

The scaphocapitate fracture syndrome: a case report and literature analysis.

Scaphocapitate fracture syndrome is rare, complex injury. We report an unusual presentation of scaphocapitate fracture syndrome, involving fracture of the scaphoid and capitate associated with volar dislocation of the lunate and scaphoid and the proximal fragment of the capitate in a 30-year-old male after a motor vehicle accident. Computed tomography was found to be helpful for achieving the correct diagnosis. Open reduction and internal fixation was performed. The scaphoid fracture was fixed using a headless compression screw, and the volar displaced proximal fragment of the capitate was red

Lunate Fracture Morphology in Kienbock's Disease-A Computed Tomography Study.

Background  The pathogenesis of Kienbock's disease is poorly understood. The coronal fracture line is acknowledged as a poor prognostic marker in the disease. Other fracture types in the Kienbock's wrist have subsequently been identified. Questions/Purposes  The aim is to study the fracture morphology of the lunate in a cohort of patients using computed tomography. Methods  Patient images were acquired using a set protocol with four-dimensional computed tomography scanning. Images were reviewed by two orthopaedic surgeons and a consultant radiologist. Static and dynamic images were assessed an

[Significance of intraosseous pressure for pathogenesis of Kienböck disease].

Femoral head necrosis is thought to be the consequence of decreased venous drainage. To confirm this pathogenetic model in lunatomalacia, the intraosseous pressure in 16 necrotic, 16 normal lunates and 16 normal capitates was measured. Intraosseous pressure was recorded in different functional positions: neutral position, extension, flexion, venous stasis, and exsanguinated conditions. Both in extension and in flexion, the intraosseous pressures of all bones exceeded those in neutral position; this corresponded to the increase in pressure during venous stasis. The increase in pressure in norma

[SL fusion - no alternative for the treatment of chronic scapholunate dissociation].

INDRODUCTION: The treatment of chronic scapholunate dissociation (SLD) is currently uneven and still presents a major challenge. Both, the p … ing of the scapholunate fusion was achieved after revision surgery, in one patient by including the capitate bone in the fusion. Two patients developed fibrous nonunion. In 2 patients, the SLD persisted, requiring a four-corner fusion and wrist denervation, respectively. In all patients, degenerative changes were seen in the radiocarpal joint at follow-up. In s …

Pin removal in slipped capital femoral epiphysis: the unsuitability of titanium devices.

In an effort to predict which slipped capital femoral epiphysis fixation devices might cause fewer retrieval problems, 27 consecutive implant-retrieval procedures were reviewed, and mechanical tests using many currently available devices were performed. Problems occurred with both stainless steel and titanium devices if implanted for > 1 year. This tendency for increased problems was statistically significant for the titanium group, and analysis of a retrieved titanium screw showed evidence of osseointegration (direct bone-metal contact at > 90% of the interface) using backscattered image scan

Intramedullary Screw and Kirschner Wire Fixation for Unstable Scaphoid Nonunion.

Surgical treatment of scaphoid nonunion is not always successful, often requiring stabilization and bone grafting to achieve healing. Even after intramedullary screw fixation, residual instability may still hinder union. The purpose of this study was to describe the addition of Kirschner wires (KWs) through the capitate and the lunate to supplement an intramedullary screw for temporary enhanced stability, possibly improving healing of unstable fractures. A case-control study reviewing 25 cases with addition of KWs and 19 controls was performed. Demographic and fracture information, time to dia

Magnetic resonance imaging and radiographic assessment of carpal depressions in children with juvenile idiopathic arthritis: normal variants or erosions?

OBJECTIVE: Magnetic resonance imaging (MRI) is playing an increasingly important role in the diagnosis and followup of juvenile idiopathic arthritis (JIA). Carpal depressions are commonly observed in healthy children and in patients with JIA. The aim of our study was to further characterize these depressions in patients with JIA. METHODS: A total of 29 MRI wrist examinations were analyzed. Depressions were classified according to morphology as either tubular or focal. Features including the presence of a vessel related to the depression, evidence of synovitis, bone marrow edema, or loss of jo

HarDNet-based deep learning model for osteoporosis screening and bone mineral density inference from hand radiographs.

PURPOSE: Osteoporosis, affecting over 200 million individuals, often remains unrecognized and untreated, increasing the risk of fractures in older adults. Osteoporosis is typically diagnosed with bone mineral density (BMD) measured by dual-energy X-ray absorptiometry (DXA). This study aims to develop DeepDXA-Hand, a deep learning model using the efficient CNN-based deep learning architecture, for opportunistic osteoporosis screening from hand radiographs. METHODS: DeepDXA-Hand utilizes a CNN-based, HarDNet, approach to predict BMD non-invasively. A total of 10,351 hand radiographs and DXA pai

Trichorhinophalangeal syndrome type I in monozygotic twins discordant for hip pathology. Report on the morphological evolution of cone-shaped epiphyses and the unusual pattern of skeletal maturation.

A pair of monozygotic twin girls with trichorhinophalangeal syndrome type I (TRPS I), followed from 8.3 to 16.1 years of age, is described. Both showed typical dysmorphic features and severe short stature, but only one had Perthes-like changes in the right capital femoral epiphysis. The radiographic findings and evolutionary changes of phalangeal cone-shaped epiphyses (PCSE) of the hands are illustrated in this report. The unusual bone maturation and growth of the twins are also described. Both presented poor growth and delayed bone age until about 13 years, followed by marked acceleration of

Quantifying in vivo kinematics of the wrist during opposite dart-throwing motion: a 4D CT study.

BACKGROUND: The kinematics of the wrist have been widely studied in terms of flexion/extension, radial/ulnar deviation, and the dart-throwing motion (DTM); however, the opposite dart-throwing motion (oDTM)-despite its functional importance in daily activities-remains poorly characterized. This study aimed to quantify in vivo kinematics of the radiocarpal and midcarpal joints and to characterize their motion patterns during oDTM. METHODS: In vivo kinematics of eleven wrists from healthy subjects were assessed using four-dimensional computed tomography (4D CT). A custom-designed device guided w

Nontraumatic hip pain in active children: a critical differential.

A case report of an 8-year-old baseball player who was diagnosed with Legg-Calvé-Perthes (LCP) disease gives an opportunity to consider the many nontraumatic causes of hip pain. Possibilities include slipped capital femoral epiphysis, septic arthritis, transient synovitis, juvenile rheumatoid arthritis, and bone tumor. Radiographs and bone scans are used to document and stage LCP, and to evaluate the effectiveness of treatment. Though the aggressiveness of treatment depends on the disease stage, the treatment of choice is generally nonsurgical containment of the femoral epiphysis with a cast o

Biomechanical comparison of three internal fixations for treatment of slipped capital femoral epiphysis in immature dogs.

The biomechanical strength and stiffness of 3 fixation techniques used to repair acute slipped capital femoral epiphysis were evaluated in bone specimens from immature dogs. A servohydraulic testing machine was used to create slipped capital femoral epiphysis in 9 pairs of femurs by shearing the capital femoral epiphysis along the physis in a craniocaudal direction. The slip was reduced and repaired with 1, 2, or 3 double-pointed, 1.6-mm (0.062-inch) smooth pin(s) and retested. The strength and stiffness of each intact femur (which served as the control) and repaired femur were compared. Resul

Evaluation of an interfragmentary compression system for the repair of equine femoral capital physeal fractures.

Femoral neck and proximal epiphyseal lengths were measured in 37 femurs from 19 cadaver foals that were 1 day to 12 months old to determine the applicability of a human interfragmentary compression system to equine femoral capital physeal fractures. Because components of the implant system are available only in fixed sizes, its use was possible in foals older than 5 weeks of age, but not in younger foals. The 135 degree angle plate conformed best to the equine femur. Femoral capital physeal fractures were created surgically and repaired with the implant system in three foals. Fracture stabilit
