Treatment of antiphospholipid syndrome (APS) is controversial. We report a case of renal microangiopathy in a 40-year-old woman with APS. The nephropathy was isolated without signs of disseminated thrombotic microangiopathy or progressive systemic sclerosis. Similarities with sclerodermatous kidney and an increase in plasma renin activity led us to initiate treatment with aspirin and captopril, with excellent control of the renal syndrome. We believe this therapeutic regimen may be an effective means of treating the renal microangiopathy of APS.

Eighty-one patients with suspected renovascular hypertension (RVH) were studied using 99mTc-MAG3 renography before and after administration of 25 mg captopril (CAP). Among the 81 cases, 22 were proven by renal arteriography or DSA. The results showed that by using CAP, the sensitivity and specificity of renography in RVH diagnosis increased from 66% to 72% and from 33% to 100%, respectively. We conclude that renography is a simple, noninvasive, sensitive and specific method for screening RVH and is suitable for use in hospitals with no gamma-cameras.

We studied the efficacy of captopril, an angiotensin-converting enzyme inhibitor in treating persistent moderate or severe proteinuria in children with various glomerular diseases other than minimal-change nephrotic syndrome. In four children the role of captopril-radionuclide nephrography in the diagnosis of renovascular hypertension was studied. Captopril (0.5-1.0 mg/kg orally) did not affect Iodine-123 Hippuran nephrography in two children with mild renal artery stenosis, but induced an accumulation of the tracer within the kidneys in two children with hemodynamically significant renal artery stenosis. As in adults Iodine 123 Hippuran nephrography appears to be a useful tool to identify patients with significant renal artery stenosis.

The effects on serum lipids of substituting captopril for other antihypertensive drugs were evaluated in 24 essential hypertensives with stable hypercholesterolemia (total cholesterol 250 to 350 mg/dL).
