Review/Perspective: Operations for Cauda Equina Syndromes - "The Sooner the Better".

BACKGROUND: Although most studies recommended that early surgery for cauda equina syndromes (CES) be performed within <48 h., the largest and most comprehensive Nationwide Inpatient Sample Database (NISC) series, involving over 25,000 CES patients recommended that time be shortened to 0-<24 h. In short, CES surgery performed "the sooner the better," was best. METHODS: The 2 major variants of CES include; incomplete/partial ICES, and those with urinary retention/bowel incontinence (RCES). Those with ICES often exhibit varying combinations of motor weakness, sensory loss (i.e. including perinea…

Cauda equina syndrome as the initial presentation of uveal melanoma metastasis.

PURPOSE: Uveal melanoma extension to the central nervous system (CNS) is exceedingly rare, and can occur through optic nerve invasion. We report a rare clinical case that presented with cauda equina syndrome as the initial manifestation of metastasis of choroidal melanoma, and showed neurotropic extension by histopathology. Our patient did not demonstrate any evidence of systemic metastasis otherwise. OBSERVATIONS: A 60-year-old male patient with treated choroidal melanoma in his right eye, with presumed clinical control, developed radiation-induced neovascular glaucoma refractory to medical …

Intradural calcifying pseudoneoplasm of the neuraxis presenting as a cauda equina syndrome.

BACKGROUND: Calcifying nonneoplastic pseudoneoplasms of the neuraxis (CAPNON) have been reported in 59 cases in literature, however, they rarely involve the spinal cord. Owing to the advances in immunohistochemical markers, their structure and origin are better understood now. CASE REPORT: We present the case of a 72-year-old female who had longstanding history of low back pain that exacerbated 20 days prior to the presentation to the emergency room with a frank cauda equina syndrome. The lumbar computed tomography scan showed a hyperdense lesion, suggestive of calcified tumor, whereas the ma…

The cauda equina syndrome in pregnant woman with a massive disc herniation.

Low back pain during pregnancy is a common cause of medical consultation. Although back pain is very common, the incidence of low back pain secondary to lumbar disk herniation in pregnancy is low (1: 10,000). Cauda equina syndrome from lumbar disk herniation is a serious complication. The delay in diagnosis and treatment can be a cause of chronic disability secondary to neurological sequelae. Numerous cases of disk herniation in pregnancy have been reported, however the association of a cauda equina syndrome as a result of disk herniation is rare. A case is presented of cauda equina syndrome i…

A lumbosacral transitional vertebra in the dog predisposes to cauda equina syndrome.

The association between the occurrence of a lumbosacral transitional vertebra (LTV) and the cauda equina syndrome (CES) in dogs was investigated. In 4000 control dogs without signs of CES, 3.5% had an LTV, while in 92 dogs with CES, 16.3% had an LTV. The lesion causing CES always occurred between the last true lumbar vertebra and the LTV. Dogs with an LTV were eight times more likely to develop CES than dogs without an LTV. German Shepherd dogs were eight times more likely to develop CES compared with other breeds. Male dogs were twice as likely to develop CES than females. Dogs with an LTV de…

Cauda equina syndrome as a postoperative complication of lumbar spine surgery.

BACKGROUND: Musculoskeletal disorders are one of the most common causes that lead to disability throughout the world and routinely present with only limited or short-term symptom relief following traditional treatments. Recent developments, such as platelet-rich plasma (PRP) and mesenchymal stem/stromal cell (MSC) interventions, are being used as regenerative biologic adjuncts in musculoskeletal medicine, yet significant variability exists in preparation strategies, activation methods, and dosing protocols. In addition, clinical interpretation remains limited by inconsistent biologic character

Patients age and results of treatment in discogenic cauda equina syndrome.

Cauda equina syndrome is a feared complication of lumbar disc disease. Despite of early surgery it frequently leaves persistent effects. Localisation, type, and duration of spinal nerve roots compression are quoted as the most important factors influencing the results of treatment. Statistical evaluation of data acquired by examinations of 58 persons operated on for this disease et the Department of Neurosurgery in Kosice within the period from January 1st, 1982 to December 31st, 1991 has shown that the further important facts are ageing of tissues and impairment of microcirculation. In patien…

Spinal epidermoid cyst and cauda equina syndrome in a teenage girl.

A sixteen year old girl presented with a four year history of hip pain followed subsequently by back pain radiating down her left leg, progressive urgency of micturition, urinary incontinence, a feeling of bladder fullness and incomplete bladder emptying, faecal impaction and finally, numbness in both of her buttocks. A diagnosis of Cauda Equina Syndrome was suspected on the history and the clinical examination. A plain X-ray of her lumbar spine revealed evidence of a slow growing mass within the vertebral canal at the level of L3. A Magnetic Resonance Imaging (MRI) scan confirmed an intradura…

[Electromyography and spinal evoked potentials in cauda equina syndrome of dogs].

In order to investigate usefulness of electrodiagnostic methods in Cauda Equina-Compression, 26 dogs with this disease were subjected to electromyography and spinal evoked potentials. 16 normal dogs served as controls. It was found that--after stimulation of the Nn.tibialis, peronaeus, pudendus and coccygeales and recording at the lumbosacral junction--latencies and nerve-conduction velocities were largely normal. However the amplitudes were decreased and--depending on the severity of the compression--the morphology of the potentials was altered although considerable standard deviations were o

Electromyographic abnormalities in redundant nerve root syndrome of the cauda equina.

This report documents the seventh patient with redundant nerve root syndrome of the cauda equina in which electromyographic abnormalities were reported. A 79-year-old woman developed low back pain radiating into the left lower extremity. Electromyography (EMG) revealed partial denervation of the left medial gastrocnemius muscle. Metrizamide myelography disclosed a complete block of the spinal canal at the L3 vertebral level, and a coiled nerve root was present behind the L2 vertebral body. Computed tomography showed marked spondylotic narrowing of the spinal canal at the L3 to L4 vertebral lev

Intramedullary Spinal Glioblastoma in a Cat, Presented as a Cauda equina Syndrome.

A ten year old castrated male domestic short haired cat, was presented with clinical signs related to the cauda equina. The results of the neurological examination, diagnostic procedures, and clinical course are presented. At necropsy a glioblastoma of the caudal spinal cord was present. The difficulties in distinguishing between neoplasms arising in the cauda equina and the caudal spinal cord are discussed. A comparative discussion of the cauda equina syndrome is presented.

Cauda equina syndrome in ankylosing spondylitis: a report of six cases.

Six patients with ankylosing spondylitis and features of a cauda equina syndrome are described. The myelographic findings are discussed in relation to the pathogenesis of the disorder and its natural history. Present experience suggests that the cauda equina syndrome is a more common complication of ankylosing spondylitis than is usually thought.

Updated perspective: STAT surgery for significant cauda equina syndromes.

BACKGROUND: For over a decade, STAT surgery has been the standard of care (SOC) for treating significant cauda equina syndromes (CES). Nevertheless, too many spinal surgeons, physician extenders, and other medical professionals still wrongly believe they have up to 24-48 hours to treat CES. METHODS: To counter this misconception, we reviewed the evolution of treatment for significant CES from the outdated dominant protocols of < 48 h, to 24 - < 48 h, to < 24 h, to 0-< 24 h, to the present SOC; STAT, "prompt", surgery performed "the sooner the better". RESULTS: A 2014 study found that there i…

A Rare Cause of Cauda Equina Syndrome: Neuroendocrine Carcinoma of the Lung.

Cauda equina syndrome (CES) is a rare condition describing the constellation of symptoms resulting from the compression of the cauda equina. Metastatic lesions are a common cause of CES, with lung lesions often implicated as the primary source. A particularly rare cause of CES is leptomeningeal metastasis (LM) from primary solid tumors. In this case, a 63-year-old male presented with urinary and fecal retention, as well as altered sensation in the genitalia. The clinical diagnosis of CES was based on the constellation of symptoms. Computed tomography (CT) imaging demonstrated a metastatic lesi…

The incidence and outcomes of traumatic cauda equina syndrome in Victoria, Australia.

BACKGROUND: Little is known about the incidence, management or long-term outcomes of traumatic cauda equina syndrome (CES), with few cohort studies. The purpose of this study is to establish the incidence and long-term outcomes of traumatic CES in Victoria, Australia. This study is a registry-based cohort study, and people with a diagnosis of traumatic CES from 2010 to 2022 were recruited from the Victorian State Trauma Registry. METHODS: An incidence rate was calculated by dividing the amount of new cases each year by the estimated population in Victoria. Demographic, injury and hospital adm…
