Most cases of atlas burst fracture do not require surgical stabilization, because they can be successfully treated with external immobilization. The authors present two cases of atlas burst fracture in which surgical stabilization was required after external immobilization. The first patient was a 50-year-old male and the second patient was a 34-year-old male. Both presented with neck pain without neurological symptoms after a traffic accident. Neuroradiological examinations revealed atlas burst fracture in both patients. They were initially treated with conservative treatment; one with a rigi

Although ossification of the posterior longitudinal ligament and ligamentum flavum are well known, ossification of the transverse ligament of the atlas is extremely rare. We present the case of a 79-year-old man who developed a gradually progressive spastic quadriparesis caused by upper cervical canal stenosis due to ossification of the transverse ligament of the atlas together with ligamentum flavum hypertrophy.

The results of a computer analysis of 399 patients with documented fractures and/or dislocations of the cervical spine are summarized. Vertebral arch fractures were present in half of all patients with radiographic evidence of cervical spine trauma. Two-thirds of the patients had two or more injuries. Isolated disk injuries were rare and, when present, were characterized by a vacuum sign. The dens fracture, with or without atlantoaxial dislocation, was rarely associated with injury elsewhere in the cervical spine. The study suggests that the routine five-film cervical spine examination is inad

INTRODUCTION: Occipitalization of the atlas is the most common malformation of the craniovertebral junction. It can be diagnosed on lateral teleradiography and its finding imposes screening for associated atlantoaxial instability. In case of instability, brisk movements of the cervical spine during surgery may result in compression and distortion of the spinal chord and vertebro-basilar vascular system.
OBSERVATION: An 18 year-old female patient was referred to our department for facial dysmorphosis and extraction of the third molars. A lateral teleradiography revealed an occipitalization of

Recent onset of head mobility limitations associated with significant stiffness along the vertebral column were the predominating symptomatology in a girl with MURCS association. Occipitoatlantoaxial junction malformation complex and vertebral hyperostosis have been identified. Three-dimensional computerized tomography (3-DCT) scan showed assimilation of the anterior arch of the atlas causing basilar invagination. She also had significant thoracic vertebral hyperostosis closely resembling senile ankylosing vertebral hyperostosis (Forestier disease). We report on what might be a novel constella

A case of burst fracture of the atlas associated with a type II fracture of the odontoid is presented. In this instance no atlanto-axial instability was detected clinically. Treatment consisted of 1 week of rest in bed followed by 3 months in a collar. Management, though conservative, resulted in full recovery.

A new method of dorsal arthrodesis in patients with atlanto-axial instability of different etiology is described. In three patients solid laminar osteosynthesis C1/C2 was achieved by insertion of an autogenous cortico-cancellous bone graft into the interlaminar space and by a bilateral application of the lately developed metal clamp. The technical principles of the clamp guarantee a permanent pressure on the graft and a solid initial stability in the segment. Additional external fixation and immobilization of the patient are not necessary. Complications as known from other procedures of the at

At present, there are few methods that are of any assistance in evaluating the ligaments of the upper cervical spine. It is obvious that a lesion of these ligaments could result in instability of the cranial cervical junction. Nine healthy adults and 45 patients with suspected rotatory instability of the upper cervical spine after trauma were examined using functional CT-scans. After CT scans of the upper cervical spine had been obtained with the patient in a neutral position, the head was passively rotated to the left and right and held in these positions by adhesive tape. CT scans were perfo

OBJECTIVE: To investigate the feasibility and clinical effect of hemi-resection of posterior arch of atlas in the upper cervical spinal dumbbell-shaped schwannomas.
METHODS: A retrospective analysis was performed on 13 patients with high level cervical dumbbell schwannomas from January 2005 to December 2018, including 10 males and 3 females, aged 19 to 67 years old. The occipital foramen to the C1 were 4 cases and 9 cases of C1,2. Tumors were removed by posterior arch of the atlas resection without internal fixation. The clinical efficacy was evaluated by visual analogue pain scale (VAS), Jap

Narrow cervical spinal canal is an important risk factor for the development of cervical myelopathy. Patients with this disease often present with congenital narrowness of the cervical spinal canal. While there are studies on patients with subaxial spinal canal stenosis (SAS), few examined the coexistence of congenital narrow spinal canal in patients with cervical myelopathy at the C1 level. We investigated the characteristics of patients with C1 stenosis (C1S) with special reference to the size of the atlas. Thirteen patients (8 men, 5 women, mean age 76 years) with C1S were retrospectively a
