Perineural Spread of Melanoma to the Brachial Plexus: Identifying the Anatomic Pathway(s).

BACKGROUND: Perineural spread of melanoma is a well-known mechanism of metastasis in cases involving cranial nerves. Brachial plexus involvement is rare, and the pathway is unknown. METHODS: A retrospective review of the Mayo Clinic database was performed to identify patients with a history of melanoma and brachial plexus compromise treated between 1994 and 2017. Inclusion criteria were a history of melanoma, a clinical diagnosis of brachial plexopathy, radiologic features consistent with perineural spread, and biopsy of melanoma within nerves. RESULTS: We identified 42 patients (24 men and

[How many interscalene blocks are there? Reflections on 109 cases studied with various techniques].

The effects of interscalene block were studied on 109 patients undergoing upper extremity elective orthopaedic surgery. Blocks were performed in a non-randomized manner with three different techniques, the site where anaesthetic solution was injected being the main distinguishing mark. The anaesthetic solution was injected into the interscalenic compartment both in the case of patients where classic technique had been carried out and in the group where the nerve stimulator had been used. In the "double needle" technique group, on the contrary, the anaesthetic solution was injected close to the

A complete regional anesthesia technique for cardiac pacemaker insertion.

Sixteen consecutive adult patients scheduled for permanent transvenous cardiac pacemaker insertion received as their total anesthetic the combination of a cervical plexus block and blocks of the second, third, and fourth intercostal nerves using a combination of 1% mepivacaine and 0.2% tetracaine with epinephrine, 1:200,000. This technique consistently provided complete surgical anesthesia of the third cervical (C3) through the fourth thoracic (T4) dermatomes, without anesthesia of the brachial plexus. Anesthesia was adequate for the surgical procedure without the need for supplemental analges

Combined sevoflurane-dexmedetomidine and nerve blockade on post-surgical serum oxidative stress biomarker levels in thyroid cancer patients.

BACKGROUND: The incidence of thyroid cancer is increasing annually. Clinical routine thyroid surgery can be performed under a cervical plexus block, but cannot mediate the stress response during the surgery. If thyroid surgery is performed under nerve block, an inappropriate level of blockade may occur. Similarly, the stress response caused by surgery is more serious than that caused by conventional anesthesia. Therefore, it is important to combine blockade with more effective anesthesia methods. AIM: To investigate the effects of combining sevoflurane-dexmedetomidine inhalation general anest

Spinal Astrocytoma with Brachial Plexus-like Presentation: A Report of 2 Cases.

CASE: We present 2 cases of spinal astrocytoma in infants mistaken for brachial plexus birth injuries (BPBI). (1) A 5-month-old infant had upper-extremity weakness resembling a global plexus injury. An magnetic resonance imaging (MRI) and biopsy revealed a low-grade cervical astrocytoma. The patient underwent chemotherapy with partial recovery of upper extremity motor function. (2) A 5-month-old infant had upper-extremity weakness resembling upper trunk BPBI. MRI and biopsy revealed pilocystic astrocytoma. The patient underwent spinal decompression and proton therapy but did not regain functio

Giant intraspinal pseudomeningoceles cause delayed neurological dysfunction after brachial plexus injury: report of three cases.

OBJECTIVE AND IMPORTANCE: Delayed neurological dysfunction after a brachial plexus injury is uncommon. We present the cases of three patients with a history of significant brachial plexus trauma and late neurological deterioration secondary to giant intraspinal extradural pseudomeningoceles. CLINICAL PRESENTATION: Three patients, each with a remote history of brachial plexus trauma, presented with slowly progressive upper-limb weakness. An examination revealed bilateral lower motor neuron weakness in the upper extremities in all patients and evidence of spastic paraparesis in one. Magnetic re

Cervical spinal stenosis and stingers in collegiate football players.

We evaluated the relationship of cervical spinal stenosis with the occurrence of "stingers" in collegiate football players who participated at our institution from 1987 through 1991. Preparticipation cervical spine radiographs of 266 players were used to measure Torg ratio. Forty players with stingers were identified: 34 had an extension-compression mechanism; 6 had a brachial plexus stretch mechanism. Time-loss neck injuries occurred in 31 players; the remaining 195 players were asymptomatic. The mean Torg ratio was significantly smaller for the stinger group (P = 0.02). The Torg ratio was le

Neurotization in brachial plexus injuries. Indication and results.

In neurotization or nerve transfer, a healthy but less valuable nerve or its proximal stump is transferred in order to reinnervate a more important sensory or motor territory that has lost its innervation through irreparable damage to its nerve. In brachial plexus injuries, extraplexal nerves such as the spinal accessory nerve, rami of the cervical plexus, or intercostal nerves are transferred onto trunks, cords, or individual nerves or else segments of the brachial plexus that maintain continuity with the spinal cord may be coapted to trunks or cords the surgeon wishes to innervate. Intrathoracic ganglioneuroma surrounding left vertebral and subclavian arteries successfully resected by modified trapdoor thoracotomy.

Neurogenic tumors are derived from tissue of the neural crest. Most of the mediastinal neurogenic tumors occur in the posterior compartment. Extension to the cervical area and the surrounding major arteries is rare. We report a patient with a ganglioneuroma arising from the brachial plexus, extending into the cervical region, and surrounding the left vertebral and subclavian arteries. The patient underwent successful resection by modified trapdoor thoracotomy.

[Launois-Bensaude disease. Treatment with wide transverse cervicotomy].

Launois-Bensaude disease (known in English-speaking countries as Madelung's disease) is a diffuse, symmetrical lipomatosis predominant on the neck, face and trunk. All the anterior cervical lipomas can be removed by a wide transverse cervicotomy without danger for the major neural structures (spinal, and facial nerves, cervical plexus). This technique was used in three patients with satisfactory cosmetic and functional results and without recurrence.

Combined extravasal compression of the right subclavian artery caused by stellate ganglion schwannoma and additional rudimentary cervical rib. Clinical case and literature review.

Unlike widespread and well-studied atherosclerosis of supra-aortic arteries, non-atherosclerotic vascular diseases are less common. Of these, one of the most unusual is extravasal compression by additional structures between scalene muscles. Close topographic and anatomical connections of scalene muscles, vertebral and subclavian arteries, brachial plexus and stellate ganglion can lead to complex syndrome including impaired blood supply to the arm, brachial plexus compression and vertebrobasilar circulatory disorders. The main treatment method for supraclavicular compression syndromes is surge

[A case of juvenile-type distal and segmental muscular atrophy of upper extremities (Hirayama disease) with the isolated cervical fusion at the C3-C4 levels].

The case, 29-year-old male, had suffered from muscular weakness and atrophy of the bilateral forearms and hands with tremor of the bilateral fingers for about 13 years. A neurological examination showed normal muscle-stretch reflexes and no sensory disturbances. A cervical spinogram revealed a fusion at the C3-C4 levels and mild spondylotic changes. We clinically diagnosed him as juvenile-type distal and segmental muscular atrophy of upper extremities (Hirayama disease) with the isolated congenital cervical fusion. Magnetic resonance imaging demonstrated an enlargement of the anterior epidural

Magnetic Resonance Imaging and Clinical Findings Associated with Choroid Plexus Spinal Cord "Drop" Metastases.

A 5 yr old castrated male whippet presented with a unique presentation of ambulatory paraparesis and subsequent diagnosis of primary intracranial choroid plexus carcinoma, with metastases to the cervical, thoracic, lumbar, and sacral spinal cord segments. Magnetic resonance imaging was performed initially of the thoracolumbar vertebral column and was followed by MRI sequences of the brain for confirmation of the presence of a primary intracranial tumor. The dog was euthanized immediately following diagnostic imaging due to the severity of clinical signs and poor prognosis. Postmortem examinati

Supraclavicular radical scalenectomy and transaxillary first rib resection for the thoracic outlet syndrome. A combined approach.

Transaxillary resection of the first rib alone was performed 97 times to relieve symptoms of irritation of the brachial plexus. Persistent or recurrent symptoms occurred in a fifth of the patients (7 and 13 patients, respectively), and necessitated reoperation using the supraclavicular approach. In all patients, at least one anomaly or acquired deformity was found that could not have been identified or safely removed by the original transaxillary approach alone. Subsequently, 94 combined operations, including supraclavicular radical scalenectomy with neurolysis of the brachial plexus and trans

Successful management of interdural spinal cysts presenting as radiculopathy caused by epidural venous enlargement: illustrative case.

BACKGROUND: The authors report the first case of thoracic interdural spinal cysts presenting as radiculopathy attributed to overdrainage-related cervical venous plexus enlargement. This case emphasizes the importance of considering interdural spinal cysts and cerebrospinal fluid overdrainage in the differential diagnosis of radiculopathy. OBSERVATIONS: A 37-year-old male patient with a history of orthostatic headache presented with bilateral deltoid muscle atrophy consistent with C5 radiculopathy. Postcontrast magnetic resonance imaging (MRI) revealed cervical epidural venous plexus enlargeme

Unexpected Anatomical Variation While Performing an Ultrasound-Guided Interscalene Block for Shoulder Surgery.

Anatomical variations of the brachial plexus are very common. Knowledge of the possible anatomical variations encountered in ultrasound imaging is crucial for the safe and effective practice of regional anesthesia. The interscalene block (ISB) targets the brachial plexus roots in the interscalene groove, between the anterior and middle scalene muscles (MSM), at the level of the sixth cervical vertebra. Blockade of the brachial plexus roots anesthetizes the shoulder region, making the ISB one of the preferred regional anesthesia options in shoulder surgeries. Abnormalities of the muscular struc

Thoracic Outlet Syndrome from Bilateral Cervical Ribs -A Clinical Case Report.

INTRODUCTION: Cervical rib is a mesenchymal or cartilaginous elongation of the transverse process of usually the seventh, rarely the sixth, and very rarely the fifth cervical vertebrae. It is an important cause of thoracic outlet syndrome as it has been reported in 5-% of patients with thoracic outlet syndrome. Bilateral cervical rib is a rare anomaly with a prevalence of 0.56% in Enugu, South Eastern Nigeria. This case is reported to emphasize the possibility of bilateral cervical rib as a cause of thoracic outlet syndrome in our environment. CASE REPORT: A 28-year-old female soldier was ref

Neurotization for restoration of arm function in complete cervical root avulsion.

Neurotization procedures represent an important therapeutic option in patients with complete root avulsion due to traumatic brachial plexus injuries. A variety of normal donor nerves can be used, including intercostal nerves, accessory nerve, parts of the cervical plexus, phrenic nerve, and/or the contralateral C7 nerve root. The reconstructive neurotization procedure should be performed within 3-6 months following the trauma and, when successful, can lead to substantial improvement in motor function for the shoulder and upper arm and in sensory function for the forearm, hand and finger region
