Cartilage-hair Hypoplasia Complicated with Liver Cirrhosis Due to Chronic Intrahepatic Cholestasis.

We herein report a rare case of cartilage-hair hypoplasia (CHH) complicated with liver cirrhosis. A 20-year-old Japanese man with CHH was found incidentally to have liver cirrhosis and an esophageal varix. This patient had been treated for infections due to immunodeficiency since early childhood. He ultimately died of liver failure at 31 years of age. An autopsy revealed an abnormality of the interlobular bile ducts and intrahepatic cholestasis. Liver cirrhosis was thought to have been caused by chronic intrahepatic cholestasis due to biliary duct hypoplasia and changes in the intestinal micro

A Molecular Mechanism Underlying Genotype-Specific Intrahepatic Cholestasis Resulting From MYO5B Mutations.

BACKGROUND AND AIMS: Progressive familial intrahepatic cholestasis (PFIC) 6 has been associated with missense but not biallelic nonsense or frameshift mutations in MYO5B, encoding the motor protein myosin Vb (myoVb). This genotype-phenotype correlation and the mechanism through which MYO5B mutations give rise to PFIC are not understood. The aim of this study was to determine whether the loss of myoVb or expression of patient-specific myoVb mutants can be causally related to defects in canalicular protein localization and, if so, through which mechanism. APPROACH AND RESULTS: We demonstrate th

Progressive familial intrahepatic cholestasis type-3 and multiple sclerosis: lessons from comorbidity.

The comorbidity between multiple sclerosis (MS) and progressive familial intrahepatic cholestasis type-3 (PFIC3) has never been described yet. ABCB4 gene encodes the multidrug resistant protein 3 (MDR3) and its mutations induce PFIC3 as well as intrahepatic cholestasis of pregnancy (ICP) and drug-induced liver injury (DILI). We describe the case of a 32-year-old female with MS and PFIC3 who was effectively treated with natalizumab and ursodeoxycholic acid (UCDA), in contrast to glatiramer acetate, dimethylfumarate, and IFNb1a associated with DILI. Our findings clarify the pharmacodynamics of M

Comprehensive bile acid profiling in hereditary intrahepatic cholestasis: Genetic and clinical correlations.

BACKGROUND & AIMS: Genetic defects causing dysfunction in bile salt export pump (BSEP/ABCB11) lead to liver diseases. ABCB11 mutations alter the bile acid metabolome. We asked whether profiling plasma bile acids could reveal compensatory mechanisms and track genetic and clinical status. METHODS: We compared plasma bile acids in 17 ABCB11-mutated patients, 35 healthy controls and 12 genetically undiagnosed cholestasis patients by ultra-high-performance liquid chromatography/multiple-reaction monitoring-mass spectrometry (UPLC/MRM-MS). We developed an index to rank bile acid hydrophobicity, and

Cross-Sectional Analysis of Progressive Familial Intrahepatic Cholestasis in Puerto Rican Children.

OBJECTIVE: Specific inherited disorders may be more common in island communities. Prior case reports suggest that cholestatic liver diseases may constitute a group of these inherited disorders in Puerto Rico. A cross-sectional survey of liver diseases in children was conducted to assess this hypothesis. METHODS: A cross-sectional analysis was performed in patients with chronic cholestasis at "Hospital Pediátrico Universitario" in San Juan, Puerto Rico. Ten potential participants with high gamma-glutamyl transpeptidase (GGTP) cholestasis were identified. Therapeutic response to ursodeoxycholic

Familial intrahepatic cholestasis associated with progressive neuromuscular disease and vitamin E deficiency.

Three Japanese patients with familial progressive intrahepatic cholestasis developed complications involving neurologic abnormalities characterized by ataxia and pigmentary retinopathy. Serum vitamin E concentrations were extremely low in all patients, suggesting a long-term vitamin E deficiency. High dose oral supplementation of alpha-tocopherol produced normal serum vitamin E levels in two patients. Parenteral administration of vitamin E resulted in no clinical improvement in one patient who first received the treatment at 14 years of age. In the other two patients, the progression of neurol

Progressive familial intrahepatic cholestasis disease burden and clinical approaches: a systematic review.

Aim: Progressive familial intrahepatic cholestasis (PFIC) comprises a group of rare, heterogeneous genetic liver disorders characterized by impaired bile formation and cholestasis. Historically, treatment focused on supportive management and symptomatic relief, but disease-specific therapies, including ileal bile acid transporter inhibitors, have recently become available. This systematic review updates previous evidence on the epidemiology, natural history, psychosocial and economic burden of PFIC, and summarizes evidence on the efficacy, safety and cost-effectiveness of therapies used primar

Identifying patient-centered outcomes in progressive familial intrahepatic cholestasis: Results from IMPACT.

OBJECTIVES: In rare diseases like progressive familial intrahepatic cholestasis (PFIC), lack of evidence results in treatment variability and strategies for families to engage care teams to understand how variability affects overall health are unknown. Patient-centered comparative effectiveness research (CER) has been identified as an approach to address these challenges, but CER-specific knowledge and infrastructure are lacking. We present the results from IMPACT (Identifying research targets by Merging Patient And Clinician Treatment information), a 2-year capacity-building project for patie

Expanded dengue syndrome complicated by fulminant liver failure and prolonged intrahepatic cholestasis.

We report a case of expanded dengue syndrome in a 38-year old man who developed fulminant liver failure and prolonged intrahepatic cholestasis. Initially presenting with fever, weakness, and jaundice, he was diagnosed with dengue by positive NS1 antigen, polymerase chain reaction, and Immunoglobulin M tests. Despite supportive care, the patient progressed to severe hepatic encephalopathy and coagulopathy ensued. Management included N-acetylcysteine infusion, thromboelastography-guided transfusions and sedation with propofol. Persistent cholestatic jaundice was managed with ursodeoxycholic acid

Xenobiotic Nuclear Receptor Signaling Determines Molecular Pathogenesis of Progressive Familial Intrahepatic Cholestasis.

Progressive familial intrahepatic cholestasis (PFIC) is a genetically heterogeneous disorder of bile flow disruption due to abnormal canalicular transport or impaired bile acid (BA) metabolism, causing excess BA accumulation and liver failure. We previously reported an intrahepatic cholestasis mouse model based on loss of function of both farnesoid X receptor (FXR; NR1H4) and a small heterodimer partner (SHP; NR0B2) [double knockout (DKO)], which has strong similarities to human PFIC5. We compared the pathogenesis of DKO livers with that of another intrahepatic cholestasis model, Bsep-/-, whic

Reversal of liver function without exchange transfusion in sickle cell intrahepatic cholestasis.

Sickle cell intrahepatic cholestasis (SCIC) is a rare but fatal complication of sickle cell disease. It is found mainly in homozygous sickle cell disease. To date, there are no standard diagnostic criteria or well-established therapeutic approaches to this condition. Herein, we report this case of a 48-year-old man with sickle cell anemia and a total bilirubin of 78.5 mg/dL without evidence of extrahepatic biliary obstruction or viral hepatitis. The patient had a hemoglobin S level of 87.9%, acute renal failure, and mild coagulopathy. Despite the disease severity, he refused exchange transfusi

Neonatal intrahepatic cholestasis caused by citrin deficiency (NICCD) in three Malay children.

Citrin deficiency is an autosomal recessive disorder caused by mutation in the SLC25AJ3 gene. It has two major phenotypes: adult-onset type II citrullinemia (CTLN2) and neonatal intrahepatic cholestatic caused by citrin deficiency (NICCD). NICCD is characterized by neonatal/infantile-onset cholestatic hepatitis syndrome associated with multiple amino acidemia and hypergalactosemia. NICCD is self-limiting in most patients. However, some patients may develop CTLN2 years later, which manifests as fatal hyperammonemia coma. We report three unrelated Malay children with genetically confirmed NICCD

Jaundice and intrahepatic cholestasis following high-dose megestrol acetate for breast cancer.

High-dose megestrol acetate, a synthetic progestin, has been advocated recently in treating patients with metastatic breast carcinoma; no significant increase in adverse effects has been reported. This report describes a patient with jaundice and intrahepatic cholestasis after high-dose megestrol acetate therapy. This cholestatic lesion may have a pathogenesis similar to that observed with estrogens and oral contraceptives.

Benign recurrent intrahepatic cholestasis: studies of bilirubin kinetics, bile acids, and cholangiography.

Three patients with benign recurrent intrahepatic cholestasis are described. They had had between five and 16 attacks of cholestasis. Between attacks the liver function tests, including serum bile acids, were normal. Serial serum bilirubin and bile acid estimations during the cholestasis in one patient revealed a consistent discrepancy between the serum bilirubin and bile acid concentrations during three consecutive attacks. In the other two patients the serum concentrations of bile acids and bilirubin varied in parallel. Analysis of the individual serum bile acids did not reveal high concentr

Placental Oxidative Stress and eNOS Alterations in Intrahepatic Cholestasis of Pregnancy Following ICSI.

Background: Pregnancies following in vitro fertilization (IVF) are associated with higher rates of intrahepatic cholestasis of pregnancy (ICP) compared to spontaneous conceptions. This study aimed to investigate oxidative stress biomarkers, endothelial nitric oxide synthase (eNOS) expression, and placental histopathology in ICP women undergoing intracytoplasmic sperm injection (ICSI). Methods: Placental samples from four groups-healthy spontaneous conception (non-ICP-SC, n = 7), ICP spontaneous conception (ICP-SC, n = 7), healthy ICSI (non-ICP-ICSI, n = 7), and ICP-ICSI (n = 7)-were analyzed.

Outcomes of subsequent pregnancy following intrahepatic cholestasis of pregnancy: A retrospective cohort study.

OBJECTIVE: Data on subsequent pregnancies after intrahepatic cholestasis of pregnancy (IHCP) are limited. We aimed to determine IHCP recurrence, identify risk factors, and describe outcomes in subsequent pregnancies. METHODS: This retrospective study at a tertiary center (2012-2024) included individuals with prior IHCP. Multiple gestations and those without a documented subsequent pregnancy were excluded. We assessed recurrence, maternal and neonatal outcomes, and compared characteristics between those with recurrent IHCP (rIHCP) and those without. Multivariable regression estimated odds rati
