Significantly displaced medial clavicle fractures are associated with high rates of painful nonunion. Operative fixation can be challenging due to limited medial bone stock and adjacent vital vascular structures. Twenty-one consecutive chest computer topography (CT) scans were analyzed to measure anterior-posterior and superior-inferior dimensions of the medial clavicle. Correlation between height and clavicular dimensions were assessed by Pearson correlation coefficient. Two cases using dual T-locking plates are described. From anterior to posterior, the medial clavicle typically measures 1.4

Fractures of the distal clavicle represent 15-30% of all clavicle fractures. The local osseoligamentous anatomy and deforming forces result in increased risk of delayed union and nonunion than fractures in other parts of the clavicle. These factors also contribute to challenges in fracture repair. Understanding these injuries and their imaging features enhances care and ensures patients are directed to appropriate management. We review the anatomy of the distal clavicle and surrounding ligaments, options for radiographic evaluation, relevant classification systems, and current concepts in mana

OBJECTIVE: To assess the results of the treatment of fractures of the middle third of the clavicle with a pre-contoured plate in athletes.
METHODS: The authors performed 26 osteosyntheseis in 25 patients with fractures of the middle third of the clavicle. The fractures were classified according to Robinson as 16 type B1 and ten type B2. The mean age was 37 years, ranging from 15 to 63 years, 20 patients were male and five were female, and all play sports in an amateur or professional manner. All patients were operated on in the acute phase of fracture within five days after trauma. Patients w

BACKGROUND: Clavicle nonunions in adolescent patients are exceedingly rare. The purpose of this study was to evaluate a series of clavicle nonunions from a pediatric multicenter study group to assess potential risk factors and treatment outcomes.
METHODS: A retrospective review of all clavicle nonunions in patients younger than 19 years was performed at 9 pediatric hospitals between 2006 and 2016. Demographic and surgical data were documented. Radiographs were evaluated for initial fracture classification, displacement, shortening, angulation, and nonunion type. Clinical outcomes were evaluat

No level 1 evidence is available to guide the surgical treatment of adolescent clavicle fractures. Adult literature is not applicable as adolescent mid-diaphyseal clavicle fractures do not develop nonunions, and only a small percentage (10% to 20%) are symptomatic from malunions. Current indications for operative fixation are: (1) completely displaced midshaft fracture with shortening of >2 cm; (2) superior displacement with skin tenting and/or an impending open fracture; (3) associated neurovascular injury; (4) open clavicular fracture; and (5) floating shoulder with a completely displaced cl

PURPOSE: To evaluate the rate of scapular dyskinesis and resulting patient outcomes after treatment of displaced midshaft clavicle fractures.
METHODS: Skeletally mature patients with isolated, displaced midshaft clavicle fractures treated with or without surgery over a 16-month period were recruited. The minimum length of follow-up at study examination was 12 months. Patient outcomes were documented using the SICK (scapular malposition, inferomedial border prominence, coracoid pain and malposition, and dyskinesis of scapular movement) Scapula Rating Scale, the Simple Shoulder Test, 3 visual a

A 22-year-old man presented with a painful 'clunking' sensation in the right mid-clavicle, and pain and dysaesthesia along the medial aspect of his right arm and hand. Three months earlier, he had been involved in a vehicle accident and sustained a right clavicle fracture. He had a large step off of the right clavicle with a medialisation of the right shoulder. At 90° abduction in external rotation of both shoulders he developed pain, paraesthesia and disappearance of the right radial artery pulsation. CT of the right shoulder in the neutral position demonstrated the clavicle-to-first rib dist

BACKGROUND: Nonoperative treatment of displaced medial clavicle fractures often leads to poor functional outcomes and painful nonunions. This study investigates the functional outcomes of patients undergoing operative fixation of these fractures.
METHODS: We investigated 27 patients undergoing operative fixation of a medial clavicle fracture; 24 had an acute, displaced fracture and 3 had fixation for nonunions. Preoperative radiographs or computed tomography scans were obtained, and data collected included age, sex, mechanism of injury, and fixation method. Follow-up included physical examina

Arthroscopic shoulder surgery is considered a safe and effective method of treating a variety of shoulder pathologies and is associated with a low complication rate. The type and rate of complications can vary, depending on the procedure, positioning, surgical time, and anesthesia. Fortunately, neurovascular injuries occur infrequently. Numerous studies have described the proximity of neurovascular structures to portals placed in shoulder arthroscopy, in both the beach chair and the lateral decubitus positions. Accurate portal placement is important to avoid damage to adjacent neurovascular st

We conducted a prospective, randomized study to determine if patients with midshaft clavicle fractures would benefit from immediate operative stabilization with a modified Hagie pin in comparison with a matched group treated with nonoperative therapy. At a level II trauma center, patients with closed midshaft clavicle fractures were prospectively randomized to receive either operative or nonoperative treatment. Fifty-seven (29 operative, 28 nonoperative) patients were enrolled in the study. Operative patients underwent open reduction and internal fixation of the clavicle using a modified Hagie

The purpose of this study was to assess outcomes following open distal clavicle resection for acromioclavicular joint arthritis or distal clavicle osteolysis, with and without associated acromioplasty. Patients with painful clinical findings limited to the acromioclavicular joint had isolated distal clavicle excision (23 shoulders). Patients with acromioclavicular joint abnormalities and rotator cuff tendinopathy also underwent acromioplasty (41 shoulders). At average follow-up of 8.3 years, pain scores improved from 4.7 (1 to 5 scale) to 2.3 (p < 0.001). Patient satisfaction improved from 1.8

This article describes a new technique for the arthroscopic treatment of distal clavicle fractures. This technique requires the use of posterior and anterior standard arthroscopic portals. The base of the coracoid process is exposed through the rotator interval. The ancillary system drill guide is placed at the undersurface of the coracoid process. A small incision is performed above the clavicular body and the corresponding part of the guide is pushed down to the clavicle. A 4-mm hole is drilled through the clavicle and the coracoid process. A double button device is pushed through both the h

Pseudoaneurysm of the subclavian artery is a rare but serious condition generally occurring as a result of penetrating injury to the shoulder region, often iatrogenic. As well as the potential for distal ischaemia, this condition can result in compression of adjacent structures. Untreated it can lead to rupture with life-threatening haemorrhage. We describe the unusual case of a patient with a background of metastatic breast carcinoma who presented with a pseudoaneurysm of the subclavian artery secondary to a pathological fracture of the clavicle. The patient was managed surgically due to the

PURPOSE: To assess the treatment outcome of temporary coracoclavicular screw fixation for displaced distal clavicle fractures.
METHODS: From January 1995 to December 2003, 30 consecutive patients with Neer type II displaced distal clavicle fractures were treated with open reduction and internal fixation using a coracoclavicular screw. The screw was removed under local anaesthesia after bony union. Patients were evaluated by clinical and radiological examination. Functional outcome was assessed by a simple shoulder test.
RESULTS: There was 100% union rate. All patients returned to their pre-i

Kirschner pins are widely used for fixation of bone, with few and very rarely serious complications. A case is described in which a Kirschner pin migrated in 10 days from the sternal end of the left clavicle into the lumen of the ascending aorta. The exact location of the pin remained unknown until right thoracotomy, during which X-ray examination revealed the unexpected situation. The pin was simply extracted by pushing it through the vessel wall. The patient recovered uneventfully. The complication could have been prevented by bending the outer part of the pins. The possible route of migrati

Following traumatic retrosternal dislocation of the clavicle, the patient presented with thoracic outlet syndrome 6 months postinjury with swelling and cyanosis of the right upper extremity, and cramps of the right arm and forearm after heavy manual work. When open reduction of the dislocated clavicle failed, the medial half of the clavicle was resected. Four years postoperation, the patient is doing well, and is asymptomatic. A review of the literature, anatomy, mode of injury, mechanism of injury, pathology, clinical picture, diagnosis and treatment are discussed.
