Clinical reasoning: a 40-year-old man with CIDP-like illness resistant to treatment.

A 40-year-old man developed tingling and numbness in the feet 2 years ago. Three months later, he noticed difficulty standing on his toes. Outside evaluation showed a small immunoglobulin G (IgG) lambda paraprotein, elevated CSF protein of 335 mg/dL (<40 mg/dL), and nerve root thickening with mild gadolinium enhancement in the cauda equina region on lumbar spine MRI. He was presumed to have Guillain-Barré syndrome (GBS) and was treated with IV immunoglobulin (IVIg).

The patient was successfully treated with combination chemotherapy. Usually, in practice, clinical reasoning and decision-making is carried out in accordance with Bayes' theorem. But when the a priori probability of disease is unknown and the likelihood ratio of a diagnostic test unavailable, one has to combine the available 'evidence' with critical thinki …

[Clinical reasoning and decision-marking in practice. A general practitioner with long-standing anaemia].

In order to calibrate his haemoglobin meter, a general practitioner had his own haemoglobin level checked regularly at a nearby laboratory. At 37 years of age, one of these assays revealed 7,0 mmol/l. Because he had no symptoms, he prescribed himself iron-substitution therapy but the haemoglobin concentration did not normalise. At 52 years of age, the patient increasingly suffered from fatigue and exertional dyspnoea and had intermittent abdominal pain, distension and rumbling. Biopsies of the small intestine revealed crypt hypertrophy and intraepithelial lymphocytosis: signs ofcoeliac disease

Clinical reasoning about new symptoms despite preexisting disease: sources of error and order effects.

BACKGROUND: Previous work that studied the evaluation of new, atypical symptoms in patients with preexisting diseases indicated that physicians largely ignored the past medical history and therefore erred in their diagnoses, when compared to a Bayesian analysis. Other studies have shown that the order in which information is presented to a decision maker can affect the inferences drawn, again contrary to a Bayesian standard. OBJECTIVES: The aim of the study was to investigate the source of disparity between clinical judgment and Bayesian analysis and to investigate the effect of alternative o

Theory-informed redesign of clinical reasoning education in the era of generative artificial intelligence.

Clinical reasoning is a foundational competency that develops over the course of training and comprises two sub-components. Diagnostic reasoning classifies a patient's condition and assigns a diagnostic label, whereas management reasoning weighs multiple defensible plans with consideration of patient preferences and contextual constraints. Clinical reasoning education has historically emphasized "in-the-head" cognitive processes, such as information gathering, hypothesis generation, problem representation, and differential prioritization, over "out-in-the-world" contextualized processes, such …

Reasoning or reciting? A temporal contamination audit of large language models in clinical medicine.

OBJECTIVE: Evaluate whether large language models reason or simply regurgitate training data in clinical diagnosis. MATERIALS AND METHODS: … h lexical and semantic similarity metrics, provides compelling evidence that LLMs engage in genuine clinical reasoning rather than regurgitating memorized training data. CONCLUSION: Models demonstrated equivalent accuracy on cases they could not have seen during training, suggesting they have internalized generalizable medical knowledge rather than memorizing speci …

The Detective Nurse Practitioner: A Clinical Reasoning Pathway to Enhance Advanced Health Assessment Skills.

BACKGROUND: Clinical reasoning is frequently underdeveloped in graduate nurse practitioner (NP) education. The National Organization of Nurse Practitioner Faculties Role Competencies define what NP students must achieve, and the Professional-Reporter-Interpreter-Manager-Educator-Nurse Practitioner (PRIME-NP) Model describes the sequence in which they develop. PROBLEM: Advanced Health Assessment students can perform a thorough physical examination but struggle to determine relevant findings, prioritize working differentials, and translate assessment data into provider-level clinical decisions.…

Comparative evaluation of large language models and clinicians in real-world glaucoma clinical reasoning.

PURPOSE: Clinical decision-making in glaucoma is complex and requires integration of heterogeneous information, including patient history, examination findings, and risk stratification. While artificial intelligence (AI) has shown strong performance in image-based ophthalmic tasks, its capability in specialty-specific clinical reasoning remains insufficiently explored. METHODS: Performance was evaluated by glaucoma specialists using a predefined rubric across three clinically oriented domains: medical accuracy (40%), key-point recall (30%), and logical completeness (30%). The weighted composi…

Clinical Reasoning: A 35-Year-Old Man With Recurrent Asymmetric Lumbosacral and Cervical Plexopathy.

A previously well 35-year-old right-handed man developed abrupt severe lower back pain radiating to the anteromedial and anterolateral right thigh. The pain began as constant with superimposed lancinating exacerbations and was uniquely provoked by minimal stretch maneuvers, such as straight leg raise beyond 20° or hip extension, leading to functional immobility. Within 2 weeks, he developed prominent proximal right lower limb weakness. Over the following 3 months, his course was characterized by repeated relapses with episodic worsening of pain and new contralateral involvement despite cortico

Clinical Reasoning: A 49-Year-Old Man With Meningoencephalitis and Persistent Altered Mental Status.

Persistent altered mental status in acute meningoencephalitis poses a significant diagnostic and therapeutic challenge. We report a 49-year-old man who presented with fever, headache, and status epilepticus. Despite initial positive bacterial cultures and appropriate antimicrobial therapy, his mentation remained poor. MRI brain suggested a likely diagnosis, which was subsequently confirmed on autopsy. This case highlights the importance of a systematic approach in evaluating persistent altered mental status in meningoencephalitis, incorporating a detailed clinical history, comprehensive system

Clinical Reasoning: A 67-Year-Old Woman With Progressive Headache, Visual Hallucinations, and Seizures.

A 67-year-old woman was admitted to the hospital for 3 months of new-onset headaches and meningismus that progressed to focal neurologic deficits. These first manifested as episodes of flashing lights and black blotches obscuring her vision for minutes. She also began to have visual hallucinations with formed letters and words projected onto nearby walls. She developed gait unsteadiness and difficulty using her phone and reading a clock. She was seen by neuro-ophthalmology and found to have a left homonymous hemianopsia, at which point she was admitted for further evaluation. Physical examinat

Clinical Reasoning: A 63-Year-Old Female Patient Presenting With Orthostatic Hypotension and Ataxia.

A 63-year-old female patient presents with orthostatic hypotension and slowly progressive cerebellar ataxia. Neurologic examination revealed ataxia, pyramidal signs, and vibration/proprioception deficits. Neuroimaging showed diffuse white matter abnormalities, cord atrophy, and corticospinal tract involvement. This case highlights how a careful history, with special attention to tempo of symptoms and family history, combined with examination findings and neuroimaging can be used to reach a rare diagnosis.

Clinical Reasoning: A 62-Year-Old Man With Symmetric Saddle Hypoesthesia and Sphincter Dysfunction.

A 62-year-old man presented with acute onset of symmetric burning dysesthesias in the buttocks, accompanied by saddle hypoesthesia and bladder, bowel, and sexual dysfunction. Initially, the clinical presentation was suggestive of cauda equina syndrome. However, the initial contrast-enhanced lumbar MRI revealed no evidence of extrinsic compression or enhancement of the lumbosacral nerve roots or conus medullaris. Further diagnostic workup uncovered a rare etiology, highlighting the importance of considering uncommon causes in the case of acute conus medullaris syndrome.

Clinical Reasoning: A 2-Year-Old Girl With Acute Encephalopathy After Febrile Systemic Illness.

A 2-year-old girl presented with an acute encephalopathy with hemiparesis after febrile systemic illness. MRI showed multifocal symmetrical lesions in both thalami, and CSF showed a slightly elevated protein. The final diagnosis in this patient led to a revision of the diagnosis of neuroborreliosis in her mother, 15 years earlier. The bilateral, symmetric thalamic involvement at presentation was an important clue to suggest the final diagnosis in the mother. This case highlights common clinical pitfalls and important neuroimaging findings that led to the final diagnosis in the case of an acute

Clinical Reasoning: A 60-Year-Old Woman With Rapidly Progressive Muscle Weakness and Ophthalmoparesis.

The clinical assessment of patients with proximal muscle weakness represents a frequent yet intricate challenge. We present the case of a 60-year-old woman who experienced a progressive, symmetrical weakness in proximal limbs and bulbar muscles over 6 months. Notable clinical findings included bilateral ophthalmoparesis; widespread muscle atrophy; and pronounced weakness in craniobulbar, cervical, and proximal muscles, more severe than in distal ones. We elucidate a methodical diagnostic approach, focusing on constructing a comprehensive differential diagnosis and identifying potential causes

Physicians have feelings: illuminating the relationship between emotional valence, clinical reasoning and context specificity.

INTRODUCTION: Research demonstrates that emotions play an important role in clinical reasoning (CR); however, the relationship between emotional valence, CR, and the context in which reasoning takes place, remains to be empirically explored. While situated cognition has been used to investigate CR and context specificity (e.g. the presence of contextual factors, things other than the information directly related to establishing a diagnosis), it has not explicitly examined the role of emotional valence during CR encounters. Our research question was how do emotional valence and arousal emerge i…

Clinical reasoning during dysphagia assessment and management in acute care: A longitudinal qualitative study.

BACKGROUND: Competent clinical reasoning forms the foundation for effective and efficient clinical swallowing examination (CSE) and consequent dysphagia management decisions. While the nature of initial CSEs has been evaluated, it remains unclear how new information gathered by speech-language therapists (SLTs) throughout a patient's acute-care journey is integrated into their initial clinical reasoning and management processes and used to review and revise initial management recommendations. AIMS: To understand how SLTs' clinical reasoning and decision-making regarding dysphagia assessment a…
