Efficacy of hyperthermic intraperitoneal chemotherapy in colorectal cancer: A phase I and III open label randomized controlled registry-based clinical trial protocol.

Standard treatment for patient with peritoneal metastases from colorectal cancer is cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC). In recent years, the efficacy of oxaliplatin-based HIPEC has been challenged. An intensified HIPEC (oxaliplatin+irinotecan) in combination with early postoperative intraperitoneal chemotherapy (EPIC) has shown increased recurrence-free survival in retrospective studies. The aim of this trial is to develop a new HIPEC/EPIC regimen and evaluate its effect on morbidity, oncological outcome, and quality-of-life (QoL). This study is d

Aging-associated changes in immunological parameters: Implications for COVID-19 immune response in the elderly.

Aging has a profound impact on the immune system, leading to a gradual decline in its function and increased systemic inflammation, collectively known as immunosenescence and inflammaging. These changes make older adults more susceptible to infections, including COVID-19, and contribute to worse clinical outcomes, such as higher morbidity and mortality rates. This review explores immunological changes associated with aging, including impaired innate immune responses, reduced T- and B-cell function, and altered cytokine profiles. A comprehensive literature search identified relevant studies on

Hormonal Contraception and Female Sexuality: Position Statements from the European Society of Sexual Medicine (ESSM).

INTRODUCTION: Hormonal contraception is available worldwide in many different forms. Fear of side effects and health concerns are among the main reasons for not using contraceptives or discontinuing their use. Although the safety and efficacy of contraceptives have been extensively examined, little is known about their impact on female sexual function, and the evidence on the topic is controversial. AIM: To review the available evidence about the effects of hormonal contraceptives on female sexuality in order to provide a position statement and clinical practice recommendations on behalf of t

Supernumerary Abducens Nerves: A Comprehensive Review.

BACKGROUND: Branching and/or replication of the abducens nerve is not an uncommon occurrence. Although numerous variations have been documented, the rarest forms are duplicated or triplicated nerves, where multiple nerve roots originate from the brainstem, travel intracranially, and attach to the lateral rectus as separate entities. METHODS: We conducted a systematic literature search on the topic of supernumerary abducens nerve, using PubMed and Google Scholar. RESULTS: After screening, 16 studies were included: 11 cadaveric studies and 6 case reports. CONCLUSIONS: In this paper, we review

Research in Defense Mechanisms: What Do We Stand?

More than a century is passed since Freud conceived the concept of defense mechanisms. In this article, we retrace the historical evolution of the construct and offer a narrative review of current clinical and empirical literature on its relevance in clinical psychology. Conceptual contributions and empirical findings on the relationship between defenses and personality, as well as between defensive styles and psychopathology (e.g., eating, depressive, and personality disorders) were described. Findings showed that defense mechanisms can help clinicians in understanding the function of patient

The Use of Apitherapy in Oncological Exotic Pet Medicine.

In recent years, the practice of apitherapy has experienced a renaissance as there has been an exponential growth in the scientific literature regarding this topic. Bee Venom Therapy (BVT) is only 1 aspect in the broad field of apitherapy and has shown to have application in oncology. One of the popular applications of BVT is as a treatment in Lyme disease in humans. The article focuses mainly on using BVT and intravenous honey infusion against a variety of cancer cells. Further clinical studies are needed to know about the efficacy of apitherapy in the fight against cancer in exotic animals.

Transient Quadriparesis and Cervical Neuropraxia in Elite Athletes.

Elite athletes are often faced with difficult decisions when faced with a cervical spinal disorder. There are many aspects to consider such as the risk of further injury, short- and long-term effects on an athlete's life both during and after his/her career, and the options for treatment. Although there have been some recent contributions to this topic, the evidence-based literature is generally devoid of high-level clinical studies to help guide the decision-making process. This article reviews the pertinent available data/criteria and offer an algorithm for return-to-play considerations.

Endorsement of Reporting Guidelines and Clinical Trial Registration by Transplantation Journals: A Cross-sectional Review.

BACKGROUND: Reporting guidelines for diverse study designs and the formal registration of clinical trials play a pivotal role in curbing bias and enhancing transparency in research methodologies. However, there has been no prior assessment of surgical transplantation journals regarding their endorsement or omission of reporting guidelines and clinical trial registration. Therefore, this study aims to investigate these journals and categorize the "instructions to authors" concerning the inclusion, recommendation, or stipulation of reporting guidelines and trial registration. METHODS: A total o

Implementation of the American- College of Physicians Guideline for Low Back Pain (IMPACt-LBP): protocol for a healthcare systems embedded multisite pragmatic cluster-randomised trial.

INTRODUCTION: Low back pain (LBP) is a key source of medical costs and disability, impacting over 31 million Americans at any given time and resulting in US$100-US$200 billion per year in total healthcare costs. LBP is one of the leading causes of ambulatory care visits to US physicians; problematically, these visits often result in treatments such as opioids, surgery or advanced imaging that can lead to more harm than benefit. The American College of Physicians (ACP) Guideline for Low Back Pain recommends patients receive non-pharmacological interventions as a first-line treatment. Roadmaps e

Seasonal variations in onset of Wegener's granulomatosis: increased in summer?

OBJECTIVE: Investigators have reported significant seasonal variations of onset of Wegener's granulomatosis (WG), but those data were not confirmed by others. We reexamined the hypothesis of a seasonal pattern of onset of WG. METHODS: We conducted telephone interviews with 59 patients with newly diagnosed WG fulfilling the American College of Rheumatology criteria. Patients were identified having been enrolled from 2001 to 2004 by French hospitals in 2 multicenter therapeutic trials. The interviews investigated precisely how and when their disease had appeared to establish an index date, defi

Phase I/II study testing the combination of AGuIX nanoparticles with radiochemotherapy and concomitant temozolomide in patients with newly diagnosed glioblastoma (NANO-GBM trial protocol).

BACKGROUND: Despite standard treatments including chemoradiotherapy with temozolomide (TMZ) (STUPP protocol), the prognosis of glioblastoma patients remains poor. AGuIX nanoparticles have a high radiosensitizing potential, a selective and long-lasting accumulation in tumors and a rapid renal elimination. Their therapeutic effect has been proven in vivo on several tumor models, including glioblastoma with a potential synergetic effect when combined with TMZ based chemoradiotherapy, and they are currently evaluated in 4 ongoing Phase Ib and II clinical trials in 4 indications (brain metastases,

An evaluation of designs for Phase I/IIa dose-finding studies in Tuberculosis.

Early development of tuberculosis (TB) treatments often separates dose-finding in healthy volunteers (Phase I) and safety and early activity in patients (Phase IIa). The BTZ-043 study (NCT04044001) is a recent study that combined these two objectives in patients in a single study. In this work, we describe and compare three different design options which consider safety and activity endpoints differently in the dose escalation process. In simulations we show that the design that incorporates information about activity together with safety in the dose escalation process allows more precise esti

A phase Ib/II randomized, open-label drug repurposing trial of glutamate signaling inhibitors in combination with chemoradiotherapy in patients with newly diagnosed glioblastoma: the GLUGLIO trial protocol.

BACKGROUND: Glioblastoma is the most common and most aggressive malignant primary brain tumor in adults. Glioblastoma cells synthesize and secrete large quantities of the excitatory neurotransmitter glutamate, driving epilepsy, neuronal death, tumor growth and invasion. Moreover, neuronal networks interconnect with glioblastoma cell networks through glutamatergic neuroglial synapses, activation of which induces oncogenic calcium oscillations that are propagated via gap junctions between tumor cells. The primary objective of this study is to explore the efficacy of brain-penetrating anti-glutam

The phase 1/2 trial of indomethacin in chronic pancreatitis (The PAIR trial): Protocol for a parallel multi-center randomized controlled trial.

BACKGROUND/OBJECTIVES: Current treatments for chronic pancreatitis focus on symptom management and therapeutics targeting disease reversal are lacking. Given the role of the cyclooxygenase-2 (COX-2) enzyme in producing prostaglandin E2 (PGE2), a key component in the inflammatory pathway of chronic pancreatitis, this study evaluates the physiologic effect of oral indomethacin, a COX-2 inhibitor, on PGE2 levels in pancreatic fluid. METHODS: This pilot two-center randomized controlled trial seeks to examine 32 subjects with chronic pancreatitis who have no contraindications to indomethacin. Subj

Current surgical treatment of non-small-cell lung cancer.

When considering surgical treatment for non-small-cell lung cancer (NSCLC), a distinction is made between early-stage disease (stages IA/B and IIA/B), locoregionally advanced disease (stages IIIA/B) and metastatic disease (stage IV). Complete surgical resection of NSCLC can provide good long-term outcome. Surgery is considered the treatment of choice in patients with early-stage NSCLC or patients with T3N1 disease. Surgery for locoregionally advanced disease remains controversial. In specific cases of T4 disease, surgery can provide long-term survival. In patients with stage IIIA-N2 disease, s
