Leaflet modification before transcatheter aortic valve implantation in patients at risk for coronary obstruction: the ShortCut study.

BACKGROUND AND AIMS: This trial sought to assess the safety and efficacy of ShortCut, the first dedicated leaflet modification device, prior to transcatheter aortic valve implantation (TAVI) in patients at risk for coronary artery obstruction. METHODS: This pivotal prospective study enrolled patients with failed bioprosthetic aortic valves scheduled to undergo TAVI and were at risk for coronary artery obstruction. The primary safety endpoint was procedure-related mortality or stroke at discharge or 7 days, and the primary efficacy endpoint was per-patient leaflet splitting success. Independen

Efficacy and safety of the traditional Chinese medicine formulation BL-1 for the treatment of low anterior resection syndrome: study protocol for a double-blind, randomized, placebo-controlled trial.

OBJECTIVE: Low anterior resection syndrome (LARS), a prevalent sequela following rectal cancer surgery, poses a substantial burden on patients' quality of life (QOL), daily functioning, and psychological well-being, and its management remains an unmet clinical need due to the absence of specific therapeutic agents. Although traditional Chinese medicine (TCM) shows promise in supporting postoperative gastrointestinal recovery, high-quality evidence for its application in LARS is lacking. This trial is designed to assess the efficacy and safety of the investigational TCM formulation BL-1 in pati

Aerosolized Beractant in neonatal respiratory distress syndrome: A randomized fixed-dose parallel-arm phase II trial.

PURPOSE: There is increasing research into novel techniques of administering surfactant to preterm infants (PTIs) with respiratory distress syndrome (RDS) receiving non-invasive respiratory support (NIRS). Although aerosolized surfactant (AS) is promising in PTIs receiving NIRS, the optimal surfactant dose and formulation, drug-device combination and patient profile is not known. The objective of this randomized clinical trial was to investigate the feasibility, safety, efficacy and impact of four dosing schedules of AS using two nebulizers in PTIs with RDS stratified by gestational age (GA).

Dexmedetomidine Versus Midazolam for End-of-Life Sedation and Agitation: Protocol for a Randomized Controlled Trial (The DREAMS Trial).

BACKGROUND: Sedation at the end of life is used to relieve distressing symptoms including agitation and delirium. Standard care may include infused benzodiazepines or antipsychotics. These agents often result in deep sedation with loss of interaction with loved ones, which may be distressing. OBJECTIVE: The DREAMS (Dexmedetomidine for the Reduction of End-of-life Agitation and for optiMised Sedation) trial aimed to compare the sedative and antidelirium effects of the alpha-2 agonist dexmedetomidine, a novel palliative care sedative, compared with midazolam, a benzodiazepine when administered

Psoriatic arthritis digital phenotyping and inflammation drivers (PDPID) study: protocol for an international multicentre prospective cohort.

INTRODUCTION: Changes in psoriatic arthritis (PsA) disease activity, particularly flares, are unpredictable and can impact patients' quality of life. Digital biomarkers derived from data collected via smart devices provide an opportunity for unobtrusive continuous monitoring of symptoms reflecting disease activity. Besides, our understanding of the mechanisms and triggers behind flares is limited. The primary objectives of the Psoriatic Arthritis Digital Phenotyping and Inflammation Drivers (PDPID) study are: (1) to develop digital biomarker capable of detecting PsA flare using data collected

Patients with severe gout treated in mixed settings.

INTRODUCTION We aimed to investigate whether patients with a definite gout diagnosis who were treated in mixed real-life settings (various hospital departments or general practice) followed treatment recommendations. METHODS We included all patients in the hospital's uptake area in 2015-2017 who had been diagnosed with gout after microscopy findings of urate crystals. Data regarding comorbidities and indications for urate-lowering therapy (ULT) were collected. The criteria for treatment success were a p-urate level less-than 6 mg/dl (less-than 0.36 mmol/l) or less-than 5 mg/dl (less-than 0.30

99mTc-FAPI SPECT/CT for predicting disease progression in fibrosing interstitial lung diseases: study protocol of a single-centre prospective cohort study.

INTRODUCTION: Fibrosing interstitial lung diseases (F-ILD) affect substantial patient populations with significant mortality and socioeconomic burden. Approximately one-third of patients with F-ILD develop a progressive fibrosing phenotype with poor outcomes. Current monitoring methods, including pulmonary function testing and high-resolution CT (HRCT), often lag behind increases in fibrotic activity, limiting early risk identification and optimal treatment timing. Fibroblast activation protein (FAP) imaging can capture fibrotic activity at the pathological level. This study aims to evaluate t

Outcome monitoring and risk stratification after cardiac procedure in neonates, infants, children and young adults born with congenital heart disease: protocol for a multicentre prospective cohort study (Children OMACp).

INTRODUCTION: Congenital heart disease (CHD) represents the most common birth defect, affecting from 0.4% to 1.2% of children born in developed countries. The survival of these patients has increased significantly, but CHD remains one of the major causes of neonatal and childhood death. The aetiology of CHD is complex, with some evidence of both genetic and environmental causes. However, there is still lack of knowledge regarding modifiable risk factors and molecular and genetic mechanisms underlying the development of CHD. This study aims to develop a prospective cohort of patients undergoing

Transanal irrigation for the management of low anterior resection syndrome (LARS): protocol for a pragmatic, multicentre, crossover randomised controlled trial.

INTRODUCTION: Advances in surgical techniques and neoadjuvant care for rectal cancer have increased sphincter preservation, allowing restoration of bowel continuity through low anterior resection (LAR). However, up to 75% of patients experience low anterior resection syndrome (LARS), which is bowel dysfunction characterised by faecal incontinence, frequency, urgency and clustering of bowel movements, significantly impairing quality of life (QoL). Current management is largely empirical, using lifestyle or pharmacological strategies with variable success. Transanal irrigation (TAI) is a non-sur

Aldosterone and Residual Cardiorenal Risk: Pathophysiology and Emerging Therapeutic Strategies.

Aldosterone, the terminal effector of the renin-angiotensin-aldosterone system, regulates sodium retention, potassium excretion, and blood pressure. Beyond these classical effects, aldosterone-associated and mineralocorticoid receptor (MR)-mediated signaling is implicated in inflammation, oxidative stress, fibrosis, albuminuria, and progressive cardiorenal injury, while its synthesis is regulated by factors beyond angiotensin II. This is particularly relevant in chronic kidney disease (CKD), where persistent renin-angiotensin system (RAS) activation, impaired potassium handling, reduced Klotho

Digital home monitoring for capturing daily fluctuation of symptoms; a longitudinal repeated measures study: Long Covid Multi-disciplinary Consortium to Optimise Treatments and Services across the NHS (a LOCOMOTION study).

INTRODUCTION: A substantial proportion of COVID-19 survivors continue to have symptoms more than 3 months after infection, especially of those who required medical intervention. Lasting symptoms are wide-ranging, and presentation varies between individuals and fluctuates within an individual. Improved understanding of undulation in symptoms and triggers may improve efficacy of healthcare providers and enable individuals to better self-manage their Long Covid. We present a protocol where we aim to develop and examine the feasibility and usability of digital home monitoring for capturing daily f

The SPIZ study: rationale and protocol of a randomized controlled trial to optimize interdisciplinary care of patients following cellular therapy through a digitally supported, cross-regional and cross-sectoral care model.

Allogeneic hematopoietic cell transplantation (alloHCT) and chimeric antigen receptor T (CAR T)-cell therapy represent established therapeutic modalities for selected hematologic malignancies but are associated with substantial morbidity, high healthcare costs, and a considerable risk of severe complications, including death. This necessitates specialized and continuous post-treatment care, which is currently insufficiently standardized and still largely delivered predominantly through in-person and nonintegrated processes. We aimed to develop a comprehensive and innovative post-treatment care
