Brain and cognitive correlates of social status across race/ethnicity and sex/gender.

INTRODUCTION: Subjective social status (SSS) in the United States relates to cognition. Few studies assess SSS in one's community, examine brain correlates, or consider associations across the intersection of sex/gender and race/ethnicity. METHODS: A total of 867 participants from the Washington Heights-Inwood Columbia Aging Project completed MacArthur Scales of SSS for the United States and the community. Cognitive reserve was residual variance in episodic memory after accounting for gray matter and white matter hyperintensity (WMH) volumes. Linear regressions assessed relationships between …

Theoretical and practical challenges for capturing reserve and resilience in aging and Alzheimer's disease: A narrative review.

The concepts of reserve and resilience in aging and Alzheimer's disease (AD) are widely used but their operationalization remains challenging and often lacks conceptual and methodological consistency. In this narrative review, we describe the chronological history of these concepts and then summarize and discuss key theoretical and practical issues, along with proposed solutions and different perspectives, in the context of aging and AD. Theoretical challenges include risk versus protective factors, dynamic brain and cognitive trajectories, overestimation of resilience and inflection points. P

Cognitive improvement after endoscopic third ventriculostomy surgery in long-standing overt ventriculomegaly in adults.

OBJECTIVE: Long-standing overt ventriculomegaly in adults (LOVA) is a chronic form of hydrocephalus that can lead to cognitive deficits. Dat … tention/executive function, language, visuospatial skills, short-term memory, and long-term memory. Cognitive reserve was also estimated through a measure of premorbid IQ to assess its potential influence together with other clinical and demographic variables. RESULTS: Patients with LOVA did not experience general cognitive decline but rather selective long-term m …

Artificial Intelligence, Cognitive Aging, and the Late-Career Surgeon: A Narrative Review.

BackgroundThe surgical workforce in the United States is aging while artificial intelligence (AI) tools are increasingly integrated into clinical practice. These developments raise questions about cognitive aging, professional longevity, and patient safety within the cognitive domains of surgical performance. This review situates these issues within the broader context of surgeon competency, cognitive aging, and the integration of emerging technologies into clinical practice.MethodsThis narrative review synthesizes literature published between 2000 and 2026 addressing cognitive reserve, cognit…

Educational attainment and sex modulate clinical outcomes in genetic frontotemporal dementia.

Individuals with autosomal dominant frontotemporal dementia (FTD) exhibit considerable variability in disease onset and progression. Both modifiable and non-modifiable factors-such as sex, educational attainment or geographic region of residence-may contribute to this heterogeneity, potentially through their influence on cognitive reserve. The aim of the present study was to investigate the role of cognitive reserve modulators within the Genetic Frontotemporal dementia Initiative (GENFI) cohort. To this end, we used functional MRI (i.e. spatial chronnectome measures) and neurodegenerative mark…

Genetic and environmental factors associated with delirium severity in older adults with dementia.

OBJECTIVE: To determine (1) whether delirium severity was associated with Apolipoprotein E (APOE) genotype status and occupational complexity, a measure of cognitive reserve, in individuals with delirium superimposed on dementia; and (2) whether decline in delirium severity was associated with these same factors over a post-acute care (PAC) stay. METHODS: Control group data (n = 142) from a completed randomized clinical trial were used to address the aims of the study. Delirium severity was calculated by combining items from the Confusion Assessment Method and the Montreal Cognitive Assessmen…

An active lifestyle is associated with better neurocognitive functioning in adults living with HIV infection.

Studies of healthy adults show that engagement in physical, social, and mental activities is associated with better cognitive outcomes, suggesting that these activities may increase cognitive reserve. Given the prevalence and real-world impact of HIV-associated neurocognitive disorders (HAND), the present study examined the association between neurocognitive outcomes and self-reported proxies for physical exercise, social activity, and mental activity (employment was used as a proxy for mental activity) among 139 HIV-infected adults (M age = 48.7; 48 % age 50+). Participants completed a neurom…

Premorbid personality characteristics and attachment style moderate the effect of injury severity on occupational outcome in traumatic brain injury: another aspect of reserve.

The concept of "reserve" has been proposed to account for the mismatch between brain pathology and its clinical expression. Prior efforts to characterize this concept focused mostly on brain or cognitive reserve measures. The present study was a preliminary attempt to evaluate premorbid personality and emotional aspects as potential moderators in moderate-to-severe traumatic brain injury. Using structural equation modeling and multiple regression analyses, we found that premorbid personality characteristics provided the most robust moderator of injury severity on occupational outcome. Findings…

The "SuperAgers" construct in clinical practice: neuropsychological assessment of illiterate and educated elderly.

INTRODUCTION: The demographic transition is a global event intensified during the last decades that represents population aging. Thus, the s … e was positive influence of educational level on the cognitive tests' score, which indicates higher cognitive reserve of the elderly with higher educational levels. CONCLUSION: The functionality and independence of the so-called SuperAgers is determined by the cognitive reserve acquired throughout life, mainly developed by the years of formal education.

Cognitive impairment in multiple sclerosis.

Cognitive impairment occurs in 40-65% of multiple sclerosis (MS) patients, typically involving complex attention, information processing speed, (episodic) memory and executive functions. It is seen in the subclinical radiologically isolated syndrome, clinically isolated syndrome, and all phases of clinical MS. In pediatric-onset MS cognition is frequently impaired and worsens relatively rapidly. Cognitive impairment often affects personal life and vocational status. Depression, anxiety and fatigue aggravate symptoms, whereas cognitive reserve partially protects. Cognitive dysfunction correlate…

Dementia prevention and reserve against neurodegenerative disease.

Similar to other complex disorders, the etiology of Alzheimer disease is multifactorial and characterized by an interplay of biological and environmental risk and protective factors. Potentially modifiable risk factors have emerged from epidemiological research and strategies to prevent neurodegeneration and dementia are currently being tested, including multimodal interventions aiming to reduce several risk factors at once. The concept of reserve was developed based on the observation that certain individual characteristics, such as life experiences, lifestyles, and neurobiological parameters

Altered cerebrovascular reactivity velocity in mild cognitive impairment and Alzheimer's disease.

Interindividual variation in neurovascular reserve and its relationship with cognitive performance is not well understood in imaging in neurodegeneration. We assessed the neurovascular reserve in amnestic mild cognitive impairment (aMCI) and Alzheimer's dementia (AD). Twenty-eight healthy controls (HC), 15 aMCI, and 20 AD patients underwent blood oxygen level-dependent imaging for 9 minutes, breathing alternatively air and 7% carbon dioxide mixture. The data were parcellated into 88 anatomic regions, and carbon dioxide regressors accounting for different washin and washout velocities were fitt

Dynamic neurocognitive adaptation: childhood and adult-midlife engagement associated with later-life brain structure and cognition in older adults with and without mild cognitive impairment.

BACKGROUND: Resilience in aging—the capacity to maintain cognition and function despite neuropathology—has been described through cognitive reserve, brain reserve, and maintenance. The dynamic Neurocognitive Adaptation (dNA) framework expands these constructs by defining resilience as a lifelong process of adaptive engagement across cognitive, physical, creative, and social domains that shape neural integrity and cognitive outcomes over time. METHODS: Fifty-eight older adults (39 cognitively normal, 19 with mild cognitive impairment) completed neuropsychological testing, amyloid assessment, an…

Age 20 Cognitive Ability Moderates the Long-Term Influence of Lifestyle Behaviors on Brain Aging in Late Midlife

Abstract We examined whether the longitudinal association between lifestyle behaviors and brain age is moderated by early general cognitive … nger brains, regardless of lifestyle behaviors, suggesting a protective effect of early high GCA or cognitive reserve on later brain health. However, for those with lower cognitive reserve, positive lifestyle behaviors appeared to be protective against brain aging nearly three decades later. Results highlight the important role of cognitive reserve and lifestyle fa …

Occupational cognitive complexity in earlier adulthood is associated with brain structure and cognitive health in midlife: The CARDIA study.

OBJECTIVE: In line with cognitive reserve theory, higher occupational cognitive complexity is associated with reduced cognitive decline in older adulthood. How and when occupational cognitive complexity first exerts protective effects during the life span remains unclear. We investigated associations between occupational cognitive complexity during early to midadulthood and brain structure and cognition in midlife. METHOD: Participants were 669 adults from the Coronary Artery Risk Development in Young Adults study (aged 18-30 years at baseline, 52% female, 38% Black). We calculated scores ref…

Silent neurovestibular frailty: a conceptual model integrating vestibular dysfunction, dual-task mobility and cognitive load as candidate contributors to falls, delirium and functional decline in older adults.

Falls, delirium and functional decline are commonly treated as distinct geriatric outcomes despite sharing vulnerabilities in sensory integration, gait regulation, attention and physiological reserve. We propose silent neurovestibular frailty as a conceptual model describing a latent ageing phenotype in which vestibular dysfunction, dual-task gait deterioration and sensitivity to cognitive load interact before overt disability becomes apparent. This construct is presented as a hypothesis-generating framework rather than a validated diagnostic entity, intended to integrate currently separate li

The protective role of education in white matter lesions and cognitive decline.

Cognitive reserve, often reflected by education, may protect against cognitive decline linked to brain pathology. White matter lesions (WMLs), common in aging, are associated with the progression from healthy cognitive status (HC) to mild cognitive impairment (MCI). This study explores education's role, as a proxy for cognitive reserve, in moderating the relationship between WML burden and the HC to MCI transition. Data from the Alzheimer's Disease Neuroimaging Initiative (ADNI) were analyzed for 153 cognitively healthy adults. Participants were divided into two groups: one (n = 85) remained c…
