CAN TEMPORARY ARTERY CATHETERIZATION EXTEND LIMITS OF ISCHEMIA TIME FOR MACROREPLANTATION?

UNLABELLED: We observe delayed referrals to appropriate Microsurgery Unit and definitive treatment of traumatic limb amputations. Cases with … ation revision (three patients). This study obtained five successful macroreimplantations. The mean cold ischemia time was longer in the group with successful macroreimplantations (7.4 hours) than of the unsuccessful group (9 hours). CONCLUSION: Macroreplantations require immediate referral to microsurgery and, although temporary artery catheterization helps s …

[Mitral valve replacement under beating heart in 137 cases].

To avoid damage of myocardial ischemia, myocardial hypoxia and reperfusion injury, we designed mitral valve replacement in beating heart under extracorporeal circulation with low dose temperature of 31 degrees C to 35 degrees C in 137 cases of rheumatic heart disease, congenital heart disease mitral stenosis and mitral insufficiency, or concurrent aortic insufficiency. The patients were rept in unblocking aorta, unfilling cardiac arrest perfusion, idle pulse and dradycardia of 40-50 times/min, nose temperature of 32 +/- 1 degrees C. Patients with concurrent aortic insufficiency should first un

Role of NFkappaB in an animal model of complex regional pain syndrome-type I (CRPS-I).

UNLABELLED: NFkappaB is involved in several pathogenic mechanisms that are believed to underlie the complex regional pain syndrome (CRPS), including ischemia, inflammation and sensitization. Chronic postischemia pain (CPIP) has been developed as an animal model that mimics the symptoms of CRPS-I. The possible involvement of NFkappaB in CRPS-I was studied using CPIP rats. Under sodium pentobarbital anesthesia, a tourniquet was placed around the rat left ankle joint, producing 3 hours of ischemia, followed by rapid reperfusion (IR injury). NFkappaB was measured in nuclear extracts of muscle and

Robot-assisted partial nephrectomy: systematic review of functional results.

PURPOSE OF REVIEW: Various ischemia type during partial nephrectomy for renal cell cancer (RCC) resulted in different postoperative function … lower decrease in glomerular filtration rates of the operated kidney in comparison to both warm and cold ischemia. This association seems also to play a role in patients with bilateral kidneys harboring complex tumors. SUMMARY: Zero ischemia and selective artery clamping provide the best functional outcomes following robotic partial nephrectomy. This seems to …

Ischemic injury of the liver in a porcine model of cardiac death assessed by in vivo microdialysis.

This study aims to evaluate the ischemic injury of the liver in a porcine model of cardiac death assessed by in vivo microdialysis. A porcine model of cardiac death was established by the suffocation method. Metabolic indicators were monitored using the microdialysis technique during warm ischemia time (WIT) and cold ischemia time (CIT). Pathological changes in ischemic-injured livers were observed by haematoxylin-eosin staining. The predictive values of biochemical parameters regarding the liver donor were evaluated by receiver operating characteristic curve analysis. All statistical analyses…

Institut Georges Lopez-2M as a Novel Lung Preservation Solution Attenuates Ischemia-Reperfusion Injury in a Rat Ex Vivo Lung Perfusion Model.

Institut Georges Lopez-2M (IGL-2M), a novel preservation solution containing polyethylene glycol (PEG 35kD, 5 g/L), preserves mitochondrial integrity and redox balance in liver grafts. This study assesses IGL-2M's effect on lung preservation during prolonged cold ischemia. Rat's heart-lung blocks were procured and subjected to 18 h cold ischemia (4 °C). Lungs were flushed and preserved using one of these preservation solutions: OCS, Perfadex Plus, IGL-2M (n = 6/group). Following ischemia, lungs underwent up to 7 h normothermic ex vivo lung perfusion. Edema was quantified by weight gain. Lung p…

Twenty-Four-Hour Ex Vivo Perfusion with Acellular Solution Enables Successful Replantation of Porcine Forelimbs.

BACKGROUND: A critical barrier to successful limb replantation and allotransplantation is the maximum allowable limb ischemia time of 4 to 6 hours. The current gold standard is to preserve amputated limbs on an ice slurry. Experimental machine perfusion has yielded promising results as an alternative. In particular, hypothermic acellular perfusion has enabled preservation of amputated limbs for up to 12 hours thus far. METHODS: Amputated forelimbs of Yorkshire pigs were preserved on static cold storage at 4°C for 4 hours (static cold storage group) or perfused at 8°C for 24 hours (perfusion g

A flexible procedure for multiple cadaveric organ procurement.

Techniques have been developed which permit removal of the kidneys, liver, heart and other organs from the same donor without jeopardy to any of the individual grafts. The guiding principle is avoidance with all organs of warm ischemia. This is achieved by carefully timed and controlled infusion of cold solutions into anatomic regions, the limits of which are defined by preliminary dissection.

Integrated cardioplegia allows complex valve repairs in all patients.

BACKGROUND: Traditionally, most surgeons have taken adversarial positions with respect to whether cardioplegia should be given warm or cold, antegrade or retrograde, continuous or intermittent. Because each method has weaknesses, myocardial protection is compromised when only one method is employed. It is our contention that an "integrated" approach that combines all of the aforementioned principles will improve myocardial protection, allowing the time needed for complex valve repairs. METHODS: Thirty-four patients (25 undergoing complex mitral valve repairs and 9 undergoing Ross procedures)

Experimental researches on heart and lung preservation.

The purpose of these experimental researches was to study the physiopathology of heart and lung preservation. The current problem of the paucity of lung and heart-lung donors can be solved either by retrieving the organs from cadavers or by increasing the time of preservation. Since the lung is more susceptible to ischemic injury if compared to the heart, we focused our studies on lung preservation techniques. Our results show that lung flushing prior to preservation is very important and the density and the potassium content of the solutions used for this purpose have to be chosen carefully.

Treatment of experimental ischemia/reperfusion injury with S-adenyosylmethionine: evidence that donor pretreatment complements other regimens.

Triple therapy with S-adenosylmethionine (SAM) (given to the donor animal, included in University of Wisconsin solution [UW], and added to the reperfusing medium) has been shown to reduce the sequential cold and warm ischemia/reperfusion injuries characteristic of the liver transplantation procedure. To clarify the actions of SAM during different stages of ischemia/ reperfusion, we have compared its benefit in five dosage regimens, using perfused rat livers after sequential periods of 24 hr cold and 20 min rewarming ischemia. When added only to UW, the presence of SAM throughout ischemia impro

Donor care before pancreatic tissue transplantation.

Publications are reviewed to identify factors related to donor care that may optimize the function of pancreatic tissue (whole or segmental organ or islet cells) after transplantation. Short cold ischemia time, avoidance of hypotension, and treatment of donor hyperglycemia appear to be beneficial, although additional properly designed studies are needed to verify those findings.

Functional recovery of preserved livers following warm ischemia: improvement by machine perfusion preservation.

BACKGROUND: Hypothermic machine perfusion preservation has the potential to relieve the current donor shortage problem by reclaiming and preserving marginal donor organs including those from viable non-heart-beating donors. A number of problems exist with the current machine perfusion technology for preserving livers, and much research is needed to determine the clinical impact of this technology in preserving non-heart-beating donor livers. METHODS: This study was conducted to compare the poststorage function and microcirculation of simple cold stored and machine perfusion preserved livers t

A heat and moisture mask attenuates cardiovascular stress during cold air exposure.

BACKGROUND: Exposure to cold has been shown to cause cardiovascular stress and increased morbidity and mortality. Inhalation of cold, dry air can increase blood pressure and induce myocardial ischemia, particularly in people with preexisting hypertensive cardiovascular disease. Face masks that can warm and humidify inhaled cold air may reduce these cold air pressor effects. METHOD: We compared blood pressure measurements using a heat and moisture exchange mask (HME), a placebo mask (PL), and no mask (NM) in 53 patients with hypertension exposed to a cold chamber environment at -5°C for 1 h. Self-inflicted finger cold injury leading to amputation: Report of a case.

A cold injury can result in devastating outcomes, leading to significant morbidity and loss of distal extremities. Amputations are common after severe frostbite injuries with delayed presentation, often mediated by post-injury arterial thrombosis. Ischemic injuries are managed according to the ischemia time. The most controversial aspect of treating a salvage injury is the time of surgical intervention, which used to be based on the previous management dogma freeze in January, amputate in July. Recently, the paradigm has shifted to early surgical management if the level of viability of the dee
