Idiopathic osteoporosis: a heterogeneous entity.

DEFINITION: Idiopathic osteoporosis refers to the development of osteopenia and fractures with minimal or no trauma in otherwise young, healthy individuals who are not postmenopausal or have other, identifiable secondary causes of osteoporosis. EPIDEMIOLOGY: It is a relatively rare disorder, with an incidence of 0.4 cases per 100,000 person-years. It appears to affect both sexes equally and results primarily in the development of trabecular bone fractures such as vertebral compression fractures and Colles' fractures, although hip fractures are also seen. PATHOPHYSIOLOGY: The disease may be t…

Study of the link between hemotopoietic and skeletal systems in patients attending a referral center for osteoporosis.

PURPOSE: To investigate the link between hematopoietic and skeletal tissues in patients with fragility fractures. METHODS: We retrospectively analyzed the medical records of women older than 40 years who attended the Bone Disease Unit of "Sapienza" University of Rome for their first visit for osteoporosis from January 2020 to June 2022. RESULTS: Fragility fractures were found in 61.8% of the sample. In particular, vertebral fractures in 35.5%, femoral fractures in 6.3%, Colles fractures in 16.5% and non-vertebral non-hip in 42.5%. Fractured patients were significantly older compared to non-f

Osteoporosis and trace elements--an overview.

More than 200 million people are affected by osteoporosis worldwide, as estimated by 2 million annual hip fractures and other debilitating bone fractures (vertebrae compression and Colles' fractures). Osteoporosis is a multi-factorial disease with potential contributions from genetic, endocrine functional, exercise related and nutritional factors. Of particular considerations are calcium (Ca) status, vitamin D, fluoride, magnesium and other trace elements. Several trace elements such as zinc and copper are essential for normal development of the skeleton in humans and animals. Fluoride accumul…

Diabetes and osteoporosis: Action of gastrointestinal hormones on the bone.

A 62-year-old woman consulted for evaluation of treatment for her type 2 diabetes diagnosed four years ago. He had been received treatment with metformin 850mg twice, with no chronic associated complications. She had hypertension and dyslipidemia. She was being treated with candesartan/hydrochlorothiazide 32/12.5mg and atorvastatin 40mg. Her weight was 92kg and height 162cm (BMI, 35.1kg/m(2)). The last analysis showed fasting glucose 168mg/dl and glycated hemoglobin 7.5%, Microalbuminuria was negative. Blood pressure and lipid profile were within the therapeutic range. Two years ago she suffer

Solitary bone cyst of the radius containing Liesegang's rings.

The authors describe the case of a 33-year-old man with a solitary cystic lesion of the left distal radius with central radiodensities that radiographically suggested a chondroid matrix. Curettage of the lesion revealed numerous calcospherites with a lamellar structure. This lamellar pattern is thought to be a result of alternating precipitation and diffusion of supersaturated solutions. This biochemical process has been called the Liesegang's phenomenon, and the structures have been termed Liesegang's rings. The presence of these structures in this benign lesion contributed to atypical roentg

Telemedical experiences at an Antarctic station.

Wintering-over in Antarctica represents a physician's most remote and inaccessible scenario, apart from a space station. Because of the hars … ion. These cases included new-onset coronary artery disease, posterior hip dislocation, complicated Colles' fracture and acute appendicitis. There were also numerous consultations for non-emergency clinical presentations normally managed by specialists. Telemedicine was a crucial link to specialists from the remote and inaccessible environment of Antarctica.

The worldwide problem of osteoporosis: insights afforded by epidemiology.

Osteoporosis is one of the major problems facing women and older people of both sexes. The morbid event in osteoporosis is fracture. However, the definition of osteoporosis should not require the presence of fractures but only a decrease in bone mass that is associated with an unacceptably high risk of fracture. In the USA, approximately 1.5 million fractures annually are attributable to osteoporosis: these include 700,000 vertebral fractures, 250,000 distal forearm (Colles') fractures, 250,000 hip fractures, and 300,000 fractures of other limb sites. The lifetime risk of fractures of the spin

Analysis and validation of a 3D finite element model for human forearm fracture.

Most researchers have performed finite element (FE) analysis of the human forearm fracture by exploring the strength and load transmission o … in the ulna (1.43 MPa). Finally, the FE model has been validated with the onset and location of the Colles' fracture in the literature. This study will provide a great benefit in terms of surgical and medical applications related to forearm fracture that require an extensive knowledge of the behavior of the bones and ligaments under various loading conditions.

The micropathology of the typical carpal tunnel syndrome.

The synovial membrane in the carpal tunnel of twenty-nine patients suffering from the typical clinical picture of carpal tunnel syndrome was examined microscopically over an eighteen month period with a follow-up of six months to two years. Unusual causes such as tendon sheath tumours and post-Colles fractures were excluded. Three micropathological pictures emerged. Oedema was found in twenty-four patients, chronic perivascular inflammation in three and fibrinous synovitis in two. No difference was detected in the clinical presentation in the three pathological appearances. The authors believe

[Unidirectional versus multidirectional palmar locking osteosynthesis of unstable distal radius fractures: comparative analysis with LDR 2.4 mm versus 2.7 mm matrix-Smartlock].

INTRODUCTION: Due to advances in the development of the unidirectional locking plates there is now an increased use of multidirectional palmar locking plates in the treatment of distal radius factures. The purpose of this study was to evaluate a possible improvement of the treatment and results. PATIENTS AND METHODS: This prospective cohort study investigated 40 patients with C1 and C2 Colles' fractures who had been treated with unidirectional and multidirectional locking plates. The average time for the follow-up examinations was 12.3 months (range 12-15 months) after surgery. The intra-oper…

Percutaneous pinning for treating distal radial fractures in adults.

BACKGROUND: Fracture of the distal radius is a common clinical problem. A key method of surgical fixation is percutaneous pinning, involving the insertion of wires through the skin to stabilise the fracture. This is an update of a Cochrane Review published in 2007. OBJECTIVES: To assess the effects (benefits and harms) of percutaneous pinning versus cast immobilisation alone and of different methods and techniques of percutaneous pinning, modalities or duration of immobilisation after pinning, and methods or timing of pin or wire removal for treating fractures of the distal radius in adults.

[The treatment of distal radius fractures using the external fixator].

This is a report on the stabilisation of fractures of the distal end of the radius type C2 and C3 and open fractures by external fixator. Between 1982 and 1989 38 patients with 43 fractures were treated. A follow-up was done 8 to 80 months after operation including 35 patients. 14 patients were polytraumatised. Especially in this group of patients, the advantages of this procedure can be seen. This advantages are a small stress for the patient, involving only a short time of assembling of the external fixator besides a high stability of the system. Our results show, as well as the results of t

Nonbridging external fixation in the treatment of unstable fractures of the distal forearm.

INTRODUCTION: Unstable fractures of the distal forearm often require surgical treatment to restore the normal anatomy and function. We have used a relatively new technique, nonbridging external fixation, in the treatment of these fractures in our hospital during the past few years. Our results are presented here. MATERIALS AND METHODS: Fifty-two patients (41 female, 11 male) with an unstable fracture of the distal forearm were treated using nonbridging external fixation at Oulu University Hospital during 1996-1999. The patients' mean age was 57 years. There were 45 Colles-type fractures, and

Airbag-induced fracture in a patient with osteoporosis.

Since the introduction of automotive airbag technology, patterns of airbag-induced injuries have emerged. Various types of upper-extremity trauma including fractures have been described. Previous reports have focused on the location of the forearm before deployment as the major determinant of fracture. We describe a case of forearm fracture resulting from an airbag deployment in a lupus patient with documented osteoporosis. Bone strength has recently been determined in laboratory testing to be an important factor determining airbag-induced fracture risk. An airbag-induced forearm fracture in a

Entrapment of the sensory branch of the radial nerve (Wartenberg's syndrome): an unusual cause.

Isolated neuropathy of the cutaneous branch of the radial nerve is a rarely recognized pathology. It was described in 1932 by Wartenberg, who suggested the name cheiralgia paraesthetica. The syndrome is described as known the entrapment of the superficial branch of the radial nerve. Many different etiologic factors for chronic nerve entrapment have been described, however our case has an unusual cause. A 52 year old man had pain and paresthesia in the area over the lateral aspect of the wrist, thumb and first web six months after Colles' fracture. The patient underwent bony spike resection aft…

A comparison of the functional results and costs of functional cast and volar-flexion ulnar deviation cast at 2-year follow-up in 105 patients aged 65 and older with dorsally displaced distal radius fracture: A randomized controlled trial.

BACKGROUND AND PURPOSE: Non-operative treatment is the most common treatment option for older patients with distal radius fracture (DRF). Traditionally, wrists have been placed in volar-flexion and ulnar deviation position (VFUDC). In recent years, there has been a trend towards using a functional position cast (FC). However, long-term results for these different casting positions are lacking. PATIENTS AND METHODS: This randomized, controlled, prospective study evaluates the functional results and costs of the 2 casting positions in patients 65 and older with DRF. Primary end point in this st

Correlations between photon absorption properties and failure load of the distal radius in vitro.

The bone mineral content (BMC) and cross-sectional properties of cadaveric radii were assessed by single photon absorptiometry (SPA). A new multiple-angle scanning technique was used in conjunction with SPA to measure the area and moments of inertia of the radial cross section in intact forearms. The radii of the same forearms were then broken in a failure test to simulate a fall on the outstretched arm. Colles' fracture was produced in 16 of 18 radii tested. The BMC divided by the bone width, which is called bone mineral density (BMD) in clinical applications, did not correlate with the load …
