Risk of HIV infection in oral contraceptive pill users: a meta-analysis.

OBJECTIVE: To review the literature and present a summary estimate of the association between HIV-1 acquisition or HIV-1 serostatus and oral contraceptives. DATA SOURCES: MEDLINE database (January 1986-October 1997), AIDSLINE database (January 1980-October 1997), bibliographies of identified articles, and experts in the field of heterosexual HIV transmission. STUDY SELECTION: In all, 591 articles were examined. Twenty-eight studies provided adequate data to calculate a risk estimate for the association of HIV-1 serostatus or HIV-1 seroconversion with oral contraceptives and were selected for

Periodontitis and Systemic Disease: The Impact of Covariate Selection.

Most evidence implicating periodontitis as a causal agent in systemic disease comes from observational research, where confounding is ubiquitous. There are substantial methodological differences among observational studies, of which an important one is the choice of covariates used to adjust for confounding. The present study assesses the impact of covariate selection on the association between periodontitis and systemic disease, using cardiovascular disease and cognitive function as examples. Data were taken from the National Health and Nutrition Examination Survey, where periodontal status w

The course of health-related quality of life from diagnosis to two years follow-up in patients with oropharyngeal cancer: does HPV status matter?

PURPOSE: To investigate the course of health-related quality of life (HRQOL) from diagnosis to 2 years follow-up among patients with oropharyngeal cancer (OPSCC), in relation to human papilloma virus (HPV) status. METHODS: This study included 270 OPSCC patients. Age, sex, tumor sublocation, tumor stage, HPV status, treatment modality, comorbidity, smoking, and alcohol use were retrieved from medical records. HPV status was positive when p16 and HPV DNA tests were both positive. HRQOL was assessed using the EORTC QLQ-C30/QLQ-H&N35 pretreatment and at 6 weeks, 6, 12, 18, and 24 months after tre

Association is not causation: treatment effects cannot be estimated from observational data in heart failure.

AIMS: Treatment 'effects' are often inferred from non-randomized and observational studies. These studies have inherent biases and limitations, which may make therapeutic inferences based on their results unreliable. We compared the conflicting findings of these studies to those of prospective randomized controlled trials (RCTs) in relation to pharmacological treatments for heart failure (HF). METHODS AND RESULTS: We searched Medline and Embase to identify studies of the association between non-randomized drug therapy and all-cause mortality in patients with HF until 31 December 2017. The tre

Effectiveness studies: advantages and disadvantages.

In recent years, so-called "effectiveness studies," also called "real-world studies" or "pragmatic trials," have gained increasing importance in the context of evidence-based medicine. These studies follow less restrictive methodological standards than phase III studies in terms of patient selection, comedication, and other design issues, and their results should therefore be better generalizable than those of phase III trials. Effectiveness studies, like other types of phase IV studies, can therefore contribute to knowledge about medications and supply relevant information in addition to that

Prenatal paracetamol exposure and asthma: further evidence against confounding.

BACKGROUND: Observational studies have reported an association between maternal use of paracetamol in pregnancy and childhood asthma, which was not explained by measured confounding factors. However, it is possible that this relation might be confounded by unmeasured behavioural factors linked to paracetamol usage; if that were the case, effects of similar magnitude of partner's paracetamol use and/or postnatal maternal use would be expected. METHODS: In the Avon Longitudinal Study of Parents and Children (ALSPAC), a population-based birth cohort, we compared the univariate effects of materna

Development and evaluation of high-fidelity simulation case scenarios for pediatric resident education.

OBJECTIVE: Pediatric residency programs need objective methods of trainee assessment. Patient simulation can contribute to objective evaluation of acute care event management skills. We describe the development and validation of 4 simulation case scenarios for pediatric resident evaluation. METHODS: We created 4 pediatric simulation cases: apnea, asthma, supraventricular tachycardia, and sepsis. Each case contains a scenario and an unweighted checklist. Case and checklist development began by reaching expert consensus about case content followed by 92 pilot simulation sessions used for conten

HPLC evaluation of clinical and pharmacological factors reported to cause false-positive carbohydrate-deficient transferrin (CDT) levels.

BACKGROUND: Carbohydrate-deficient transferrin (CDT) is an alcohol biomarker used for detection and follow-up of excessive alcohol consumption. This HPLC study evaluated some clinical conditions and medications previously suggested to interfere with and cause non-alcohol related elevations of CDT. METHODS: Serum samples were collected from patients with end-stage liver disease (n=50), type 2 diabetes mellitus (n=46), cystic fibrosis (n=24), an elevated C-reactive protein level (CRP>100 mg/L; n=15), and from patients taking enzyme inducing or non-enzyme inducing antiepileptic drugs (n=43). The morbidity attributable to Schistosoma japonicum infection in 3 villages in Dongting Lake region, Hunan province, PR China.

In common with other helminth infections, symptoms associated with Schistosoma japonicum infection have a low specificity, and many infections are asymptomatic. The presence of the parasite in a sick individual from an endemic area does not mean that S. japonicum is the aetiological agent. However, estimates of the proportion of all symptomatic episodes in a community attributable to S. japonicum infection can be used to determine its public health impact. Using parasitological and morbidity survey data from 3 villages in China, the fractions of diarrhoea, bloody stool, and abdominal pain epis

Violence by male partners against women during the childbearing year: a contextual analysis.

OBJECTIVES: Many contextual analyses that bridge the micro-level-macro-level gap in identifying risk factors for adverse outcomes have not used methods appropriate for multilevel data. The purpose of this paper is to illustrate the application of appropriate multi-level analytic methods and discuss their implications for public health. METHODS: A previously published individual-level model of physical violence perpetrated by male partners during the childbearing year was reanalyzed to include variables describing the neighborhoods where the women resided. Logistic regression with estimation m

Abdominal obesity and the risk of esophageal and gastric cardia carcinomas.

BACKGROUND: Esophageal adenocarcinoma is rapidly increasing in incidence. Body mass index (BMI) is a risk factor, but its distribution does not reflect the demographic distribution of the cancer (which is highest among White men). Abdominal obesity patterns may explain this discordance, but no studies exist to date. METHODS: Nested case-control study within 206,974 members of the Kaiser Permanente multiphasic health checkup cohort; subjects received detailed questionnaires, a standardized examination including BMI and anthropometric measurements, and follow-up of esophageal and cardia cancers

Deconfounded-Debiased Estimation and Inference for High-Dimensional Mediation Analysis With Pervasive Hidden Confounders.

Mediation analysis is a powerful tool for elucidating the causal mechanisms by which exposures influence outcomes through mediators. However, conventional approaches often yield biased estimates of mediation effects in scenarios involving high-dimensional exposures and high-dimensional mediators, alongside a small number of pervasive hidden confounders that simultaneously affect the exposures, mediators, and outcome or the exposures and outcome. To address these challenges, we propose a deconfounded-debiased method for estimation and inference in high-dimensional mediation analysis based on th

Regression-based Proximal Causal Inference for Right-censored Time-to-event Data.

Unmeasured confounding is a major concern in obtaining credible inferences about causal effects from observational data. Proximal causal inference is an emerging methodological framework to detect and potentially account for confounding bias by carefully leveraging a pair of negative control exposure and outcome variables, also known as treatment and outcome confounding proxies. Although regression-based proximal causal inference is well-developed for binary and continuous outcomes, analogous proximal causal inference regression methods for right-censored time-to-event outcomes are currently l

Confounding Factors in ECG-based Detection of Sleep-disordered Breathing.

OBJECTIVES: To assess the relevance of various potential confounding factors (comorbidities, obesity, body position, ECG lead, respiratory event type and sleep stage) on the detectability of sleep-related breathing disorders from the ECG. METHODS: A set of 140 simultaneous recordings of polysomnograms and 8-channel Holter ECGs taken from 121 patients with suspected sleep related breathing disorders is stratified with respect to the named factors. Minute-by-minute apnea detection performance is assessed using separate receiver operating characteristics curves for each of the subgroups. The det

Pneumococcal vaccination and otitis media in Australian Aboriginal infants: comparison of two birth cohorts before and after introduction of vaccination.

BACKGROUND: Aboriginal children in remote Australia have high rates of complicated middle ear disease associated with Streptococcus pneumoniae and other pathogens. We assessed the effectiveness of pneumococcal vaccination for prevention of otitis media in this setting. METHODS: We compared two birth cohorts, one enrolled before (1996-2001), and the second enrolled after introduction of 7-valent pneumococcal conjugate and booster 23-valent polysaccharide vaccine (2001-2004). Source populations were the same for both cohorts. Detailed examinations including tympanometry, video-recorded pneumati

Can men provide accurate confounder data about their partners for Time-to-Pregnancy studies?

PURPOSE: In studies of time to pregnancy (TTP), women's health-related behaviors may confound other determinants of TTP. In many occupation-based TTP studies, all information is collected through the male partner. There are no data on the validity of the man's report of his partner's fertility-related behavior. METHODS: We studied 202 men and their partners from the most recent pregnancy. Validity of men's reporting on their partner's use of oral contraceptives (OCs) as the last birth control method and her smoking around the beginning of TTP and agreement of coital frequency were assessed.
