INTRODUCTION: Subclinical rhythmic EEG discharges in adults (SREDA) is a very rare benign EEG pattern. The electrophysiological features and atypical variants of SREDA has wide spectrum and they are poorly known. It resembles ictal discharges, and overinterpretation of SREDA may lead to misdiagnosis of epilepsy. Herein, we aimed to report patients with SREDA to identify the frequency and characterized clinical, demographic, electrophysiological features.

A 68-year-old previously independent woman presented multiple times to hospital over the course of 3 months with a history of intermittent weakness, vacant episodes, word finding difficulty and reduced cognition. We describe a rare case of sclerosing osteomyelitis of Garré in a 63-year-old woman with uncontrolled right thigh pain. She had suffered from lower back pain and radiating pain on the right lower leg for a year and 4 months and so had spine surgery 8 months ago. A large proportion of patients presenting on the acute medical take are frail and elderly and a significant proportion of these will have symptoms such as confusion, reduced mobility and electrolyte disturbances. These symptoms are typically attributed either to the iatrogenic effects of prescribed medications, disturbances in fluid balance and possible infective causes. We describe the case of a gentleman who presented with delirium, reduced mobility and hyponatraemia who was subsequently found to have pituitary failure secondary to pituitary apoplexy.

A 67 year old lady came to this clinic with multiple translucent, asymptomatic papulocystic lesions on the face which got worse in summer. This case report presents a full-term pregnancy in a retroflexed uterus. Although the fetus presented at first as a cephalic presentation, the longitudinal lie appeared to be breech. Further diagnostic procedures revealed a uterus subseptus. No explanation could be found for this persisting retroflexion during pregnancy.

Over a 24-year interval I have seen 22 cases of rupture of the medial head of the gastrocnemius muscle where it inserts into the soleus aponeurosis. The rupture may result in a swelling that is easily confused with thrombophlebitis and result in unnecessary hospitalization and anticoagulant therapy with its attendant expense and possible complications. The process is self-limiting and heals without sequelae in three to six weeks.

A 3-year-old child is presented with a benign anterior mediastinal teratoma that was primarily located inferior to the left lung and confused with pleural fluid. The lesion was not apparent at 4 months ofage. Computerized axial tomography of the specimen clearly established the diagnosis.
