The impact of school-based mitigation measures on the transmission of respiratory pathogens: the case of SARS-CoV-2.

BACKGROUND: School-based mitigation measures can curb transmission of severe respiratory pathogens but may also impose high societal costs. Understanding their impact at the population level is key for pandemic preparedness evaluations. METHODS: We developed an individual-based model of SARS-CoV-2 transmission in households, schools, and the general community, calibrated to epidemiological estimates from complete contact tracing data collected in the province of Reggio Emilia, Italy, during March-April 2021 (a period characterized by the dominance of the Alpha variant and strict community res…

An epidemiological survey of chlamydial and gonococcal infections in a Canadian arctic community.

BACKGROUND: Sexually transmitted infections are leading causes of morbidity for Canadian Aboriginal women. To date, very few initiatives have been successful in screening, treating, and limiting these infections among these populations. OBJECTIVES: To evaluate the efficacy of universal screening, treatment and contact tracing as a means of capturing a more accurate count of chlamydia and gonorrhea prevalence and limiting transmission among Inuit communities. METHODS: 181 participants were screened for chlamydia and gonorrhea and interviewed in a cross-sectional survey (Aug-Sept/03). Informat…

Sexually transmissible infection management practices among primary care physicians in Singapore.

BACKGROUND: Primary care physicians manage a significant number of sexually transmissible infections (STI); however, there has not been a survey to assess the standard of medical care, completeness of notifications, provision of counselling and contact tracing by primary care physicians in Singapore. METHODS: An anonymous postal survey was conducted in which 1557 questionnaires were mailed out to general practitioners (GP), and government primary care and emergency department doctors. RESULTS: In all, 736 questionnaires (47.3%) were returned, and the majority of respondents were graduates fr…

Can the male-to-female ratio of gonorrhoea in one sexually transmissible infection treatment facility be solely used to judge its efficiency?

BACKGROUND: This facility has for a long time audited its efficacy in contact tracing (case finding) and found results comparable with national guidelines. In addition, we consistently measure control of the disease using three indices. A departure from the norm in one of these (the male-to-female ratio) prompted us to explore whether local case finding, and therefore control, was lacking resulting in the identification of a statistical anomaly. We have learnt a lesson, which may be of use to others who critically evaluate their work. METHODS: Review of statutory clinic quarterly returns and …

The diversity of the opa gene in gonococcal isolates from men who have sex with men.

OBJECTIVES: To use a molecular typing method (opa-typing) to characterise gonococcal isolates obtained from men who have sex with men (MSM) attending a genitourinary medicine (GUM) clinic in Edinburgh during 2001. To compare the results of opa-typing with A/S (auxotype/serovar) phenotyping, and with epidemiological information obtained by contact tracing. METHODS: Isolates were opa-typed by a PCR-RFLP method where the restriction fragments resulting from digestion with three separate restriction enzymes were resolved by polyacrylamide gel electrophoresis. Isolates were considered to have the …

A comparison of contact patterns derived from the population structure in agent-based models and empirical contact survey data.

Agent-based models (ABMs) are powerful tools for simulating disease spread, relying on individual-level interaction rules from which emergent dynamics arise. An important component in ABMs is contact behaviour. To reduce computational complexity, contact behaviour in ABMs is often assumed as random mixing within structurally defined settings (as, e.g., workplaces). with setting composition typically based on empirical data such as census information. However, the validity of this approach to represent contacts remains unclear. To address this gap, we compare the contact structure derived throu

Declining incidence rate of tuberculosis among close contacts in five years post-exposure: a systematic review and meta-analysis.

BACKGROUND: Individuals in close contact with active pulmonary tuberculosis (TB) patients showed a high risk of recent infection and, once infected, higher risk of developing active TB in the following years post-exposure. But the peak time of active disease onset is unclear. This study aims to estimate post exposure TB incidence risk among close contacts to provide reference for clinical and public health strategies. METHODS: We searched PubMed, Web of Science, and EMBASE for articles published until December 1, 2022. The incidence rates were quantitatively summarized by means of meta-analys

Tuberculosis contact investigation following the stone-in-the-pond principle in the Netherlands - Did adjusted guidelines improve efficiency?

BackgroundIn low tuberculosis (TB) incidence countries, contact investigation (CI) requires not missing contacts with TB infection or disease without unnecessarily evaluating non-infected contacts.AimWe assessed whether updated guidelines for the stone-in-the-pond principle and their promotion improved CI practices.MethodsThis retrospective study used surveillance data to compare CI outcomes before (2011-2013) and after (2014-2016) the guideline update and promotion. Using negative binomial regression and logistic regression models, we compared the number of contacts invited for CI per index p

Yield of contact investigations in households of patients with drug-resistant tuberculosis: systematic review and meta-analysis.

Contact investigations among individuals living with drug-susceptible tuberculosis patients (source cases) have shown a high yield of tuberculosis disease and latent tuberculosis, but the yield of such investigations in households of drug-resistant tuberculosis source cases is unknown. In this systematic review and meta-analysis, we found 25 studies that evaluated a median of 111 (interquartile range, 21-302) household contacts of drug-resistant tuberculosis source cases. The pooled yield was 7.8% (95% CI, 5.6%-10.0%) for active tuberculosis and 47.2% (95% CI, 30.0%-61.4%) for latent tuberculo
