High-flow nasal oxygenation versus standard oxygenation for gastrointestinal endoscopy with sedation. The prospective multicentre randomised controlled ODEPHI study protocol.

INTRODUCTION: Hypoxaemia is a major complication during gastrointestinal endoscopy (GIE) procedures (upper/lower) when performed under deep sedation in the procedure room. Standard oxygen therapy (SOT) is used to prevent hypoxaemia. Data suggest that risk factors for hypoxaemia under deep sedation during GIE are obstructive sleep apnoea syndrome, a body mass index above 30 kg/m², high blood pressure, diabetes, heart disease, age over 60 years old, high American Society of Anesthesiologists physical status class and the association of upper and lower GIE. High-flow nasal oxygenation (HFNO) may …

Propofol and fentanyl for laser photocoagulation in non-intubated preterm infants with retinopathy of prematurity.

OBJECTIVES: Retinopathy of prematurity (ROP) is a vasoproliferative retinal disorder of preterm infants, characterized by disrupted retinal vascular development and abnormal neovascularization that result from preterm birth and supplemental oxygen exposure. It is a leading cause of preventable childhood blindness. Laser photocoagulation is a common treatment, but there is no evidence-based consensus on the most appropriate sedation method for this procedure. This study aimed to determine if deep sedation with propofol and fentanyl during laser photocoagulation for ROP is safe and effective whi…

Manual versus target-controlled infusion of balanced propofol during diagnostic colonoscopy – A prospective randomized controlled trial.

INTRODUCTION: There is an increasing interest in balanced propofol sedation (BPS) for colonoscopy in outpatient settings. Propofol is a potent anesthetic agent for this purpose and has a narrow therapeutic range, which increases a risk of cardiovascular and respiratory complications in case of improper administration. OBJECTIVE: The aim of this study was to compare patients’ safety and comfort of endoscopists in two methods of BPS targeting deep sedation – propofol target-controlled infusion (TCI) and manual intravenous titration technique (MT) – during colonoscopy. METHODS: This prospective…

Effects of anesthetic agents on the evaluation of systolic and diastolic function in mice.

AIM: This study aimed to optimize non-invasive echocardiographic evaluation of myocardial function in a mouse model of diastolic heart failure, emphasizing the methodological challenges in assessing diastolic and left atrial (LA) function. Recognizing that clinical human studies frequently assess cardiac performance in non-sedated subjects, this investigation compared systolic and diastolic functional outcomes in mice subjected to heart failure with preserved ejection fraction (HFpEF) using two anesthetic agents: Avertin (mild sedation) and isoflurane (deep sedation). Additionally, we present …

Comparison between monitored anesthesia care and general anesthesia in patients undergoing device closure of atrial septal defect.

BACKGROUND: Percutaneous atrial septal defect (ASD) device closure is usually performed under general anesthesia (GA) because it can ensure protection of the airway from the effects of insertion of the transesophageal echocardiogram probe insertion. However, recent studies have suggested that this procedure can be performed safely under deep sedation also. Thus, we compared the efficacy of using monitored anesthesia care (MAC) with deep sedation versus GA to perform this procedure and the post-procedural outcomes and incidence of complications associated with these two methods of anesthesia. …

Effects of Anesthetic Depth Using the Bispectral Index During Sleep Endoscopy With PAP Titration.

OBJECTIVES: Airway collapsibility varies with the level of anesthetic depth during drug-induced sleep endoscopy (DISE). Bispectral index (BIS) monitoring uses EEG signals to quantify anesthetic depth. We conducted a prospective study to examine changes in VOTE scores and pharyngeal opening pressure (PhOP) during DISE with positive airway pressure (DISE-PAP), at light and deep sedation. METHODS: Anesthetic depth was monitored using BIS during DISE-PAP for 17 patients. We classified BIS ≥ 55 as light sedation and < 55 as deep sedation. The DISE exam was performed and subsequently PAP applied to…

Successful application of early tracheostomy in an intubated patient who suffered from irritative stimuli by an oral tracheal tube.

We experienced a case in which simultaneous weaning from sedation and mechanical ventilation were difficult because of instability of tracheal tube fixation that was caused by size mismatch between the trachea and the tube and by severe tracheal deviation. Irritative stimuli caused by the oral tracheal tube prevented conversion from deep sedation to light or no sedation. In this case, very early tracheostomy, which provided better tube fixation and successfully reduced the irritative stimuli to the trachea, was effective to help achieve discontinuation of sedation and facilitated successful we…
