Intraoperative Electroencephalogram Suppression Predicts Postoperative Delirium.

BACKGROUND: Postoperative delirium is a common complication associated with increased morbidity and mortality, longer hospital stays, and greater health care expenditures. Intraoperative electroencephalogram (EEG) slowing has been associated previously with postoperative delirium, but the relationship between intraoperative EEG suppression and postoperative delirium has not been investigated. METHODS: In this observational cohort study, 727 adult patients who received general anesthesia with planned intensive care unit admission were included. Duration of intraoperative EEG suppression was re…

Neuroinflammation in sepsis: sepsis associated delirium.

Sepsis-associated delirium (SAD) is a clinical manifestation of the involvement of the central nervous system (CNS) during sepsis. The purpose of this review is to provide a concise overview of SAD including the epidemiology and current diagnostic criteria for SAD. We present in detail the pathophysiology with regards to blood-brain-barrier breakdown, cytokine activation and neurotransmitter deregulation. Treatment and prognosis for SAD are also briefly discussed. SAD is the most common form of delirium acquired in the ICU (Intensive Care Unit), and is described in about 50% of septic patients…

Delirium in intensive care unit patients.

Delirium is defined as a disturbance of consciousness with cognitive changes or perceptual disturbances, which has developed over a short period of time, and is caused by a medical condition or a postsurgical state. Although historically dismissed as an inconvenient and transient problem, recent studies have reported that delirium is associated with more complications, increased length of hospital stay, and higher mortality. Although delirium is a prevalent condition after cardiothoracic surgery and in the intensive care unit (ICU), the condition appears to be largely underdiagnosed. Several d…

Delirium in hospitalised older persons: review.

Delirium, a mental disorder that becomes established over a few hours or days, is characterised by fluctuating attention and cognitive states. This article reviews the disorder, which has all the features of an important geriatric syndrome: it appears mainly in persons older than 65 years of age, is closely linked with very prevalent diseases and complications arising in the elderly, and is the mode of presentation of many other diseases in this age group. We discuss diagnostic, clinical preventive and therapeutic aspects and analyse the most common risk and precipitating factors in our hospit…

Delirium following abrupt discontinuation of fluoxetine.

Sudden discontinuation of serotonin reuptake inhibitors (SRI) can lead to a number of psychological (e.g., nervousness, anxiety, crying spel … n, likely as a result of its longer half-life. We report an unusual case of a patient who developed delirium after abrupt discontinuation of fluoxetine.

[Disulfiram-induced delirium: a case report].

There are occasional reports from Europe and North America of disulfiram-linked psychosis. But there are rare reports from Japan, because disulfiram and cyanamide are used as antidipsotropic agent in clinical practice and the former is not administered so frequently as the latter in Japan. The authors described delirium in a patient who was a 50-year-old male alcoholic. He gradually showed memory impairment, disorientation, impaired judgment and delusion. One month prior to his delirium, disulfiram 200 mg/day was administered. The clinical symptoms of delirium and slowing of the EEG in the dif…

Postoperative delirium: the tryptophan dyregulation model.

A model previously presented by Uchida in this journal [Med. Hypotheses 53 (1997) 103] described a mechanism for postoperative delirium. It described an increased level of melatonin resulting in a central "serotonin shortage". This construct adequately explained only the hypoactive type of delirium. Recently it has been shown that a reduction in urinary metabolites of melatonin is associated with hyperactive delirium, whereas urinary metabolites were increased in the hypoactive variant. These findings suggest that this initial paradigm requires modification. We propose that both the agitation …

Thiamine absorption in alcoholic delirium patients.

OBJECTIVE: The role of a deficiency of vitamin B1 in the development of alcoholic complaints is confined to the case of the Wernicke-Korsakov syndrome. Findings concerning a deficiency of thiamine in alcoholics in comparison with normal persons are contradictory and there are no differentiated tests in the case of delirium tremens. In this study the vitamin B1 absorption in patients with delirium tremens was of interest in connection with the presence or absence of hallucinations and autonomic symptoms. METHOD: Male patients (N = 70) with delirium tremens were compared with a group of 13 cont…

Delirium: how its concept has developed.

The development of the concept of delirium spans nearly 2,500 years. Its core clinical features were recognized at least as early as the 16th century, while its management reflected a humane approach from the beginning of the modern era. In the 19th century delirium became linked with the concepts of disordered consciousness and confusion, but these two terms were also used in regard to certain functional mental disorders. The most important contribution in this century was the work of Engel, Romano, and associates, who postulated that the syndrome was due to reduction in brain metabolic rate,…

Virtual reality for delirium: immersive training courses promote empathy and expertise in delirium management among registered nurses.

BACKGROUND: The use of virtual reality (VR) for educational purposes offers more complex training experiences than conventional methods. It remains unclear whether a training using a VR delirium application (VR-A), compared to traditional presentation-based training (TPT), has an impact on empathy and delirium-related knowledge among nursing professionals (NP). METHODS: Prospective, monocentric quasi-experimental crossover study involving clinically active nursing professionals. Two 45-minute training sessions were delivered in varying sequences (a) or b) first): a) VR-A offering a critical c…

Delirium education and post-anaesthetics care unit nurses' knowledge on recognising and managing delirium in older patients.

BACKGROUND: Postoperative delirium (POD) is a major complication following a surgical procedure. There is evidence that improving knowledge about POD could enhance POD care and patient outcomes. AIM: The study aimed to evaluate whether the amount of delirium education among registered nurses working in post-anaesthetics care units (PACU) impacts on their self-reported confidence and competence in recognising and managing delirium as well as prior knowledge on factors that influence the risk of delirium onset for older people. METHOD: The current study utilised an online survey on delirium ca…

"SQiD, the Single Question in Delirium; can a single question help clinicians to detect delirium in hospitalised cancer patients?" running heading Single Question in Delirium" (Bcan-D-20-01665).

AIM: A serious syndrome for cancer in-patients, delirium risk increases with age and medical acuity. Screening tools exist but detection is frequently delayed or missed. We test the 'Single Question in Delirium' (SQiD), in comparison to psychiatrist clinical interview. METHODS: Inpatients in two comprehensive cancer centres were prospectively screened. Clinical staff asked informants to respond to the SQiD: "Do you feel that [patient's name] has been more confused lately?". The primary endpoint was negative predictive value (NPV) of the SQiD versus psychiatrist diagnosis (Diagnostic and Stati…

Improving Delirium Screening and Detection in the Emergency Department (ED): The Implementation and Evaluation of the ED Delirium Detection Program.

BACKGROUND: Delirium is missed in over 75% of older adults presenting to the Emergency Department (ED). The study aimed to determine the efficacy of the ED Delirium Detection Program (ED-DDP) to improve delirium detection while evaluating implementation outcomes. METHODS: The ED-DDP consisted of a train-the-trainer model where delirium champions (DCs) trained ED nurses to perform delirium screening using an electronic health record (EHR)-embedded Brief Confusion Assessment Method (bCAM). The ED-DDP was implemented across 3 diverse EDs using a stepped-wedge cluster randomized trial, consisting…

Delirium in the long-term care: a challenge for clinicians and researchers-the negative outcomes related to delirium: a scoping review.

BACKGROUND: Most previous studies on the outcomes of delirium have focused on acute care hospital settings. However, the number of residents in long-term care (LTC) facilities is increasing, and these individuals are at risk of experiencing adverse outcomes related to delirium. Recent studies suggest that delirium in LTC settings is associated with serious consequences-such as increased mortality, cognitive decline, and reduced physical function-similar to those reported in acute care. However, high-quality studies specific to this population remain limited. PURPOSE: This scoping review aimed…

A pandemic of delirium: an updated systematic review and meta-analysis of occurrence of delirium in older adults with COVID-19.

PURPOSE: Delirium has been recognized as an atypical presenting feature of COVID-19 in older adults and is independently associated with mortality. We aimed to perform an updated systematic review of the literature and proportional meta-analysis to assess prevalence and incidence of delirium in older adults with COVID-19, addressing differences according to sex, frailty status, and settings. METHODS: We searched databases for English-language articles on prevalence and incidence of delirium in older adults with COVID-19, published between March 2020 and January 2023. RESULTS: Of the 1171 art…
