Antibiotics for the prophylaxis of bacterial endocarditis in dentistry.

BACKGROUND: Infective endocarditis is a severe infection arising in the lining of the heart with a high mortality rate. Many dental procedures cause bacteraemia and it was believed that this may lead to bacterial endocarditis (BE) in a few people. Guidelines in many countries have recommended that prior to invasive dental procedures antibiotics are administered to people at high risk of endocarditis. However, recent guidance by the National Institute for Health and Clinical Excellence (NICE) in England and Wales has recommended that antibiotics are not required. OBJECTIVES: To determine whethe

[Significance of infection control in dentistry: a review].

Dental care is a field of high priority regarding the risk of infections. Since many carriers are not aware of their infection, it may happen that the dentist meets a patient, in whom an earlier infection can be proven by serology, but the patient is not aware of it and the clinical signs and symptoms are missing, as well. For this reason, the dentist has to consider every patient potentially infected. On the other hand, health-care workers are not only susceptible persons to infections but they can also be sources of infections. In order to prevent the nosocomial infections the dentist has to

Dental education and dentistry system in Iran.

Before 1979, there were only 5 undergraduate dental schools in Iran with a total admission of 200 students per year, and only 2,000 dentists and about 50 specialists practicing in the country. Currently, there are 18 dental schools with a total admission of 750 undergraduate students, 5 postgraduate programs in 10 disciplines with a total of 100 students, more than 11,000 dentists (1 dentist per 5,500 population) and nearly 1,000 specialists in the country. Two new schools have recently begun offering specialty training courses in 2 disciplines. The length of the dentistry curriculum is 6 year…

Regional anesthesia for dentistry and oral surgery.

Pain is a perception that results from activation of a specific set of receptors by noxious stimuli. Processing by the central nervous system results in the stimulus to be perceived as pain. Thermal receptors respond to temperature changes, mechanoreceptors respond to pressure, tension, stretch, and chemoreceptors respond to substances released during the inflammatory process such as prostaglandins, histamine, serotonin, and bradykinin. The current standard of veterinary medical practice is adequate pain management for all patients. Regional anesthesia is one component of overall pain manageme

Mechanisms leading to musculoskeletal disorders in dentistry.

BACKGROUND: The authors reviewed the implications of prolonged, seated working postures on dental operator health and the potential development of musculoskeletal disorders, or MSDs. TYPES OF STUDIES REVIEWED: The authors reviewed studies to assess the mechanisms associated with the development of musculoskeletal pain and MSDs among dental operators. Some studies investigated work duration, operator positioning and the physiological effects of various static sitting postures. Others explored the relationships between prolonged muscle contraction and muscle imbalances, as related to the develo

Neurological complications of local anaesthetics in dentistry.

Neurological complications following the administration of a local anaesthetic can be alarming. By reading reports of such incidents, dentists who find themselves in similar situations will be able to reassure their patients and act accordingly. The authors have reviewed the literature for those neurological complications that have been reported and offer an explanation of their aetiology. Examples of complications covered are facial nerve palsy, transient amaurosis, post-injection paraesthesia, Horner's syndrome, transient paralysis of combined cranial nerves III, IV and VI, sudden unilateral

Management of periodontal tissues for restorative dentistry.

Proper management of periodontal tissues is required to achieve predictable long-term success with restorative dental procedures. Forced eruption as well as several surgical techniques may be used to achieve and maintain adequate biologic width during restorative and esthetic dental procedures. The technique that will yield optimal results depends on the relationship between the restoration's margins and the surrounding periodontium. A classification system that describes these interrelationships and provides treatment recommendations is included.

The role of biomaterials as occupational hazards in dentistry.

Many of the biomaterials and auxilliary products used in dentistry are chemically and biologically reactive and may be of concern in occupational safety programmes. Observations from 1936 to 1975 indicated that most occupational problems were related to skin contact with procaine, soaps, eugenol, iodine, formalin, phenol, and other disinfectants. Methyl methacrylate monomer was identified as an irritant and allergen in the later part of this period. Investigations from 1975 to 1985 indicated that disinfectants and detergents were still important causes of dermatoses, whereas reactions to proca…

Test battery for metal allergy in dentistry.

Some alloys used in restorative dentistry may evoke an allergic contact stomatitis in certain persons. In order to protect patients from materials with undesired reactions, and considering corrosion characteristics of different alloys used, it is useful to devise an adequate patch test battery to include the most relevant metals. Dental alloys are composed of a combination of various metals. 12 different ions of frequent occurrence (Au3+, Pd2+, Zn2+, Mo6+, Sn2+, Ga3+, In3+, Co2+, Cr3+(6+), Ni2+, Fe2+(3+) and Si4+) were epicutaneously tested as the aqueous solution of the respective salt. The c…

[Ambulatory therapy with fosfomycin in dentistry and stomatology].

1-2 g/day or 3-4 g/day phosphomycin were administered per os to children and adults respectively for an average of 5.2 days in the outpatient treatment of 28 males and 18 females with parodontitis, gingivitis and dysodontiasis of bacterial origin. All patients presented swelling of the soft parts and 34 (74%) had hyperpyrexia. A clinical cure was obtained after 9 days in all cases. Fever lasted an average of 2.4 days and disappeared after 6 days. Swelling lasted an average of 3.6 days and disappeared after 8 days. Tolerance was excellent. Two patients (4.3%) displayed diarrhoea and nausea and

Periodontology and its relevance to clinical dentistry.

The important role played by restorations and prostheses in the accumulation of plaque necessitates careful design of all contours to enable proper removal of plaque by the patient. Early diagnosis of impacted teeth is essential as well as careful design of gingival flaps preserving the maximum of attached gingiva where their removal or exposure is planned. Difficulties arising from closed bite and orthordontic treatment, the need for careful consideration to the importance of endodontic therapy, and removable partial dentures in relation to the health of periodontium, are discussed.
