Transmetatarsal amputations in patients with diabetes mellitus: A contemporary analysis from an academic tertiary referral centre in a developing community.

Transmetatarsal amputation (TMA) involves the surgical removal of the distal portion of metatarsals in the foot. It aims to maintain weight-bearing and independent ambulation while eliminating the risk of spreading soft tissue infection or gangrene. This study aimed to explore the risk factors and surgical outcomes of TMA in patients with diabetes at an academic tertiary referral center in Jordan. Medical records of all patients with diabetes mellitus who underwent TMA at King Abdullah University Hospital, Jordan, between January 2017 and January 2019 were retrieved. Patient characteristics al…

Comparison of inhalational methoxyflurane, intranasal fentanyl, and intravenous morphine for treatment of prehospital acute pain in Norway (PreMeFen): a randomised, non-inferiority, three-arm, phase 3 trial.

BACKGROUND: Adequate pain control is essential; however, acute pain is often undertreated in prehospital care. This study aimed to evaluate three analgesic regimens for treating acute pain in an ambulance setting. METHODS: PreMeFen is a randomised, open-label, non-inferiority, three-arm, phase 3 trial conducted in the working areas of the ground ambulance service of the Innlandet Hospital Trust, Norway. Patients aged 18-69 years and 70 years and older with traumatic or medical acute pain scoring 4 or higher on the Numeric Rating Scale (NRS) were randomly assigned (1:1:1) to receive in titrati

Importance of physiological age in determining indications for adult spinal deformity surgery in patients over 75 years of age: a propensity score matching analysis.

PURPOSE: Physiologically, people age at different rates, which leads to a discrepancy between physiological and chronological age. Physiological age should be a priority when considering the indications for adult spinal deformity (ASD) surgery. The primary objective of this study was to determine the characteristics of the postoperative course, surgical outcomes, and complication rates to extend the healthy life expectancy of older ASD patients (≥ 75 years). The secondary objective was to clarify the importance of physiological age in the surgical treatment of older ASD patients, considering f

Influences of early thyroid hormone manipulations: delays in pup motor and exploratory behavior are evident in adult operant performance.

The effects of thyroid hormone depletion and enhancement on litter size, survival, body mass, ambulation, quadrant crossing, home orientation, day of eye opening, and free serum T3 and T4 levels were examined in Study 1. In Study 2, the effects of the timing of prenatal insult and the level of thyroid hormone depletion on litter size, survival, body mass, and free serum T3 and T4 levels were examined. Upon the completion of Study 1, randomly selected pups were maintained on ad-libitum water and food for 2 years, and performance was evaluated on fixed and variable ratio schedules, fixed and var

When care and harm coexist: Challenges in forensic classification of prolonged fatal starvation.

Deaths involving prolonged starvation and immobility are commonly interpreted through frameworks of neglect, restraint, abuse or medical failure. This paper re-examines a published forensic case involving a 47-year-old woman who died following prolonged inanition, immobility and repeated non-lethal trauma. Autopsy findings included extreme cachexia, fixed joint ankylosis, circumferential lower limb ulcers, bilateral rib fractures in different stages of healing and marked organ atrophy in the absence of disease or toxicological explanation. The reported history of recent independent ambulation …

Impact of Premorbid Function and Comorbidities in Geriatric Traumatic Brain Injury Patients: A Systematic Review.

Population aging is increasing geriatric traumatic brain injury (TBI), with outcomes depending on premorbid health beyond chronological age. This systematic review examined how premorbid functional status, frailty, and comorbidities influence mortality, functional recovery, and other outcomes in adults aged ≥60 years with TBI. Following the PRISMA 2020 statement, PubMed, Scopus, Web of Science, and Google Scholar were searched (2016-2026). Twenty observational studies (n = 128,793) were included. Narrative synthesis was performed; risk of bias was assessed with Joanna Briggs Institute tools. F

[Contract of the German Health Insurance AOK Hessen Concerning Prolonged Weaning-Methods and First Experiences].

INTRODUCTION: In recent years, the number of patients requiring mechanical ventilation has increased steadily. In some cases, weaning is not successful. These patients depend on home mechanical ventilation and intensive outpatient care. Surprisingly, most of these patients were never treated in a weaning center. Thus, weaning might still be possible in at least some of them. Health insurance companies have recognized this problem. METHODS: AOK Hessen, a major health insurance company in the German federal state of Hesse, identified some starting points for improvement after having surveyed th

Surgical treatment of congenital patellar dislocation in skeletally mature patients: surgical technique and case series.

BACKGROUND: Congenital patellar dislocation is a rarely encountered condition and is readily treated in childhood to prevent lasting disability, knee pain, decreased range of motion and ambulation problems. This condition is very rarely seen in skeletally mature patients, and the treatment of the condition represents a challenge to the orthopedic surgeon. SURGICAL TECHNIQUE AND METHODS: Patients were treated with soft tissue reconstruction and tibial tubercle transfer with or without a prior medial close-wedge distal femoral osteotomy, depending on the degree of valgus deformity. We, then, se

Educational career and predictors of type of education in young adults with spina bifida.

Children with spina bifida (SB) often require special education. To date, little information is available about the educational career of these children. This study focuses on educational career and predictors of attending special education of young adults with SB, using a cross-sectional study including 178 young Dutch adults with SB aged from 16-25. The main outcome was attending regular versus special education. For searching predictive power we selected age, gender, type of SB, level of lesion, hydrocephalus (HC), number of surgical interventions, ambulation, continence and cognitive funct

Boundary work in the implementation of an assistive exercise product to facilitate physical activity for children with cerebral palsy.

Purpose: Children gain increased health and well-being by participating in physical activity (PA) and exercise. Globally, only 19% of children meet the World Health Organisation recommendations on PA and the rates are even lower among children with disabilities. Children with disabilities are often dependent on assistive exercise products (AEP) to be able to perform PA and exercise. The aim was to identify facilitating and hindering factors for PA of children with cerebral palsy, and additionally to identify conditions for implementation of an AEP for PA in this group. Materials and methods: I

Acute Glaucoma Attack Following Microvascular Decompression Surgery for Trigeminal Neuralgia.

Microvascular decompression (MVD) is widely accepted as an effective surgical method to treat trigeminal neuralgia (TN), but the risks of morbidity and mortality must be considered. We experienced a case of acute angle-closure glaucoma attack following MVD for TN in an elderly patient, considered to be caused by lateral positioning during and after the surgery. A 79-year-old female underwent MVD for right TN in the left lateral decubitus position, and TN disappeared after the surgery. Postoperatively, the patient tended to maintain the left lateral decubitus position to prevent wound contact w

Pharmacologic management of movement disorder after midbrain haemorrhage.

Movement disorders following midbrain haemorrhage are infrequently encountered in rehabilitation, and are uncommonly corrected by pharmacologic means. This report describes a 20 year-old male with a prior history of cocaine abuse who presented with a 4 day history of dysarthria and blurred vision following methamphetamine abuse. Physical examination demonstrated hypertension, left facial hemispasm, bilateral upward gaze paresis and ataxic gait. Magnetic resonance imaging/magnetic resonance angiography (MRI/MRA) showed multifocal parenchymal haematomas in the mesencephalic tegmentum, subcortica

Effect of manipulation of the GABA system on dopamine-related behaviors.

The interaction between GABAergic and dopaminergic systems within the central nervous system was investigated in rats using the open-field apparatus and apomorphine-induced stereotypy, and in mice using haloperidol-induced catalepsy. The single intraperitoneal administration of baclofen 3.0 mg/kg, 4,5,6,7-tetrahydroisoxasolo-(5,4-c) piridin-3-ol (THIP) 10.0 mg/kg and picrotoxin 2.0 mg/kg decreased both ambulation and rearing frequencies of the rats in the open-field; only the GABA agonists increased the duration of animal immobility. THIP (10.0 mg/kg) increased the duration of haloperidol-indu

Reliability and validity of the German translation of the de Morton Mobility Index (DEMMI) performed by physiotherapists in patients admitted to a sub-acute inpatient geriatric rehabilitation hospital.

BACKGROUND: Mobility is a key outcome in geriatric rehabilitation. The de Morton Mobility Index (DEMMI) is an internationally well-established, unidimensional measure of mobility with good psychometric properties. The aim of this study was to examine the reliability and construct validity of the German translation of the DEMMI in geriatric inpatients. METHODS: This cross-sectional study included patients admitted to a sub-acute inpatient geriatric rehabilitation hospital (reliability sample: N = 33; validity sample: N = 107). Reliability, validity, and unidimensionality were investigated. RE

Computed Tomographic Assessment of Body Fat in Dachshunds: A Pilot Study.

Obesity has not been linked to causing intervertebral disc degeneration, but has been shown to influence time to ambulation, a strong long-term prognostic indicator in dogs with intervertebral disc disease. However, monitoring obesity to date is imprecise and subjective in the clinical setting. Having an objective formula based on morphometric measurements would potentially be more precise to track our patients' weights. Dogs have been shown to gain weight along their lumbar spine more rapidly than other areas. Varying body conformations make extrapolation from nonchondrodystrophic dogs to Dac
