[Upper airway morphology in Down Syndrome patients under dexmedetomidine sedation].

BACKGROUND AND OBJECTIVES: Children with Down Syndrome are vulnerable to significant upper airway obstruction due to relative macroglossia and dynamic airway collapse. The objective of this study was to compare the upper airway dimensions of children with Down Syndrome and obstructive sleep apnea with normal airway under dexmedetomidine sedation. METHODS: IRB approval was obtained. In this retrospective study, clinically indicated dynamic sagittal midline magnetic resonance images of the upper airway were obtained under low (1mcg/kg/h) and high (3mcg/kg/h) dose dexmedetomidine. Airway anterop…

Low-dose intramuscular dexmedetomidine as premedication: a randomized controlled trial.

BACKGROUND: Dexmedetomidine-induced bradycardia or hypotension has recently attracted considerable attention because of potentially grave consequences, including sinus arrest and refractory cardiogenic shock. A route other than intravenous injection or a low dose may help minimize cardiovascular risks associated with dexmedetomidine. However, few studies have addressed the clinical effects of low-dose intramuscular dexmedetomidine as premedication. MATERIAL AND METHODS: Forty American Society of Anesthesiologists physical status I adult patients undergoing suspension laryngoscopic surgery wer…

Dexmedetomidine controls supraventricular tachycardia following cardiac surgery in a child.

Dexmedetomidine is an α2-adrenergic agonist which initially received US Food and Drug Administration (FDA) approval in the United States in 1999 for the sedation of adults during mechanical ventilation and then in 2009 for monitored anesthesia care. Although generally viewed as an adverse effect, bradycardia and the negative chronotropic effects may be beneficial in certain patient populations and have occasionally been used as a therapeutic maneuver. We present a case summary describing intraoperative and postoperative use of dexmedetomidine to treat and control supraventricular tachycardia i…

Dexmedetomidine premedication attenuates ketamine-induced cardiostimulatory effects and postanesthetic delirium.

BACKGROUND: Dexmedetomidine is a new potent and highly selective alpha 2-adrenoceptor agonist with sedative-hypnotic and anesthetic sparing properties. Because of its sympathoinhibitory activity, it may prove useful in balancing the cardiostimulatory effects and attenuating the adverse central nervous system effects of ketamine. METHODS: A double-blind, randomized and comparative parallel-group study design was employed in 40 volunteers with ASA physical status 1 who were scheduled for elective superficial surgery under ketamine anesthesia. Dexmedetomidine (2.5 micrograms/kg, n = 20) or midaz…

Propofol-dexmedetomidine versus propofol-ketamine for endoscopic retrograde cholangiopancreatography sedation.

Deep sedation during elective endoscopic retrograde cholangiopancreatography (ERCP) ensures immobility, success and stability in prone positions, yet propofol monotherapy risks hypotension, bradycardia and respiratory depression. Therefore, it is of interest to compare propofol-dexmedetomidine (PD) versus propofol-ketamine (PK) in 120 ASA I-III adults undergoing ERCP. PD provided superior hemodynamic stability (MAP fluctuation 7.9±3.1% vs. 13.8±4.7%, p=0.001), reduced propofol needs (178±46 mg vs. 241±59 mg, p=0.001) and faster recovery (11.8±3.4 vs. 17.2±5.1 min, p=0.001). Endoscopists rated …

Dexmedetomidine for LISA procedure: a prospective observational single center experience.

BACKGROUND: Respiratory Distress Syndrome (RDS) is the most common respiratory problem of preterm newborns. The preferred way to manage RDS is with Less Invasive Surfactant Administration (LISA), which involves direct laryngoscopy in spontaneously breathing infants. Despite its widespread diffusion, the optimal sedation protocol for LISA remains unclear. Dexmedetomidine, an α2-adrenergic agonist, offers sedative and analgesic properties with minimal respiratory depression. This pilot study aims to evaluate dexmedetomidine for LISA procedure focusing on the frequency of adequate sedation (defin…

Photothermal-responsive dexmedetomidine-loaded MPDA-GelMA hydrogel promotes bone regeneration.

The clinical demands for repairing bone defects have not been fully met, as conventional treatments are often limited by drawbacks such as burst drug release and immune rejection. The development of smart biomaterials enabling localized and controlled drug release represents a promising strategy to address these challenges. In this study, a photothermal-responsive hydrogel was fabricated by incorporating dexmedetomidine-loaded mesoporous polydopamine nanoparticles (DEX@MPDA) into gelatin methacryloyl (GelMA). The resulting composite hydrogel exhibited excellent shape adaptability and achieved …

Does dexmedetomidine induce bone regeneration in cranial defects in rabbits?

Dexmedetomidine has been shown to exert protective and curative effects on various tissues and organs in different pathological processes. This study aimed to investigate the effect of dexmedetomidine on the regeneration process after making holes in the parietal bones of rabbits. Twenty-four male Oryctolagus cuniculus rabbits were allocated to three groups, and an 8-mm circular parietal critical-sized bone defect was induced in each animal. Group_C (control) received saline; Group_LD (low dose) was given dexmedetomidine 2.75 µg/kg; Group_HD (high dose), dexmedetomidine 5.5 µg/kg; all were adm…

Dexmedetomidine Stimulates Cellular Senescence of Lung Cancer Cells Via NOX5.

Dexmedetomidine is a highly selective α- 2 adrenergic receptor agonist which has been considered as a promising anticancer agent. However, the underlying molecular mechanisms whereby Dexmedetomidine exerts its anticancer and chemopreventive actions need to be elucidated. In this study, we found that Dexmedetomidine reduced cell viability of the lung cancer cell line A549 cells in a dose-dependent manner. Importantly, Dexmedetomidine reduced the telomerase activity in A549 cells by reducing the gene levels of hTERT but increasing the gene levels of TERF2. The senescence- associated-β- galactosi…

Propofol and Dexmedetomidine Ameliorate Endotoxemia-Associated Encephalopathy via Inhibiting Ferroptosis.

BACKGROUND: Sepsis is recognized as a multiorgan and systemic damage caused by dysregulated host response to infection. Its acute systemic inflammatory response highly resembles that of lipopolysaccharide (LPS)-induced endotoxemia. Propofol and dexmedetomidine are two commonly used sedatives for mechanical ventilation in critically ill patients and have been reported to alleviate cognitive impairment in many diseases. In this study, we aimed to explore and compare the effects of propofol and dexmedetomidine on the encephalopathy induced by endotoxemia and to investigate whether ferroptosis is …

Fighting ferroptosis: Protective effects of dexmedetomidine on vital organ injuries.

Vital organ injury is one of the leading causes of global mortality and socio-economic burdens. Current treatments have limited efficacy, and new strategies are needed. Dexmedetomidine (DEX) is a highly selective α2-adrenergic receptor that protects multiple organs by reducing inflammation and preventing cell death. However, its exact mechanism is not yet fully understood. Understanding the underlying molecular mechanisms of its protective effects is crucial as it could provide a basis for designing highly targeted and more effective drugs. Ferroptosis is the primary mode of cell death during …

Dexmedetomidine directly binds to and inhibits Toll-like receptor 4.

BACKGROUND: While a number of anesthetics has been shown potentially associated with neurotoxicity in the developing brain, dexmedetomidine, a drug that was rather recently introduced into the perioperative setting, is considered beneficial from neurological wellbeing. However, the underlying mechanism of how dexmedetomidine affects brain health remains to be determined. Based on our recent study, we hypothesized that dexmedetomidine would directly bind to and inhibit Toll-like receptor 4 (TLR4), a critical receptor largely expressed in microglia and responsible for neurological insult. METHO…

Dexmedetomidine suppresses glucose-stimulated insulin secretion in pancreatic β-cells.

Proper glycemic control is crucial for patient management in critical care, including perioperative care, and can influence patient prognosis. Blood glucose concentration determines insulin secretion and sensitivity and affects the intricate balance between the glucose metabolism. Human and other animal studies have demonstrated that perioperative drugs, including volatile anesthetics and intravenous anesthetics, affect glucose-stimulated insulin secretion (GSIS). Dexmedetomidine (DEX) decreases insulin release and affects glucose metabolism; however, the specific mechanism underlying this phe…

Persistent Paradoxical Reaction to Midazolam despite General Anesthesia with Dexmedetomidine.

Midazolam is a widely used benzodiazepine due to its rapid onset of action and relatively safe side effect profile. It is used for sedation, anxiolysis, and induction of general anesthesia. However, in <1% of instances, it may cause a paradoxical excitement: agitation, restlessness, myoclonus, stiffening of the limbs, and aggression. We report a case report in which preoperative administration of midazolam caused onset of the aforementioned symptoms that were not attenuated by general anesthesia with dexmedetomidine. This case report aims to create awareness about the rare adverse reactions of…
