The crop of adult Phormia regina consists of a duct that diverges from the esophagus, just in front of the cardia, and extends ventrally and posteriorly into the thorax and abdomen where it forms a bilobed sac. Flattened epithelial cells produce the cuticular lining of the crop. When empty, or partially full, the epithelial cells and cuticular lining form folds extending into the lumen, thus providing for expansion as the crop sac fills. Covering the sac on the hemolymph side is a layer of anastomosed, intrinsic muscles connected to one another by intercellular cytoplasmic bridges. Mitochondri

BACKGROUND: Surgical treatment of complicated colonic diverticular disease is still debatable. The aim of this prospective study was to evaluate the outcome of laparoscopic sigmoid colectomy in patients with diverticulitis. Patients offered laparoscopic surgery presented with acute complicated diverticulitis (Hinchey type I, II, III), chronically recurrent diverticulitis, bleeding, or sigmoid stenosis caused by chronic diverticulitis.
METHOD: All patients who underwent laparoscopic colectomy within a 12-year period were prospectively entered into a database registry. One-stage laparoscopic re…

Appendiceal diverticulitis is a rare presentation, often presenting outside of the classical age grouping for appendicitis. We describe a case of appendiceal diverticulitis and issue a cautionary note in the management of patients with atypical features of right lower quadrant pain. In light of its clinical course the finding of a non-inflamed appendiceal diverticulum should prompt one to consider incidental appendicectomy. We also review the literature on appendiceal diverticulitis.

Sonography is the primary diagnostic method in the painful left lower quadrant. Experienced investigators can diagnose an acute diverticulitis in more than 90%. In unclear cases or when complications are suspected, CT should be performed. Sonography can diagnose most differential diagnosis of the painful left lower quadrant as colitis, appenagitis or haematomas.

Colonic diverticulitis occurs when diverticula within the colon become infected or inflamed. It is becoming an increasingly common cause for hospital admission, particularly in western society, where it is linked to a low fibre diet. Symptoms of diverticulitis include abdominal pain, diarrhoea and pyrexia, however, symptoms are often non-specific and the clinical diagnosis may be difficult. In addition, elderly patients and those taking corticosteroids may have limited findings on physical examination, even in the presence of severe diverticulitis. A high index of suspicion is required in such…

OBJECTIVE: We know of only one previous study that has described the sonographic appearances of the inflamed Meckel's diverticulum in several cases (nine patients). Our study reviews sonographic, clinical, and pathologic findings in 10 patients with an inflamed Meckel's diverticulum. We also describe unique findings on color Doppler sonography for two of our patients.
CONCLUSION: Sonographic findings of an inflamed Meckel's diverticulum may mimic findings for acute appendicitis or intestinal duplication. In patients with rectal bleeding due to diverticulitis, the visualization of a tubular hy…

Meckel's diverticulitis is a rare disease. In addition to physical examination, abdominal ultrasound can help to pinpoint the diagnosis. By presenting a case report we would like to demonstrate the typical ultrasonographic findings in acute Meckel's diverticulitis and differentiate it from acute appendicitis. A 60-year-old patient was admitted to our hospital with the diagnosis of acute appendicitis. Abdominal ultrasound was performed and a blind ending, liquid-filled segment of small bowel in the right lower quadrant of the abdomen found. This segment was not compressible, no peristalsis was …

PURPOSE: To study the ranking of CT in the diagnosis of sigma diverticulitis. Comparison of results of the colon monocontrast enema with those obtained via CT and histopathology as gold standard.
MATERIAL AND METHODS: 32 patients were included who were operated on for sigma diverticulitis. In 30 patients both CT and monocontrast enema were performed and in two patients CT only.
RESULTS: Acute inflammatory wall alterations are identified by CT with a sensitivity of 89.7% vs 96.3% via contrast enema. Pericolic reactions of the environment were seen via CT with 89.7% sensitivity. In case of cov…

UNLABELLED: Since the beginning of 1993 until May 1994 in our hospital 25 patients were operated upon for diverticular disease. 16 patients required a laparoscopic assisted elective one-stage resection. One laparoscopic Hartmann's procedure was performed on account of a free perforation. 4 patients underwent laparoscopic reconstruction of the intestinal continuity after an Hartmann's resection. In 4 cases it was necessary to change to the conventional procedure.
TECHNIQUE: the patient is placed in dorso-sacral position (modified lithotomy position) in order to allow transanal colonic access.

In acute diverticulitis of the sigmoid colon, the initial therapeutic decision is wether to employ medical or surgical treatment and, thereafter, wether elective resection of the involved bowel is needed. According to the criteria used for the diagnosis of diverticulitis and to the length of the medical follow-up 10 to 25 per cent of patients with diverticulosis will develop some form of peridiverticular inflammation. Adequate treatment of diverticulitis requires antimicrobial therapy directed against both facultative and obligate anaerobic gram-negative bacteria. Computed tomography is now wi…

In emergency, the most commonly used examination, other than plain radiographs of the abdomen, is the water-soluble contrast enema (Gastrografin). It demonstrates three different pictures, which may be more or less associated one to the other: "peridiverticulitis", featuring serrate lesions, abnormal stiffness and fixity and, in some cases, a long narrowing or stricture of the colon; diverticula, mainly in the sigmoid colon, becoming sharp-pointed or spark-liked; spillage of contrast material out of the colic lumen or into a neighbouring organ (fistula). Ultrasonography may be a useful emergen…

Two extraordinary cases of perforated diverticulitis with extraperitoneal presentation are described. The diagnosis should be kept in mind in unusual cases of pain or sepsis in the elderly. A brief review of pathogenesis is given. Surgical treatment with primary resection and drainage is emphasized.

Twelve cases are presented of perforated diverticulitis that required emergency surgical intervention. The present guidelines for surgical action are indicated for the different situations originated by perforated diverticulitis: localized peritonitis (pericolonic abscess), suppurated peritonitis and fecaloid peritonitis. The Hartman technique is explained as the method of choice for patients with generalized peritonitis.

Acute colonic diverticulitis is mainly diagnosed clinically, as endoscopy or coloncontrast enema might be dangerous. In this study diagnostic criteria and typical sonographic findings of acute diverticulitis are summarized. The comparison of these findings - in stadium IIa of diverticulitis - to other sonographic colon findings shows high sensitivity (0.96), specificity (0.98) and predictive values (pV +0.85, pV -0.99) for sonographic findings of acute colonic diverticulitis. Thus sonography can improve diagnostic and follow-up controls of acute diverticulitis.

Recent reports on the CT evaluation of patients with diverticulitis of the left colon prompted a retrospective review of the CT findings in six patients with diverticulitis of the cecum and ascending colon. All had pain and tenderness in the right lower quadrant. In each case, CT findings showed regional thickening of the colonic wall, an extraluminal mass involving the cecum and/or ascending colon, haziness and linear strands in adjacent pericolic fat, and thickening of nearby fascial planes. Extraluminal air was noted in four patients, and contrast-filled structures suggesting diverticula we…

A high percentage of people in the civilized Western world develops colonic diverticula. Signs of illness are coming from inflammation around the diverticula. Resection therapy is indicated after several inflammation episodes. The operation has often to be performed in case of emergency: Ileus, bleeding, perforation. We recommend the primary resection of the sigmoid colon with an transverse colostomy or the Hartmann operation even in case of perforation or peritonitis. We operated 140 patients. The mortality rate in our hospital was 12% of the elective and 27% of the emergency operations.
