BACKGROUND: Pharyngoesophageal diverticula have many subtypes, with Zenker's diverticulum being the most common. First described in 1983, a Killian-Jamieson diverticulum is an outpouching in the anterolateral wall at the pharyngoesophageal junction. This is located inferiorly to the cricopharyngeus muscle, unlike Zenker's diverticula which occur superiorly. Killian-Jamieson diverticula are rare and are commonly misdiagnosed as Zenker's diverticula. Less than 30 reports of Killian-Jamieson diverticula have been described in the literature.

Meckel's diverticulum (MD) is the most common gastro-intestinal congenital malformation (approximately 2% in the overall population). The lifetime risk of related complications is estimated at 4%. These include gastro-intestinal bleeding, obstruction or diverticular inflammation. Meckel's diverticulum (MD) is a persistent embryonic remnant occurring in 2% of the general population. This entity is usually clinically silent and is only discovered when complications occur. Diagnosis is rarely made preoperatively. Surgical resection of symptomatic MD is widely accepted but the most appropriate management of asymptomatic MD remains unclear. The giant diverticulum of the colon is a rare entity first diagnosed by Bouvin and Bonte in 1946. Few cases have been reported in the literature. It is normally located in the antimesenteric border of the sigmoid colon. In most cases it is considered to be an uncommon complication of a common disease: colonic diverticulosis. We present a case of giant diverticulum of the sigma diagnosed in an 80-year-old man and we describe the plain-film and CT findings.

A young male who had minimal dysphagia since childhood complained of increasing difficulty in swallowing for a few months. Upper Gastrointestinal endoscopy was normal on two occasions done by two gastroenterologists. Barium swallow showed minimal extrinsic pressure on the oesophageal wall. X-ray chest was normal. CT scan showed a large growth close to the oesophagus. Resection of the growth showed a carcinoma completely filling an oesophageal diverticulum with a normal oesophageal lumen.

Twenty one surgically documented cases of Meckel's diverticulum in children treated at Kasturba Hospital, Manipal since 1981 are reviewed. Of them, 14 presented with intestinal obstruction, 2 with pain abdomen, 2 with Littre's hernia and 2 were found incidentally. Only one patient presented with gastrointestinal bleeding. The clinical profile of these patients is analyzed.

Esophageal diverticula are rarely found at the terminal portion, where they are called supradiaphragmatic or epiphrenic diverticula and occur in association with motility disorders of the terminal esophagus. We present here two cases of epiphrenic esophageal diverticulum, one of which was treated surgically.

For a period of 12 years (1977-1988) 64 children with Meckel's diverticulum (32 boys and 32 girls) have been operated. In early childhood (younger than 3 years) were 14.1 per cent; most of the others (59.4 per cent) were in school age. The development of a carcinoma in a hypopharyngeal diverticulum is reported as a rare complication in a 74-year-old female. The carcinoma was neither radiologically nor endoscopically detected. Sixteen cases of Meckel's diverticulum are reported, 14 of them detected during emergency operative intervention for acute surgical abdomen and 2 during planned operation. The complicated forms of Meckel's diverticulum were 10: acute diverticulitis 4 (25 per cent), intestinal obstruction 4 (25 per cent), neoplastic process 1 (6.25 per cent), intestinal hemorrhage 1 (6.25 per cent). Thirty-three Meckel's diverticula were found intraoperatively and 31 were removed for various indications. One patient died. The management was justified in the light of own experiences and literature data on the pathological changes in the diverticulum and complications caused by them. Even large and broad-based diverticula should be removed in emergency situations, if no special or temporary contraindications are present.
